Prenatal growth patterns of the upper jaw complex with implications for laryngeal echolocation in bats.

Craniofacial morphology is extremely diversified within bat phylogeny, however growth and development of the palate in bats remains unstudied. The formation of both midline and bilateral orofacial clefts in laryngeally echolocating bats, morphologically similar to the syndromic and non-syndromic cleft palate in humans, are not well understood. Developmental series of prenatal samples (n = 128) and adults (n = 10) of eight bat species (two pteropodids, four rhinolophoids, and two yangochiropterans), and two non-bat mammals (Mus musculus and Erinaceus amurensis), were CT-scanned and cranial bone

The relationship between bone mineral density and immobilization duration in hemiplegic limbs.

OBJECTIVE: Prolonged immobilization in stroke is known to result in hypercalciuria, hypercalcemia, accelerated bone resorption, and osteoporosis. Furthermore, bone mineral loss accelerated with increasing duration of hemiplegia. Although stroke is a common disease that causes sudden immobilization, relatively few investigations of bone metabolism in stroke have been reported. The aim of this study was to investigate the changes in bone mineral density of the forearms and legs related to duration of hemiplegia-induced immobilization after stroke. METHODS: Forty-one hemiplegic patients with str

The use of vascularised bone capping to prevent and treat amputation stump spiking in the paediatric population.

BACKGROUND: Overgrowth of the stump skeleton is a major complication seen in children after an amputation. In advanced cases, perforation of the bone spike through the skin can occur. Many surgical treatments have been employed to treat and prevent this, with best results seen when non-vascularised osteo-chondral bone grafts are placed to try to mimic a trans-articular amputation. We reviewed our outcomes using vascularized bone flaps to prevent and treat spiking. PATIENTS AND METHODS: Between 2000 and 2016 we carried out six vascularised osteo-cartilaginous bone capping procedures. Five pati

Osteosclerotic metaphyseal dysplasia with extensive interstitial pulmonary lesions: a case report and literature review.

Osteosclerotic metaphyseal dysplasia (OMD) is a very rare sclerosing bone disorder. To date, four cases have been documented in three reports. Here, we present the case of a 12-year-old girl with a history of recurrent respiratory infections, hypotonia, developmental delay, genu valgum, and hepatosplenomegaly. Radiographs revealed profound, ivory-white sclerosis of the metaphyses and epiphyses of the long bones in both the upper and lower extremities. Sclerosis also affected the ends or margins of the flat bones, including the mandible, clavicles, scapulae, ribs, iliac crests, ischia, pubic bo

Fixation of osteoporotic fractures in the upper limb with a locking compression plate.

Locking Compression Plate (LCP) has the advantageous feature that screws can be locked in the plate leaving an angular stable construct. There is no need to have contact between the plate and the bone to achieve stability resulting from friction of the plate-bone-construct. Therefore the plate does not need to be contoured exactly to the bone and the healing bone's periosteal blood supply is not affected. The LCP is used as a bridging plate to gain relative stability in multi-fragmentary, diaphyseal or metaphyseal fractures. Depending on the fracture, the combination hole can also allow the LC

Extremity injuries sustained by the UK military in the Iraq and Afghanistan conflicts: 2003-2014.

INTRODUCTION: Extremity injuries predominate in warfare, however their nature, and overall burden to the individual and the health service is yet to be characterised for the UK military in the recent conflicts of Iraq and Afghanistan. METHODS: We reviewed the all extremity injuries in survivors from the Joint Theatre Trauma Registry (JTTR) between 2003 and 2014. All cases recorded in the JTTR between the invasion of Iraq on the 19th March 2003 and cessation of combat operations in Afghanistan on the 27th October 2014, were examined. Casualties who were killed in action or died from their woun

[A murder case from 900 years ago? Analysis of extensive cranial trauma observed in a historical skeleton recovered in central Poland].

This work presents the results of study of a human skeleton from the early Middle Ages recovered in Pecławice (province of Łódź), presenting signs of extensive cranial trauma suffered perimortem. The skeleton belonged to a 20-30 year-old male of sturdy build, with prominent bone processes, marked right-side asymmetry of the bones and joints of the upper extremities, and tallness (stature well above average for early medieval times). Except for the skull, the skeleton lacks any pathologic or traumatic lesions. The right side of the skull bears signs of three extensive injuries involving the fro

Bone growth after replantation in children.

Bone growth and the development of epiphyseal plates are disturbed after the replantation of amputated extremities in children, but the potential for continued skeletal growth is almost always maintained in the replanted part. In this reported series of 12 children, all younger than 15 years old, 13 amputated parts of the upper limb have been successfully replanted. After long-term follow-up (from 21 to 216 months), bone growth of the replanted parts was clinically and radiologically evaluated. Two different growing segments were distinguished: the proximal bone segment, directly injured from

Spinal epidural abscess of uncommon presentation following urinary tract infection: A case report.

BACKGROUND: An epidural abscess is a rare but serious medical condition where a pocket of pus forms in the epidural space - the area between the outer covering of the spinal cord (the dura mater) and the bones of the spine. It's usually caused by a bacterial infection, most commonly Staphylococcus aureus. The infection can spread to this area from other parts of the body, through the bloodstream, or it may be introduced directly during spinal procedures like epidural injections or surgery. Symptoms often include severe back pain, fever, and neurological deficits like weakness or numbness, whic

Dual mini-plate vs. T-plate fixation for Edinburgh type IB proximal clavicle fractures: a comparative clinical study.

OBJECTIVE: To compare the clinical efficacy of dual mini-plate fixation and T-plate fixation in the management of Edinburgh type IB proximal clavicular fractures (PCFs). METHODS: A total of 100 patients with PCF who underwent surgical treatment using either dual mini-plate fixation (mini-plate group) or a T-plate fixation (T-plate group). Therapeutic efficacy, operative parameters, recovery metrics, postoperative complications, subjective pain, upper extremity function, shoulder range of motion, and serum bone metabolic markers were compared between the two groups. RESULTS: Clinical outcomes

Ipsilateral breast metastasis from a pulmonary adenocarcinoma: a case report and a focused review of the literature.

Metastases to the breast from extramammary malignancies are extremely rare. Ruling out the diagnosis of primary breast tumor is important in order to decide on clinical management and predict prognosis. We report a case of metastasis to the breast from a pulmonary adenocarcinoma, with extensive micropapillary component, diagnosed concomitantly with the primary tumor. A 52 year-old female patient presented with mammary gland tingling and dyspnea accompanied with fatigued of 4 months duration and a nodular shadows in the front of the upper lobe was found on a chest computed tomography (CT) scan.

Syndrome of the Month: Radioulnar Synostosis With Amegakaryocytic Thrombocytopenia Type 2.

Radioulnar synostosis with amegakaryocytic thrombocytopenia type 2 (RUSAT-2) is a rare inherited bone marrow failure syndrome characterized by congenital or progressive thrombocytopenia, frequent radioulnar synostosis, and variable multisystem involvement. It is caused by heterozygous germline pathogenic variants in the MDS1 and EVI1 Complex Locus (MECOM) gene, which encodes transcription factors essential for hematopoietic stem cell regulation and embryonic development. Disruption of highly conserved zinc finger domains within MECOM impairs long-term hematopoietic stem cell maintenance, leadi

[A case of small cell lung cancer with dermatomyositis that deteriorated with leukocytopenia due to chemotherapy].

A 64-year-old woman presented skin lesions on her face, upper extremities and finger erythema (heliotropism and Gottron's sign). She had weakness in her lower extremities. She was given a diagnosis of dermatomyositis (DM), because the serum examination showed that a myositis-specific antibody was positive whereas Jo-1 antibody was negative. The findings of chest X-ray and computed tomography showed that she had limited small cell lung cancer, but no interstitial pneumonia. She was treated with standard chemotherapy consisting of cisplatin and etoposide with accelerated hyperfractionation radio

Focal fibrocartilaginous dysplasia in the ulna with the radial head dislocation: a case report and literature review.

Focal fibrocartilaginous dysplasia (FFCD) is a rare benign bone lesion, which mainly occurs in the proximal tibia. In the upper extremity, only 21 cases have been reported so far and 14 of these involved the ulna. We present an additional case of FFCD in the distal ulna that showed progressive bowing of the right forearm and the radial head dislocation. Resection of the abnormal tissue, osteotomy of the radius and ulna, and gradual lengthening of the ulna were performed. On the basis of the previously published data of 14 cases and our data, we have summarized the etiology, clinical features,

Lengthening of the humerus for upper extremity limb length discrepancy.

We present a case of humeral lengthening for a limb length discrepancy of 7.7 cm due to proximal humeral growth arrest in a 12-year-old boy. Lengthening of 69 mm was achieved using the Wagner technique. Bone grafting was not required. Humeral lengthening may be performed for upper extremity limb length discrepancy with good results. Objectives of treatment must include protection of the radial nerve from traction injury and from entrapment in the osteotomy site. Elbow extension may be lost during lengthening, but in the adolescent age group, full return of elbow motion can be expected.

Cat Bite-Associated Infections of the Upper Extremity Requiring Surgical Treatment: A Report of Two Cases Involving Pasteurella multocida and Capnocytophaga felis.

Although cat bite-associated infections of the upper extremity are common, their impact might be underestimated due to the mild initial clinical findings. However, such infections could progress to deep musculoskeletal involvement and require surgical intervention. Herein, we report two cases of cat bite-associated infections of the upper extremities that required surgical treatment. The first case involved a rapidly progressing forearm infection caused by Pasteurella multocida that was successfully managed with surgical debridement and appropriate antimicrobial therapy. In contrast, the secon

[Chronic paronychia and synovialitis of extensor tendons due to Mycobacterium marinum. Is diagnosis or treatment the problem?].

Most infections of the upper extremity are caused by staphylococcus or streptococcus and respond well to beta-lactam antibiotics. Hand surgeons should be aware of the possible diagnosis of Mycobacterium marinum infection: 90% of the lesions are found in the upper extremity. We present a case of a chronic, cutaneous lesion of the right middle finger with synovialitis of the extensor tendons observed in a 35-year-old woman. Routine cultures from tissue of the infected finger led to the diagnosis of paronychia due to staphylococcus aureus. Despite surgical and antibacterial treatment, the lesion

Bridge Plating for Distal Radius Fractures in Low-Demand Patients With Assist Devices.

Bridge plate fixation has traditionally been described for the treatment of high-energy distal radius fractures with extensive comminution, associated instability, and polytrauma with the need for immediate upper extremity assisted weight bearing. Certain patient populations who may similarly benefit from such effective and expedient stabilization include patients with multiple comorbidities who may have lower-energy fractures, poor bone quality, and a baseline reliance on ambulatory assist devices. This article reviews treatment considerations for distal radius fractures in low-demand patient

Multiple cranial neuropathies: presenting signs of systemic lymphoma.

A 58-year-old man four years after orthototic liver transplantation presented with a mental neuropathy, multiple ocular motor nerve palsies, and an upper extremity motor neuropathy. Serial neuroimaging, cerebrospinal fluid analysis, total body computerized tomography and paraesophageal needle biopsy failed to reveal the underlying etiology. Sural nerve biopsy showed endoneurial perivascular infiltration with lymphoid cells, and bone marrow biopsy confirmed the diagnosis of a Burkitt's-type lymphoma. Systemic lymphoma with neural infiltration should be suspected in the immunosuppressed patient
