[Cerebellar ataxia with hypogonadotropic hypogonadism and mental disturbance].

Cerebellar ataxia with hypogonadotropic hypogonadism is a rare condition. Both hypothalamic LHRH and pituitary gonadotropin deficiency may appear with cerebellar ataxia. In this report, a 19-year-old man who presented with progressive cerebellar ataxia and mental disturbance was found to have low plasma gonadotropin levels. No rise in gonadotropin was demonstrated after repeated stimulation with LHRH. Our findings suggested that the cause of hypogonadism was pituitary gonadotropin deficiency, or both hypothalamic and pituitary dysfunction. The endocrinological data and clinical features of our…

[Periodic alternating nystagmus in familial congenital cerebellar ataxia].

We noted periodic alternating nystagmus (PAN) in two patients (mother and daughter) who were diagnosed as familial congenital cerebellar ataxia. The mother was 51-year-old and her PAN had about 4 minutes cycle. During eye closure there was a deviation of arms and trunk parallel to the direction of the slow phase of nystagmus. Vestibular stimulation during rotatory and caloric tests disclosed enhanced vestibulo-ocular reflexes, changes of direction and prolonged cycle of PAN. The patient had no visual suppression during caloric stimulation. The daughter was 14-year-old and although she had no s…

Variants of Nijmegen breakage syndrome and ataxia telangiectasia.

Ataxia telangiectasia (AT) is an autosomal recessive disorder characterized by telangiectasia, progressive ataxia, sinopulmonary infections and a combined immunodeficiency (usually consisting of IgA deficiency, IgE deficiency, IgG2 and IgG4 deficiency and a disturbed T cell immunity). The alpha-fetoprotein level is elevated. Cytogenetic studies reveal a very specific chromosome instability with multiple chromosome 7 and/or 14 rearrangements (preferential breakpoints 14q32, 14q12, 7q35 and 7p12). X-ray hypersensitivity is one of the hallmarks of the disease. Nijmegen Breakage Syndrome (NBS), an…

Magnetic resonance imaging in hereditary and idiopathic ataxia.

We used magnetic resonance imaging (MRI) to study brain and spinal cord morphology in hereditary and idiopathic ataxia. Our interest was in whether the classical neuropathologic categories--cerebellar cortical atrophy (CCA), olivopontocerebellar atrophy (OPCA), and spinal atrophy (SA)--could be identified in vivo and which clinical phenotype corresponded to which morphologic category. To this end, we measured the size of the cerebellar vermis, cerebellar hemispheres, fourth ventricle, middle cerebellar peduncles, basis pontis, medulla oblongata, and cervical spinal cord on T1-weighted images o…

High spontaneous intrachromosomal recombination rates in ataxia-telangiectasia.

Ataxia-telangiectasia (A-T) is an inherited human disease associated with neurologic degeneration, immune dysfunction, and high cancer risk. It has been proposed that the underlying abnormality in A-T is a defect in genetic recombination that interferes with immune gene rearrangements and the repair of DNA damage. Recombination was studied in A-T and control human fibroblast lines by means of two recombination vectors. Unexpectedly, spontaneous intrachromosomal recombination rates were 30 to 200 times higher in A-T fibroblast lines than in normal cells, whereas extrachromosomal recombination f…

Cutaneous granulomatous lesions in patients with ataxia-telangiectasia.

Ataxia-telangiectasia is a genetic syndrome with progressive cerebellar ataxia, oculocutaneous telangiectasias and other skin manifestations, variable immune system defects, chronic progressive sinopulmonary disease, and a high incidence of cancer. Cutaneous granulomas developed and persisted in eight patients with ataxia-telangiectasia, despite treatment with intravenously administered immune globulin, topical antibiotic therapy, and potent topical corticosteroid therapy. We were unable to identify an infectious agent; the granulomas may develop in an attempt to localize antigen in the presen…

[Ataxia telangiectasia and characterization of its immunological disorders].

Ataxia telangiectasia (AT) is a primary immunodeficiency syndrome characterized by cerebellar ataxia, extrapyramidal signs, oculocutaneous telangiectasia, recurrent respiratory infections and development of malignancies. AT is a complex autosomal recessive disorder involving several systems other than lymphoid cells or the central nervous system. Such a diversity of abnormalities includes hypersensitivity of fibroblasts and lymphocytes to ionizing radiation (anomaly of DNA repair), non-random chromosomal rearrangements in lymphocytes, elevated serum level of alpha-fetoprotein, premature aging …

Early-onset cerebellar ataxia with retained tendon reflexes.

The authors report a clinical review of 16 childhood cases with early-onset cerebellar ataxia with retained tendon reflexes. The preservation of tendon reflexes distinguishes this disorder from Friedreich's ataxia. The mean age of onset of symptoms was 7.1 years. The main presenting symptom was abnormal gait (100%). Ataxia of gait and limbs and normal or increased tendon reflexes were found in all cases. This disorder is associated with dysarthria, pyramidal signs in the limbs, and in some instances, sensory loss. Other important differences from Friedreich's ataxia are absence of optic atroph…

Hypersensitivity of cultured ataxia-telangiectasia cells to etoposide.

Fibroblasts from patients with ataxia-telangiectasia (A-T) were found to be hypersensitive to killing by the antineoplastic agent etoposide. The A-T fibroblast strains GM5823, GM367, and GM2052 were twofold to threefold more sensitive to killing by etoposide than fibroblasts from normal controls (AG1521, AG1522, and IMR90). A simian virus 40 (SV40)-transformed, immortal human fibroblast line (GM5849) derived from the A-T cell line GM5823 was also studied. GM5849 retained the unusual sensitivity of nontransformed A-T fibroblast lines to x-irradiation, bleomycin, and neocarzinostatin (zinostatin…

Studies on lymphocyte cell surface in ataxia-telangiectasia.

Lymphocyte surface proteins of patients with ataxia-telangiectasia were separated by polyacrylamide gel electrophoresis and compared by autoradiography. The patients lacked one of the two main bands (Band I at the origin). The second main band (Band II) was absent in some cases. All patients had one or two additional bands of smaller molecular weight than Band II except one case who had no band detectable. In the patients, alkaline phosphatase, total ATPase and Mg2+. ATPase were increased but 5'-nucleotidase was normal. The results suggest abnormality in the plasma membranes of the patients' l…

Autosomal recessive spino-olivo-cerebellar degeneration without ataxia.

Five adult siblings from a sibship of ten suffering from an external ophthalmoplegia with a spastic paraplegia are reported. In addition, optic nerve atrophy was present in three of the patients and dementia in two; extrapyramidal signs and cerebellar ataxia were found only in one patient. Contrary to earlier studies of patients with comparable neurological signs the pattern of inheritance was autosomal recessive. Neuropathological investigation of the index case, who had never shown ataxia, nevertheless showed demyelination of the spinocerebellar and the olivocerebellar pathways, and also a s…

Familial cerebellar ataxia with hydrocephalus in bull mastiffs.

A family of bull mastiff pups in which 16 animals developed certain neurological signs is described. It is probable that the disease is inherited in an autosomal recessive manner. Clinical and pathological examination of six animals revealed ataxia, behavioural abnormalities and a visual defect associated with symmetrical lesions in the cerebellar nuclei, lateral vestibular nucleus and inferior colliculus together with a communicating hydrocephalus. The cerebellar lesions consisted of vacuolation, gliosis and axonal degeneration. Evidence for both axonal and oligodendroglial abnormalities was …

[Irradiation sensitivity of cells heterozygote for ataxia telangiectasia].

The frequency of chromosome aberrations after in vitro exposure to different radiation doses in lymphocyte cultures of heterozygotes according to ataxia-teleangiectasia (AT) was compared to that in control donors. A increased frequency of dicentrics and rings of heterozygotic cells according to AT was observed upon exposure to relatively low radiation doses (40 and 80 rad). Meanwhile the increased dose (160 rad) brought about no differences. No differences were found in the frequency of cells with chromosome aberrations.

Ataxia and hearing loss secondary to perilymphatic fistula.

Ataxia is rarely attributed to lesions of the peripheral vestibular system. In 1973, the first case of ataxia and hearing loss secondary to a labyrinthine fistula was reported. Until now, this syndrome has not been reported in patients under the age of 10 years. A case is presented of a 5-year-old boy with symptoms of ataxia and hearing loss as well as vertigo and tinnitus after head trauma. Three physical findings appear to be most characteristic of patients with perilymphatic fistulas: a positive fistula response, positive positional testing with the involved ear down, and evidence of vestib…

Ataxia-telangiectasia with ovarian gonadoblastoma and contralateral dysgerminoma.

Although neoplasms are unusually frequent in patients with ataxia-telangiectasia, the occurrence of primary tumors of the ovary in such patients is exceedingly rare. This report describes a 17-year-old phenotypic female with ataxia-telangiectasia, who was found to harbor an ovarian gonadoblastoma and a contralateral dysgerminoma. The latter tumor has occurred in only one other patient with ataxia-telangiectasia, while an association with gonadoblastoma has never been documented previously. Additional unusual features rarely encountered in patients with gonadoblastoma included origin of the tum…

Contrastive Learning Model for Wearable-Based Ataxia Assessment.

OBJECTIVE: Frequent and objective assessment of ataxia severity is essential for tracking disease progression and evaluating the effectiveness of potential treatments. Wearable-based assessments have emerged as a promising solution. However, existing methods rely on inertial data features directly correlated with subjective and coarse clinician-evaluated rating scales, which serve as imperfect gold standards. This approach may introduce biases and restrict flexibility in feature design. To address these limitations, this study introduces a novel contrastive learning-based model that leverages …
