A cross-over study of the cycloplegic effects of a single topical application of cyclopentolate-phenylephrine and routine atropinization for 3.5 days.

Static refraction in children after the topical instillation of atropine twice a day during 3.5 days was compared with the refraction after a single instillation of one drop of 0.85% cyclopentolate + 1.5% phenylephrine in a randomized cross-over study. Atropine eye drops were applied by the parents at home, while the combination drop was applied by personnel at the eye clinic. Refraction was determined by retinoscopy in 40 children (3-6 years) in a single blind manner. No statistically significant difference in cycloplegic effect was found between the 2 methods. The results imply that in clini…

Comparison of ocular accommodation throughout the day in children using 0.05% low-dose atropine versus controls: A pilot study.

PURPOSE: The purpose of this pilot study was to assess the effect of 0.05% low-dose atropine on ocular accommodation (amplitude and accuracy) and how its effect changed throughout the day. METHODS: Sixteen children aged 6-16 years using 0.05% atropine and 16 controls not using atropine were enrolled. Monocular accommodative amplitude was measured objectively using a Grand Seiko WAM-5500 open field autorefractor (formerly manufactured by RyuSyo Industrial Co.) and subjectively using the push-up method. Accuracy of accommodation was measured using monocular estimation (MEM) retinoscopy. All mea…

Initial atropine dose of 0.5 mg versus 1 mg in unstable bradycardia: A propensity score-matched retrospective cohort study.

BACKGROUND: The optimal initial intravenous atropine dose for unstable bradycardia remains uncertain. While American Heart Association guidelines recommend 1.0 mg, European Resuscitation Council algorithms recommend 0.5 mg. We compared the effectiveness of these two doses in emergency department patients with unstable bradycardia. METHODS: In this single-center retrospective cohort study, adults receiving intravenous atropine as first-line treatment for unstable bradycardia (heart rate < 50 beats/min with hypoperfusion signs) were grouped by initial dose (0.5 mg or 1.0 mg). Propensity score m…

Paradox of atropine in myopia control: exploring dose-dependent efficacy, rebound effects, and optimal risk-benefit balance-a perspective.

The global surge in myopia prevalence, particularly the rising incidence of high myopia and its associated blinding complications-including myopic maculopathy, retinal detachment, and glaucoma-has transformed this condition into a pressing public health priority. Atropine, the most extensively studied pharmacological intervention for myopia control, now stands at a critical crossroads. The shift from high-concentration to low-concentration formulations has markedly improved tolerability, yet a fundamental paradox persists: the dose that minimizes side effects and rebound may be insufficient to…

Atropine often results in complete atrioventricular block or sinus arrest after cardiac transplantation: an unpredictable and dose-independent phenomenon.

BACKGROUND: A paradoxic response to atropine with development of atrioventricular (AV) block has been described in patients after heart transplantation (HTx). We investigated further the incidence and dose-response relationship of this paradoxic atropine response and explored predictive factors. METHODS: We investigated 25 clinically stable patients (age 55 +/- 2 years) 18 to 126 months after HTx. After endomyocardial biopsy, a temporary pacemaker was introduced and patients were monitored. Atropine was given in ascending doses (0.004 mg/kg body weight initially, total cumulative dose 0.035 m…

Dobutamine-atropine stress echocardiography and clinical data for predicting late cardiac events in patients with suspected coronary artery disease.

PURPOSE: To compare the relative value of clinical variables with dobutamine-atropine stress echocardiography to predict cardiac events during long-term follow-up. Dobutamine stress echocardiography is increasingly used for the detection of coronary artery disease, but little is known of its prognostic value. PATIENTS AND METHODS: A total of 430 patients (310 men; mean age 61 years, range 22 to 90) were enrolled in the study. Patients were referred for chest pain complaints and were unable to perform an adequate exercise stress test. All patients underwent dobutamine-atropine stress test (inc…

Reduced myopia progression with low-dose atropine in DIMS non-responders and evidence of altered pupillary light reflex kinetics.

This study evaluated the efficacy of adding nightly 0.025% atropine to Defocus Incorporated Multiple Segments (DIMS) spectacle treatment in children with ongoing myopia progression and explored whether dynamic pupillometry could help identify such progression. In a mixed-method design, a longitudinal arm included 20 DIMS non-responders, defined by annual progression of ≥ 0.5 D spherical equivalent refraction or ≥ 0.2 mm axial elongation, who received adjunctive atropine and were followed for 12 months. In a separate cross-sectional arm, dynamic pupillometry was performed in 60 DIMS-treated chi…

Low concentration atropine for myopia control affects accommodative function, but not near acuity: a real-world prospective case series.

INTRODUCTION: The prevalence of myopia is increasing in children around the world. Optical (glasses, contact lenses) and medical (low-concentration atropine - LCA) management options to slow progression are gaining popularity in the UK; the launch of commercial formulations of LCA is anticipated for 2025. It therefore seems timely to report our experience with the impact of LCA on accommodation and near visual acuity (NVA). METHODS: In this service evaluation - registration number 1582, we retrospectively reviewed the electronic records of 76 children and young people (CYP) using LCA (August …

Myopi-X lenses vs. low-dose atropine in myopia control: a Turkish retrospective study : Study design: retrospective observational study.

BACKGROUND: The global prevalence of myopia is rising rapidly, with projections indicating that by 2050, half of the world's population will be affected. High myopia is associated with an increased risk of sight-threatening complications, contributing to a substantial public health burden. Atropine 0.01% has been widely used for myopia control in non-Asian populations, supported by evidence demonstrating its efficacy. Myopi-X® lenses, designed to induce myopic defocus, represent an optical alternative to pharmacological intervention. Given that atropine requires monthly preparation, long-term …
