Distribution of carcinogenic polycyclic aromatic hydrocarbons in urban soil across major cities of Bihar, India: seasonal variation, source apportionment, and health risk assessment.

This study investigates the distribution of sixteen priority polycyclic aromatic hydrocarbons (PAHs), various sources of PAHs, and their probable cancer-causing risks in the soil samples collected from urban cities in Bihar, India. During the winter season, the ∑16 PAH concentration was dominant and ranged from 979.36 to 5149.37 ngg-1 with a mean value of 2684.79 ngg-1, while for the summer season, it ranged from 690.06 to 4539.55 ngg-1 with a mean value of 2194.31 ngg-1. The (4-ring) PAH compounds were the major contributors, accounting for 40% and 37% in the winter and summer seasons, respec

A tetracycline analog improves acute respiratory distress syndrome survival in an ovine model.

BACKGROUND: Acute respiratory distress syndrome (ARDS) mortality remains high with no effective pharmacotherapy available. A chemically modified tetracycline (COL-3) is a potent inhibitor of matrix metalloproteinases. Prophylactic COL-3 administration has been shown to be effective in ARDS treatment. In the present study, the therapeutic effect of COL-3, given shortly after injury, was investigated in an ovine ARDS model. METHODS: The ovine ARDS model was induced by combined 40% body area third-degree burn, smoke inhalation, and barotrauma injuries. The sheep were randomly assigned into two g

Intussusception technique of intrabronchial silicone stents: description of technique and a case report.

Respiratory tract infection with human papillomavirus has been associated with major airway complications, including tracheal stenosis. We report a case of recurrent respiratory papillomatosis infection complicated with iatrogenic airway fire injury causing airway stenosis. This was treated with reconstruction and a silicone Y stent placement. Three years after the stent placement, the patient presented with wheezing and shortness of breath. Bronchoscopy revealed tracheal narrowing proximal to the tracheal limb of the Y stent. The stenosis was treated with a stent intussusception technique wit

[The use of oxygen as drug and its relevance for wound healing].

Plastic surgeons often have to deal with problematic wounds. In reconstructive surgery, as well as in chronic wounds, tissue oxygen supply is often critically low. Similarly in the treatment of severely burned patients, perfusion and oxygen supply to the areas beneath burn wounds are often critical. This paper explains the mechanisms and impact of oxygen for wound healing. It is important to mention that it has been shown that oxygen even used at ambient pressure can improve wound healing. Whereas treatment with oxygen under hyperbaric conditions is not everywhere available, at least normobari

Monitoring pulmonary vascular permeability using radiolabeled transferrin.

A simple, noninvasive technique for monitoring pulmonary vascular permeability in patients in critical care units is discussed. High vascular permeability is observed in patients with clinically defined adult respiratory distress syndrome (ARDS) but not in patients with hydrostatic pulmonary edema or in patients with minor pulmonary insults who are considered to be at risk of developing ARDS. The technique has been used in the field of therapeutics and pharmacology to test the effects of the putative antipermeability agents methylprednisolone and terbutaline sulfate. There appears to be a good

Smoke inhalation: radiologic manifestations.

The medical records and radiographs of 62 patients admitted for smoke inhalation were reviewed to determine the value of the plain chest radiograph in the early detection of inhalation injury. Pulmonary edema resulting from the inhalation injury often leads to pulmonary insufficiency, and its early diagnosis is crucial to the management of patients with this condition. In addition to the usual presentation of pulmonary edema, subtle radiographic findings of interstitial edema such as perivascular fuzziness and peribronchial "cuffing" were observed. Of 56 patients with significant inhalation in

Terrorism and blast phenomena: lessons learned from the attack on the USS Cole (DDG67).

Blast phenomena and injuries to the musculoskeletal system have been well documented for the past 50 years. The USS Cole was attacked in Aden Harbor in Yemen on October 12, 2000. Seventeen sailors were killed and 39 were wounded. The bombing of the USS Cole and an analysis of the pattern of injury are unique compared with previous terrorist bombing attacks in which the predominant injury pattern is from Type II and Type III blast phenomena. Because the ship superstructure did not collapse, there were no confounding variables in examining the pattern of injury as there would have been with shra

A rare case of hairy leukoplakia in a young immunocompetent patient.

We present a case of oral hairy leukoplakia (OHL) in a male, 21-year-old immunocompetent patient. The patient had white, asymptomatic plaques located bilaterally on the tongue margin. The patient noticed the signs days after burning his tongue with hot food, which triggered the investigation. The case was treated initially as candidiasis and frictional keratosis. Exfoliative cytology indicated the presence of actinomycetes in the lesions, and a blood count suggested mild leukopenia (3,910/mm³; reference value 4,500 - 11,000/mm³). Photodynamic therapy and antiseptic mouthwash were used, with no

A case of carbon monoxide poisoning before a vehicle fire: Availability of volatile hydrocarbons analysis.

Benzene is one of volatile hydrocarbons contained in fire smoke, and the concentrations in the blood are known to be positively correlated with that of carbon monoxide-hemoglobin (CO-Hb) in fire-related deaths. In this report, we present a vehicle fire case in which CO and benzene concentration is atypically un-correlated. The car driven by the vehicle dweller ran into an oncoming lane at high speed, hitting a traffic signal pole. A vehicle fire started when the rescuer opened the car door. A burned body and briquette stove were found when the fire was extinguished. An autopsy revealed a small

Direct airway injury results in elevated levels of sulfidopeptide leukotrienes, detectable in airway secretions.

Sulfidopeptide leukotrienes (LTC4/D4/E4) are suspected to be important lipid mediators in inflammatory responses in the lung. Previous investigations have provided evidence to support enhanced synthesis and secretion of these eicosanoids into bronchoalveolar lavage fluid in patients with Adult Respiratory Distress Syndrome (ARDS). We have prospectively examined the relationship between sulfidopeptide leukotriene levels in tracheal aspirates of 14 intubated and mechanically ventilated patients. When compared with the aspirate from one patient who required ventilation because of respiratory musc

The neurobehavioral toxicity of trichloroethylene.

Trichloroethylene is an industrial solvent used primarily in degreasing operations with some use as an anesthetic agent as well. The primary route of exposure is inhalation and the central nervous system effects consist of headache, nausea, sleepiness, burning of the eyes. Human cases of intoxication have been associated with trigeminal neuropathy, however this is probably caused by a breakdown product dichloroacetylene. Fatal exposures may be the result of cardiac failure. Chronic exposure in industrial settings may cause alterations in a variety of behavioral parameters such as reduced memor

Oropharyngeal Bleeding Due to Cannabidiol Oil Vape Use.

Vaping has become an increasingly popular alternative to smoking in recent years. We present a rare and unusual case of upper airway bleeding caused by inhalation of a cannabidiol (CBD) oil-based vape due to a chemical burn. There are no case reports of this injury in the literature, and we discuss the clinical presentation, diagnosis and our management of this potentially life-threatening injury. A 27-year-old man presented to the accident and emergency department after using a CBD oil vape. After one inhalation of the CBD oil vape, the patient experienced immediate onset pain in the orophary

Therapeutic Inhaled Sphingosine for Treating Lung Infection in a Mouse Model of Critical Illness.

BACKGROUND/AIMS: Sphingosine, a sphingoid long chain base, is a natural lipid with antimicrobial properties. Recent animal studies have shown that preventive sphingosine inhalation can rescue susceptible mice, such as cystic fibrosis-, burn injured- or aged mice from bacterial pulmonary infection. While preventing lung infections in susceptible patients has obvious clinical merit, treatment strategies for an established infection are also direly needed, particularly in the times of rising antibiotic resistance. Here, we tested the potential of sphingosine in treating an established pulmonary i

Air and soul: the science and application of aerosol therapy.

This paper reviews the history of aerosol therapy; discusses patient, drug, and device factors that can influence the success of aerosol therapy; and identifies trends that will drive the science of aerosol therapy in the future. Aerosol medication is generally less expensive, works more rapidly, and produces fewer side effects than the same drug given systemically. Aerosol therapy has been used for thousands of years by steaming and burning plant material. In the 50 years since the invention of the pressurized metered-dose inhaler, advances in drugs and devices have made aerosols the most com

Surfactant therapy for acute respiratory distress in severe pediatric burn injury: a case series.

After severe burn injury, pediatric patients often succumb to complications of respiratory failure. Surfactant has been used to improve pulmonary gas exchange for severe respiratory distress in other pediatric populations but has not been studied in pediatric burn-injured patients. Here, the authors report a case series of seven severely burned pediatric patients who received surfactant for acute respiratory distress and severe hypoxemia. Seven cases were reviewed of pediatric patients who received surfactant for severe acute respiratory distress. Data analyzed included age, TBSA burned, heigh

Acute and longer-term cardiovascular conditions in the Deepwater Horizon Oil Spill Coast Guard Cohort.

INTRODUCTION: In 2010, the U.S. Coast Guard (USCG) led a clean-up response to the Deepwater Horizon (DWH) oil spill. Human studies evaluating acute and longer-term cardiovascular conditions associated with oil spill-related exposures are sparse. Thus, we aimed to investigate prevalent and incident cardiovascular symptoms/conditions in the DHW Oil Spill Coast Guard Cohort. METHODS: Self-reported oil spill exposures and cardiovascular symptoms were ascertained from post-deployment surveys (n = 4,885). For all active-duty cohort members (n = 45,193), prospective cardiovascular outcomes were clas

Emergency percutaneous tracheostomy in a severely burned patient with upper airway obstruction and circulatory arrest.

We report the life-saving use of Griggs percutaneous tracheostomy in an arrested patient with complex upper airway obstruction, as a result of burns, smoke injuries and iterative tracheal intubation attempts. The technique was performed blindly at bedside to treat an acute episode of failed ventilation and intubation and cardiac arrest in a patient with altered neck anatomy. The intervention salvaged the situation, leaving a definitive airway. The feasibility of using an emergency Griggs percutaneous tracheostomy versus cricothyroidotomy is suggested in selected cases.
