Testosterone replacement therapy increases the risk of nonunion after surgical fixation of lower extremity fractures.

INTRODUCTION: Testosterone replacement therapy (TRT) impacts bone mineral density and has been suggested to impact bone remodeling, yet its effects on fracture healing have not been completely characterized. METHODS: A large-scale, multicenter retrospective cohort analysis was conducted utilizing a national database. Adult male patients who underwent surgical fixation for lower extremity fractures between 2005 and 2025 with a minimum of 2 years of follow-up were included. Patients were stratified into TRT and control cohorts based on documented use of testosterone replacement therapy, with 90

Development and Validation of Machine Learning Models for Postjudgment Estimation of High Compensation Ratios After Lower Limb Fracture Surgery: Retrospective Study.

BACKGROUND: Orthopedic surgery is the second most common subspecialty involved in medical malpractice claims, wherein lower limb surgery carries a higher risk of claims and involves higher compensation amounts. However, effective tools for postjudgment estimation of high compensation ratios and consistency assessment against prior similar cases after lower limb fracture surgery are currently lacking in medicolegal risk management and judicial practice. OBJECTIVE: This study aimed to develop and validate multiple machine learning (ML) models to estimate a medical malpractice compensation ratio

Association of gut microbiota and arginine-agmatine metabolism with postoperative delirium in elderly patients following lower limb fracture surgery: a prospective nested case-control study.

OBJECTIVE: This study aimed to investigate the role of gut microbiota and arginine-agmatine metabolic levels in the pathogenesis of postoperative delirium (POD) in elderly patients following lower limb fracture surgery. METHODS: This prospective nested case-control study enrolled 111 patients aged ≥65 years who were scheduled for elective lower limb orthopedic surgery. POD was assessed twice daily (08:00-10:00 and 18:00-20:00) using the Confusion Assessment Method-Chinese Reversion (CAM-CR). Thirty patients who developed POD were matched to thirty non-POD patients via propensity score matchin

Ultrasonographic features and risk factors of postoperative lower limb deep venous thrombosis in patients with lower limb fractures.

Deep venous thrombosis (DVT) has insidious clinical symptoms, and only a few patients suffer from lower limb swelling, tenderness and dorsal flexion pain. We aimed to explore the ultrasonographic features and risk factors of postoperative lower limb DVT in patients with lower limb fractures. Ninety patients with lower limb fractures admitted from January 1st, 2021 to June 30th, 2023 were selected. Color Doppler ultrasonography was performed one month after operation, and the ultrasonographic features were recorded. DVT was diagnosed according to the results of venography, based on which the pa

Evaluation of the effect of vascularized fibula graft harvesting on coronal plane alignment and functional outcomes of the lower limb.

Recent studies on fibular osteotomy for varus gonarthrosis and possible subsequent biomechanical changes have attracted increasing attention to the topic. Existing studies have focused mainly on proximal fibular osteotomy with short follow-up periods. The aim of this study was to investigate changes in the alignment of the coronal plane of the ankle and knee joints in patients who underwent vascularized fibula graft harvest (VFGH). The evaluation was based on functional outcomes and radiological measurements. In the comparison between the VFGH side and the contralateral side, no significant dif

Factors influencing functional outcome of fractures of the anterior process of the calcaneus.

INTRODUCTION: Fractures of the anterior process of the calcaneus (APC) are easily overlooked in clinical practice. Most patients have good to excellent clinical outcome after conservative treatment, while some patients may have persisting symptoms and unfavorable functional outcomes. The aim of this study was to identify the risk factors associated with unfavorable functional outcome after conservative treatment in APC fractures. METHODS: All patients presenting with APC fractures and receiving conservative treatment from April 2019 to April 2020 were retrospectively assessed. The primary out

The Order of Operative Repair Does Not Influence Outcomes in Patients with Concomitant Popliteal Artery and Orthopedic Injuries.

BACKGROUND: The most challenging lower extremity traumatic injuries involve concomitant vascular and orthopedic injuries with amputation rates approaching 50%. Controversy exists as to how to prioritize the vascular and orthopedic repairs. We reviewed patients with popliteal artery and lower extremity orthopedic injuries to analyze the sequence of the vascular and orthopedic repairs on outcomes. METHODS: All adult patients with a diagnosis of concomitant popliteal artery and lower extremity fracture or dislocation were identified through a review of an institutional trauma registry performed

Correction of axial deformity during lengthening in fibular hypoplasia: Hexapodal versus monorail external fixation.

INTRODUCTION: Childhood fibular hypoplasia is a rare pathology which may or may not involve limb-length discrepancy and axial deformity in one or more dimensions. The objective of the present study was to compare the quality of the axial correction achieved in lengthening procedures by hexapodal versus monorail external fixators. The hypothesis was that the hexapodal fixator provides more precise correction. MATERIAL AND METHODS: A retrospective multicenter study included 52 children with fibular hypoplasia. Seventy-two tibias were analyzed, in 2 groups: 52 using a hexapodal fixator, and 20 u

The aetiology of rickets-like lower limb deformities in Malawian children.

UNLABELLED: Debilitating rickets-like lower limb deformities are common in children throughout the world, particularly in Malawi, Africa where the causes are unknown. We have identified that Blount disease and calcium deficiency rickets are the likely causes of these deformities and propose calcium supplementation as a potential treatment of Malawian rickets. INTRODUCTION: Surgical correction of rickets-like lower limb deformities is the most common paediatric operation performed at Beit Cure Orthopaedic Hospital, Malawi. The aim of this study was to investigate the aetiology of these deformi

Management of lower extremity malalignment during running with neuromuscular retraining of the proximal stabilizers.

This case report presents a high-level female runner with a history of more than 25 years of recurrent lower extremity (LE) injuries and LE malalignment problems during running. Due to her LE malalignment, she has been unable to run for the past 3 years. A definitive diagnosis was not found for her inability to run so by exclusion we diagnosed dysfunction of the hip, spine, and pelvic stabilizers that resulted in her in LE malalignment. To treat this muscle weakness, we designed a Pilates-evolved functional movement intervention to improve the control and strength of the proximal stabilizers a

Grebe dysplasia - prenatal diagnosis based on rendered 3-D ultrasound images of fetal limbs.

Grebe dysplasia is a rare skeletal dysplasia characterized by severe acromesomelic shortening of the long bones in a proximal to distal gradient of severity, with bones of the hands and feet more severely affected than those of the forearms and legs, which in turn are more severely affected than the humeri and femora. In addition, the bones of the lower extremities tend to be more severely affected than the bones of the upper extremities. Despite the severe skeletal deformities, the condition is not lethal and surviving individuals can have normal intelligence. Herein we report a case of Grebe

The thoracodorsal artery perforator flap with a vascularized scapular segment for reconstruction of a composite lower extremity defect.

High-energy trauma resulting in a composite defect of the lower extremity confronts the microvascular surgeon with more difficulties than do free flap reconstruction elsewhere in the body, since the choice of distant recipient vessels is particularly difficult. Combining principles of perforator flap surgery with those of composite tissue transfer, we designed a thoracodorsal artery perforator flap with a vascularized bone segment from the scapula for reconstruction of a composite lower extremity defect in a patient following a paragliding accident. This is the first report on the application

Adhesive capsulitis of the shoulder (frozen shoulder) produces bone loss in the affected humerus, but long-term bony recovery is good.

The objective of the study was to assess the short- and long-term effects of adhesive capsulitis (frozen shoulder) on the bone mineral density (BMD) of the affected extremity. BMD and clinical status of 22 patients (group A) with active-phase unilateral adhesive capsulitis and 31 patients (group B) with a previous adhesive capsulitis (average 9 years before the examination) were determined. BMD was measured from the proximal humerus, humeral shaft, radial shaft, ulnar shaft, and distal forearm of both upper extremities using dual-energy X-ray absorptiometry (DXA). In group A, the mean BMD of t
