Chronic obstructive pulmonary disease: a definition and implications of structural determinants of airflow obstruction for epidemiology.

Chronic obstructive pulmonary disease (COPD) may be defined as a process characterized by the presence of chronic bronchitis or emphysema that may lead to the development of airways obstruction; airways obstruction need not be present at all stage of the process and may be partially reversible. The pathologic changes in the lungs due to smoking affect three regions: the bronchi, bronchioles, and parenchyma. The bronchi show enlargement of the submucosal glands with dilation of their ducts; infiltration with neutrophils and lymphocytes is present but not prominent. Respiratory bronchiolitis is …

Watchful waiting in laryngo-tracheobronchial amyloid: A case report.

We report the long term follow up of a now 66-year-old male with laryngo-tracheobronchial amyloidosis. The patient initially presented with dysphonia and dyspnoea. He was diagnosed with laryngeal reflux and asthma. Seven years later, the patient underwent cross sectional thoracic imaging showing diffuse thickening of the distal trachea and proximal main bronchi. The patient's FEV1 and FVC remained stable over 13 years and no intervention was required.

Venovenous Extracorporeal Membrane Oxygenation to Facilitate Removal of Endobronchial Tumors.

Short-term extracorporeal membrane oxygenation is a useful adjunct to thoracic procedures. We report the cases of 2 middle-aged men who were supported with venovenous extracorporeal membrane oxygenation to facilitate tumor debulking and recanalization of the carina and mainstem bronchi. Neither patient had major complications or adverse events. These cases suggest that short-term extracorporeal membrane oxygenation is safe in patients undergoing complex resection or debulking of endobronchial lesions.

Polysplenia in an elderly male: diagnostic approaches and review of the literature.

Polysplenia is a congenital disorder affecting the asymmetric organs, including the heart, lungs, bronchi, liver, stomach, pancreas, intestines, and spleen. Primary manifestations are heterogeneous and can include congenital heart disease, atypical placement of major abdominal organs, biliary atresia, and intestinal malrotation. Knowledge of these congenital abnormalities will enhance patient care. In this report, we present an elderly, asymptomatic individual with polysplenia to highlight the cardinal features and management issues.

Aerodigestive and Esophageal Diseases.

Aerodigestive disorders (AeroD) encompass a wide range of clinical syndromes, reflecting disruption of the interactions between the respiratory and alimentary tracts. The aerodigestive tract includes the pharynx (nasopharynx, oropharynx, and laryngopharynx), larynx, trachea, bronchi, and bronchioles, esophagus, and esophageal gastric junction. Airway protection (AirPro) remains a key but underrecognized function of the alimentary tract. The esophagus plays an important role in AirPro such that most forms of esophageal disease are considered "AeroD."

A case of tracheobronchomegaly.

Tracheobronchomegaly (Mounier-Kuhn syndrome) is a rare condition characterized by an abnormally enlarged trachea and main bronchi. Herein, we present a case of 79-year-old male with idiopathic pulmonary fibrosis and acute hypoxemic respiratory failure due to multilobar pneumonia. Computed tomography of the chest demonstrated a markedly dilated trachea, with the transverse diameter of 31mm and the sagittal diameter of 30mm. The clinical manifestations as well as its diagnosis, classification, and treatment are discussed.

Tracheobronchopathia osteochondroplastica—case report and literature review.

Tracheobronchopathia osteochondroplastica (TBO) is a rare disease of unknown etiology characterised by a formation of multiple, cartilaginous and osteocartilaginous submucosal nodules in the trachea and major bronchi. The course of the disease is usually benign but the narrowing of the respiratory tract can lead to chronic non-specific clinical symptoms. We present a case of a 50-year old man with chronic exertional dyspnoea and stenosis of the trachea visible in imaging tests, in whom the symptoms were caused by TBO.

Tracheobronchopathia osteochondroplastica: four cases.

Tracheobronchopathia osteochondroplastica (TO) is a rare and benign disorder of unknown cause affecting the large airways. It is characterized by the presence of multiple osseous and cartilaginous nodules in the submucosa of the trachea and main bronchi that is characterized by the progression of submucosal bone and/or cartilage including nodules through the lumen of trachea and bronchus. We present four cases that were diagnosed TO while investigating for the causes of hemoptysis and chronic cough. We plan to emphasize TO in differential diagnosis in proper patients.

[Tracheobronchial complications of assisted ventilation in children].

Assisted ventilation in children may cause acquired lesions of the trachea and bronchi. Symptoms may appear even during or after assisted ventilation. Tracheobronchoscopy is the best method for diagnosis of tracheobronchial abnormalities. Tracheobronchial granulomas and stenosis, and tracheobronchomalacia are the most common adverse effects of assisted ventilation via either endotracheal tube or tracheostomy. The frequency of tracheobronchial lesions is higher in case of assisted ventilation via tracheostomy, with a high incidence of stomal complications.

Williams-Campbell syndrome: a rare congenital bronchiectasis.

Williams-Campbell syndrome (WCS) is a rare congenital disorder characterised by the absence of cartilage in sub-segmental bronchi, leading to bronchiectasis. We report a case of a late adolescence male presenting with chronic respiratory symptoms initially misdiagnosed as asthma, later confirmed to have WCS following detailed evaluation. This case highlights the importance of considering WCS in the differential diagnosis of unexplained bronchiectasis and emphasises the role of advanced imaging techniques in its diagnosis.

Wegener's granulomatosis: an unusual cause of upper airway obstruction.

A 10-year-old child with a 2-month history of tracheitis presented with acute stridor, for which he required tracheostomy. Granulation tissue was found in the subglottic region and he was treated with antibiotics and corticosteroids. A week after successful decannulation of his tracheostomy, his stridor recurred and, on endoscopy under general anaesthesia, circumferential granulomas extending into both main bronchi were found. A diagnosis of Wegener's granulomatosis was made and confirmed on histology.

[An unusual cause of aorto-bronchial fistula: tuberculosis aortitis].

The aortic rupture in the pulmonary parenchyma or the bronchi rarely results in an haemoptysis. It means in most of the cases the rupture of an aortica aneurysm. We relate the observation of a aorto-bronchial fistula from a tuberculosa origin in an old woman case. Although the tuberculosa aortitis is becoming very exceptional, it still remains the cause of aorta rupture, with the formation of a false aneurysm which is rapidly fatal and so, it is important to search for it before any capricious haemoptysis.

Collagenous endobronchial obliteration after chemoradiation followed by consolidation osimertinib: a case report.

INTRODUCTION: Consolidation osimertinib after concurrent chemoradiation is a standard treatment for unresectable stage III EGFR-mutated NSCLC, but endobronchial toxicity has not been well described. To our knowledge, this is the first reported case of collagenous endobronchial obliteration after concurrent chemoradiation therapy and consolidation osimertinib. CASE PRESENTATION: We report an 83-year-old woman who developed acute dyspnea and complete right-lung atelectasis 3 months after completion of concurrent chemoradiation therapy while receiving consolidation osimertinib. Bronchoscopy show…

Tracheoplasty for expiratory collapse of central airways.

BACKGROUND: Severe central airway obstruction due to expiratory collapse occurs with malacia of intrathoracic trachea and main bronchi, often with chronic obstructive pulmonary disease. Bronchoscopically observed, it is confirmed by inspiratory-expiratory computerized tomographic chest scans. Prior attempts at surgical stabilization have not given dependable results. METHODS: Posterior tracheobronchial splinting with polypropylene mesh (Marlex) holds cartilages in more normal configuration, and fixes redundant membranous walls. Fourteen consecutive patients were so treated for severe dyspnea.…

Metal airway stent implantation in children: follow-up of seven children.

Long segment malacia of the trachea or main stem bronchi in children is not always suitable for surgical correction; patients may therefore remain ventilator-dependent and/or experience severe obstructive crises. We treated 7 children (ages, 4 months to 9 years) with extreme structural central airway obstruction with stent implantations. Six were mechanically ventilated; 5 had frequent life-threatening obstructive spells requiring deep sedation or paralysis. Diagnoses were: syndrome-associated tracheobronchomalacia (n = 4), malignancy infiltrating the carina (n = 1), congenital tracheal stenos…

Acetylcholine-induced liquid secretion by bronchial epithelium: role of Cl- and HCO3- transport.

Inhibitors of Cl- and HCO-3 secretion reduce acetylcholine-induced liquid, but not mucin, secretion by bronchial submucosal glands [S. K. Inglis, M. R. Corboz, A. E. Taylor, and S. T. Ballard. Am. J. Physiol. 272 (Lung Cell. Mol. Physiol. 16): L372-L377, 1997]. Immunohistochemical demonstration of cytochrome P-450 monooxygenase in Clara cells throughout the tracheobronchial airways of the rabbit.

The nonciliated bronchiolar epithelial (or Clara) cell is considered the primary pulmonary site of cytochrome P-450-dependent monooxygenase activity. Despite the general conception that Clara cells are restricted to bronchioles, we have previously shown that a nonciliated cell with the cytological features of the Clara cell predominates throughout rabbit tracheobronchial airways. The present study was designed to determine if the cytochrome P-450 monooxygenase system has the same distribution. Trachea, terminal bronchioles and 3 generations of bronchi were selected by microdissection from fixe…
