INTRODUCTION: The present study introduces a new ultra-high-frequency 14-lead electrocardiogram technique (UHF-ECG) for mapping ventricular depolarization patterns and calculation of novel dyssynchrony parameters that may improve the selection of patients and application of cardiac resynchronization therapy (CRT).
METHODS: Components of the ECG in sixteen frequency bands within the 150 to 1000 Hz range were used to create ventricular depolarization maps. The maximum time difference between the UHF QRS complex centers of mass of leads V1 to V8 was defined as ventricular electrical dyssynchrony

A family group of seven members is presented, two of which have pre-excitation syndrome. These subjects are identical twin brothers. One of them has the W-P-W syndrome tipe B, and the other has L-G-L syndrome. The latter had an associated atrial-septal defect, and the other twin had no associated cardiovascular lesions. Both underwent electrocardiographic and vectorcardiographic studies, as well as His bundle electrograms. In the case with W-P-W, the diagnosis was made by electrocardiography, and was confirmed by vertocardiography. The His bundle electrogram showed the habitual findings in thi

We describe a patient with frequent episodes of unusual paroxysmal supraventricular tachycardia. During the electrophysiological examination, the tachycardia was easily induced and terminated by atrial pacing. The earliest activation during right atrial activation mapping was located near the atrioventricular node and the His bundle. However, detailed mapping of the aortic root demonstrated that the local activation in the non-coronary aortic cusp preceded the activation at the His bundle region. Radiofrequency catheter ablation at this site terminated the tachycardia with no complications.

The theory that Sudden Infant Death Syndrome (SIDS) may be related to lethal cardiac arrhythmias or heart block due to structural abnormalities of the conduction system is attractive and still of particular interest. We analyze 69 autopsied cases of SIDS (46 males and 23 females, infants ranging in age from 3 to 365 days) and 24 age-matched cases of explained death (ED) as controls (16 males and 8 females), infants who died from extracardiac cause (cerebral and respiratory). SIDS and ED groups were divided into three subgroups according to the age: (A) from 3 to 60 days; (B) from 61 to 120 day

We report a rare case of broad complex atrioventricular reentrant tachycardia (AVRT) in an 18-year-old female, mediated by two distinct decrementally conducting accessory pathways (APs). The reentrant circuit consisted of antegrade conduction via an atrio-fascicular (AF) fiber and retrograde conduction through a concealed posteroseptal AP. Both APs exhibited decremental properties, and the circuit was independent of the atrioventricular (AV) node and His-Purkinje system, representing a classic example of pathway-to-pathway reentry. Sequential ablation of the retrograde and then the antegrade l

Left bundle branch area pacing (LBBAP) is a physiological pacing method as it yields narrow QRS complexes and minimizes the risk of asynchronous ventricular activation. Due to its numerous advantages, it has gained wide acceptance among electrophysiologists. We explain a simplified technique of LBBAP with a conventional stylet-driven lead without use of sheath support and demonstrate its feasibility in a case series with four patients.

Biventricular pacing remains the cornerstone of cardiac resynchronization therapy (CRT) in patients with heart failure, with well-established benefits. Left bundle branch pacing (LBBP) offers a physiologic alternative by engaging the native conduction system to restore synchrony and has generated significant enthusiasm. However, the growing adoption of LBBP should be tempered by recognition that a one-size-fits-all approach may not address the underlying substrate, particularly in those with intraventricular conduction delay. While a less-than-optimal LBBP implant may be sufficient in bradycar

Since the introduction of left bundle branch pacing (LBBP), a search for precise parameters confirming successful capture of conduction system was conducted. Most of the proposed electrocardiographic criteria refer to patients with narrow QRS complexes. We present a patient with heart failure in whom cardiac resynchronization was achieved using conduction system pacing. While measuring left ventricular activation time, an isoelectric interval of 74 ms between stimulus and R-wave appeared resulting in prolongation of V6 RWPT to 124 ms. Considering the immediate narrowing of QRS complexes follow

For patients with congenital heart disease (CHD), chronic ventricular pacing may lead to progressive cardiomyopathy owing to electromechanical dyssynchrony. Cardiac conduction system pacing (CSP) has been proposed as a physiologic pacing strategy-directly engaging the His-Purkinje system and preserving electromechanical synchrony. CSP may be indicated for a wide variety of children and adults with CHD and has emerged as an important tool in the armamentarium for cardiac implantable electronic device operators. This review provides the rationale, background, and supportive evidence for CSP in p

BACKGROUND: Ablation strategies to treat bundle branch reentrant ventricular tachycardia (BBRT) are well described. However, reports of long-term follow-up outcomes in BBRT patients without structural heart disease (SHD) are limited.
OBJECTIVE: The purpose of this study was to investigate the long-term follow-up prognosis of BBRT patients without SHD.
METHODS: Changes in electrocardiographic and echocardiographic parameters were used to evaluate progression during follow-up. Potential pathogenic candidate variants were screened using a specific gene panel.
RESULTS: Eleven consecutive BBRT p

A 62 year-old-woman presented with a right bundle branch block (RBBB) and left axis deviation (LAD) tachycardia. Radiofrequency (RF) energy was delivered to the left posterior fascicle (LPF) where 2 presystolic Purkinje potentials (P1 and P2) preceding onset of the QRS complex were recorded. During RF energy applications, the QRS morphology of the tachycardia changed from RBBB and LAD to RBBB and right axis deviation without termination of the tachycardia. The Purkinje potential was observed following the QRS complex during the tachycardia because of conduction block of the LPF distal to the r

Paroxysmal wide QRS tachycardia, based on a nodoventricular accessory connection, is an uncommon arrhythmia. In this report, the endocardial and epicardial mapping and cryoablation of a nodoventricular fiber, documented to participate in medically refractory tachycardia in an 11-year-old boy, are described. Epicardial cryothermia, applied at the earliest site of right ventricular activation, resulted in the abrupt termination of tachycardia. Endocardial cryothermia was subsequently applied in the perinodal region, the presumed site of origin of the nodoventricular fiber. No tachyarrhythmias we

The case of a premature baby with junctional ectopic tachycardia is described. The arrhythmia had an unusually benign course, contrary to what has been reported in the literature. Sinus rhythm was restored after 3 days of treatment with quinidine, and there has been no recurrence.

Serial His bundle recordings were obtained during 1:1 atrioventricular (AV) conduction in 90 patients with chronic bundle branch block over a mean interval of 30 months. Atrioventricular conduction time (AH) increased greater than or equal to 10 msec in 25 (28%) and infranodal conduction time (HV) increased greater than or equal to 8 msec in 29 (32%), but only 10 patients had parallel increases in AH and HV intervals. Increases in conduction times were independent of age, time interval between studies, cause of heart disease or initial AH or HV intervals. Women were significantly more likely t

Because ventricular fibrillation or other arrhythmia have been suggested as a cause of unexplained sudden infant death, many authors focused their studies on the conduction system especially in the 1970s. These studies were not conclusive and many pathologists have abandoned the study of the conduction system. We have reviewed the conduction system of 87 sudden infant death cases between 15 days and 1 year old: 55 were SIDS cases; 17 had mild lesions insufficient to explain death and 15 cases were sudden explained deaths considered as controls. Fetal dispersion of the AV node and/or His bundle
