Cryoablation of the right anteroseptal or midseptal accessory pathways in children: A 2-year single-center experience.

BACKGROUND: Radiofrequency catheter ablation (RFCA) in the septum close to the atrioventricular node or His bundle has an increased risk of irreversible complications. Cryothermal energy has the advantages of reversible cryomapping and increased catheter stability. The study aims to evaluate the efficacy and safety of cryoablation of the right anteroseptal or midseptal accessory pathways (APs) in pediatric patients. METHOD: A retrospective review was performed with 26 pediatric patients (16 males and 10 females; median age 6.0 years and average body weight 24.9 kg) with paroxysmal supraventri…

Infraatrial supraventricular tachycardias: mechanisms, diagnosis, and management.

Tachycardias are traditionally classified as either ventricular tachycardia (VT) or supraventricular tachycardia (SVT). VT can be defined as a tachycardia which requires only ventricular structures for perpetuation. SVT is defined in terms of exclusion of VT and hence is any tachycardia which requires participation of at least one supraventricular structure for perpetuation. Certain SVTs require only participation of the atrioventricular node (AVN) and the His bundle (HB) but not the atrial myocardium or any of the great thoracic veins for perpetuation and hence can be described as "infraatria…

Acute electrophysiological effects of flecainide acetate on cardiac conduction and refractoriness in man.

The electrophysiological effects of flecainide acetate (2 mg/kg as an intravenous infusion over five minutes) were assessed in 47 patients u … gnificant only at ventricular level. No significant change occurred in refractoriness of the normal atrioventricular node. Pronounced prolongation of retrograde "fast" AH pathway refractoriness was observed in those patients with dual AH pathways. Anterograde and retrograde accessory pathway refractoriness were both greatly increased. These electrophysiological propert …

[Transcatheter radiofrequency ablation in supraventricular tachycardia in children: immediate results and mid-term follow-up].

OBJECTIVES: The purpose of this study was to evaluate the efficacy and safety of radiofrequency ablation of supraventricular tachycardias due to a reentry circuit in a group of closely followed-up pediatric patients. PATIENTS: Forty-six pediatric patients (mean age 12.6 years, range 3-18) underwent a total of 48 radiofrequency ablation procedures for reciprocating supraventricular tachycardia. Thirty-nine had an orthodromic reentry tachycardia, 6 an atrioventricular node reentry tachycardia and 1 a permanent junctional reciprocating tachycardia. METHODS: To ablate the overt accessory pathway…

Notch signaling regulates murine atrioventricular conduction and the formation of accessory pathways.

Ventricular preexcitation, which characterizes Wolff-Parkinson-White syndrome, is caused by the presence of accessory pathways that can rapidly conduct electrical impulses from atria to ventricles, without the intrinsic delay characteristic of the atrioventricular (AV) node. Preexcitation is associated with an increased risk of tachyarrhythmia, palpitations, syncope, and sudden death. Although the pathology and electrophysiology of preexcitation syndromes are well characterized, the developmental mechanisms are poorly understood, and few animal models that faithfully recapitulate the human dis

Invasive measurements of atrioventricular conduction parameters prior to and following ventricular septal defect closure with the amplatzer device.

BACKGROUND: Because of anatomical proximity, closure of paramembranous ventricular septal defect (pm-VSD) with an Amplatzer occluder has the potential to affect the atrioventricular (AV) conduction system. PATIENTS AND METHODS: Electrophysiology studies (EPS) were performed prior to and following closure of the pm-VSD in 19 children aged 8.9 +/- 4.5 years. Data from electrocardiograms, Holter monitoring and exercise tests before intervention and at follow up (1.9 +/- 0.8 years) were reviewed. RESULTS: There were no EPS catheter-induced complications. Incidence of total conduction anomalies w

Atrioventricular nodal reentry. Clinical, electrophysiological, and therapeutic considerations.

BACKGROUND: Atrioventricular (AV) nodal reentry is a relatively common cause of regular, narrow QRS tachycardia. The underlying basis for this arrhythmia is functional (and anatomic) duality of pathways in the region of the AV node, although the exact boundaries of the reentrant circuit have not been convincingly defined. During the more common type of AV nodal reentry (seen in approximately 90% of cases), a slow conducting pathway is used in the anterograde direction, and a fast pathway is operative in the retrograde direction. In the uncommon form, the direction of impulse propagation within

Antiarrhythmic actions of diltiazem during experimental atrioventricular reentrant tachycardias. Importance of use-dependent calcium channel-blocking properties.

The purpose of this study was to determine if the known frequency-dependent effects of diltiazem on inward calcium current result in selecti … dependent effect prevented AVRT when RP/RT became greater than unity. In conclusion, rate-dependent atrioventricular node depression by diltiazem results in greater tachycardia slowing and higher rates of termination during atrioventricular reentrant tachycardias with faster initial rates and shorter retrograde conduction intervals.

The effect of fascicular block on ventriculoatrial conduction during AV reentrant tachycardia.

Two patients with Wolff-Parkinson-White syndrome and surgically mapped anterior left free wall atrioventricular bypass tracts had orthodromic atrioventricular reentry tachycardia conducted with complete left bundle branch block (CLBBB), complete right bundle block (CRBBB), left anterior fascicular block (LAFB), and a narrow QRS. Ventriculoatrial conduction increased by 35 and 85 ms with CLBBB compatible with the left free wall location of the bypass tracts. In one patient, resolution of CLBBB occurred in two stages. Initially, left posterior fascicular block (LPFB) resolved, decreasing VA cond

Atrial pacing for sick sinus syndrome.

Atrial pacing is the most physiological way to pace patients with sinus node disease, as it provides both AV synchrony and a normal ventricular activation pattern. Long-term studies comparing atrial and ventricular pacing imply that atrial pacing results in fewer cardiac complications and, possibly, reduced mortality. Ventricular pacing should thus, if possible, be avoided in patients with sinus node disease. The potential risk of impending high-grade AV block during atrial pacing is low, with an annual incidence around 1% if patients are selected appropriately. Approximately 40-50% of patient

Role of endogenous adenosine in postdefibrillation bradyarrhythmia and hemodynamic depression.

In halothane-nitrous oxide-anesthetized pigs, the effect of the competitive adenosine antagonist, BW-A1433U (a derivative of 1,3-dipropyl-8-phenylxanthine), on postdefibrillation bradyarrhythmia and hemodynamic depression was investigated. In protocol 1, repetitive episodes of ventricular fibrillation lasting 15 seconds before transthoracic DC shock were performed in five animals, before (control) and after the administration of BW-A1433U (5 mg/kg i.v.). An unsuccessful initial shock was immediately followed by a rescue shock of 40 A. In ventricular fibrillation episodes requiring rescue shock

Complete atrioventricular block as an initial manifestation of aortic dissection: A case report.

The right coronary artery involvement occurs in 1-2% of aortic dissection and may cause atrioventricular (AV) block due to obstruction of blood supply to the AV node. A 59-year-old man with a history of hypertension presented to the emergency department with chest pain and complete AV block. After the diagnostic approach, aortic dissection Stanford A was diagnosed. Aortic dissection must be suspected in hypertensive patients who present with AV block and chest pain.

Termination of a long RP supraventricular tachycardia by carotid sinus massage.

Carotid sinus massage is a useful diagnostic and therapeutic maneuver in patients with supraventricular tachycardia. Abrupt tachycardia termination with carotid sinus massage typically indicates a mechanism dependent on the atrioventricular node. Here, we present the case of a 79-year-old patient presenting with a long-RP supraventricular tachycardia terminated with carotid sinus massage that was proven invasively to be due to a focal atrial tachycardia. We discuss mechanistic explanations for this unusual phenomenon and review the literature.

Radiofrequency catheter ablation of the AV node to improve the function of an antitachycardia implantable defibrillator.

A case of coexisting atrial fibrillation and ventricular tachycardia in a patient with an implantable cardioverter defibrillator is described. Despite careful reprogramming, the device was not always able to distinguish between the two arrhythmias and continued to deliver inappropriate antitachycardia therapy including DC shocks. Attempts to pharmacologically control the atrial fibrillation were unsuccessful so radiofrequency ablation of the atrioventricular node was performed. Following successful ablation, there have been no further false detections no episodes of further ventricular tachyca…

Anatomy of congenital complete heart block and relation to maternal anti-Ro antibodies.

The anatomy of the conduction system was studied in 8 patients who had isolated congenital complete heart block. Atrial-axis discontinuity was seen in 7 hearts from children born to mothers with anti-Ro antibodies. The anticipated area of the atrioventricular node was occupied by fibrous and adipose tissue. The heart from the only child whose maternal serum was anti-Ro negative had nodoventricular discontinuity of the conduction system. This difference in anatomy may have etiologic significance.

Dysrhythmias and sudden death in acromegalic heart disease. A clinicopathologic study.

A clinicopathologic investigation was carried out in a patient with acromegaly from acidophylic adenoma of the hypophysis. Pronounced cardiomegaly and disturbances in impulse formation and conduction led to sudden death. The problem of acromegalic cardiomyopathy has been reviewed. Particular attention has been focused on the histopathologic findings in the working and specific myocardium, and in the intrinsic nervous supply to the sinoatrial node, which were correlated with the electrocardiographic features of atrioventricular conduction delay and sick sinus syndrome.
