Invasive fungal infections (IFIs) are a major threat to patients undergoing solid organ transplantation (SOT). Owing to improvements in surgical techniques, immunosuppression therapy and antifungal prophylaxis, the incidence of IFIs has been decreasing in recent years. However, IFI-associated morbidity and mortality remain significant. Invasive candidiasis (IC) and aspergillosis (IA) are the main IFIs after SOT. Risk factors for IC and IA continue to evolve, and thus strategies for their prevention should be constantly updated and targeted to both individual patient risk factors and local epid

Invasive fungal disease (IFD) is becoming more prevalent in transplant and oncology patients as a result of the potent immunosuppressive therapies used to prevent allograft rejection and/or the use of cytotoxic chemotherapy to treat cancer. Mortality attributable to IFD remains high despite advances in antifungal therapies. There is a continued need for laboratory diagnostics to help improve clinical outcomes. In recent years, culture-based detection strategies and histopathology have been supplemented with molecular and proteomic techniques as well as antigen detection methods. Refinements in

Candida albicans can cause mucosal infections in humans. This includes oropharyngeal candidiasis, which is commonly observed in human immunodeficiency virus infected patients, and vulvovaginal candidiasis (VVC), which is the most frequent manifestation of candidiasis. Epithelial cell invasion by C. albicans hyphae is accompanied by the secretion of candidalysin, a peptide toxin that causes epithelial cell cytotoxicity. During vaginal infections, candidalysin-driven tissue damage triggers epithelial signaling pathways, leading to hyperinflammatory responses and immunopathology, a hallmark of VV

Intravascular diseases due to Candida species, including endocarditis and cardiac device-associated infections, are rare yet devastating manifestations of invasive candidiasis affecting an already vulnerable population. Despite their significant associated morbidity and mortality, limited prospective data exist to inform the optimal diagnostic and therapeutic approaches to these entities. Herein, we review the existing literature pertaining to the epidemiology, diagnosis, and management of infectious endocarditis, rhythm management device infections, and circulatory support device infections c

BACKGROUND: Candida auris is an emerging multidrug-resistant yeast pathogen with several risk factors, including diabetes mellitus (DM). This study analyzed the epidemiological aspects of C. auris patients with DM (CaDM). We searched related databases without time and linguistic limitations for studies reporting DM among patients with proven C. auris infection (PCaI). METHODS: A meta-analysis was conducted using STATA (version 17) with the random-effects model based on DerSimonian and Laird methods, employing the metan and metaprop commands. RESULTS: A total of 43 studies were included. Our ra

Candidozyma auris (formerly Candida auris) has emerged as a formidable global health threat, characterized by its multidrug resistance (MDR), high transmissibility in healthcare settings, and significant mortality. The World Health Organization classifies it as a 'critical priority' fungal pathogen. Conventional infection control paradigms, often applying a 'one-size-fits-all' approach, have struggled to contain C. auris due to its unique environmental resilience and transmission dynamics. This review departs from traditional linear analyses and proposes a novel, scenario-based framework to de

BACKGROUND: Candida glabrata is an important cause of invasive candidiasis. Echinocandins are the first-line treatment of invasive candidiasis caused by C. glabrata. The epidemiological echinocandin sensitivity requires long-term surveillance and the understanding about whole genome characteristics of echinocandin non-susceptible isolates was limited.
RESULTS: The present study investigated the echinocandin susceptibility of 1650 C. glabrata clinical isolates in China from August 2014 to July 2019. The in vitro activity of micafungin was significantly better than those of caspofungin and anid

Invasive candidiasis is a common and serious infection in premature newborns. Preventing and treating fungal infections is very important to improve the prognosis of premature infants. Fluconazole and amphotericin B are used as the first choice in the treatment of invasive fungal infections of the newborns. In some cases, fluconazole and amphotericin B cannot be used due to nephrotoxicity, hepatotoxicity or resistant strains. Micafungin, which is among recently developed echinocandins, is the drug of choice in these cases. The use of micafungin in newborns is new and there is a limited experie

BACKGROUND: Measurement of serum (1,3)-β-D-glucan has emerged as an important diagnostic strategy for invasive fungal infections (IFI). Previous studies suggested that the eosinophil count would be related with the value of serum (1,3)-β-D-glucan.
METHODS: 572 blood samples obtained from 126 patients at the First Affiliated Hospital of Zhengzhou University were analyzed. The unsuitable samples were excluded according to the patient selection.
RESULTS: In our study, we found that in the 572 cumulative results, the absolute eosinophil count was significantly associated with the value of (1,3)-

Given the risk of central nervous system infection, relatively high weight-based echinocandin dosages may be required for the successful treatment of invasive candidiasis and candidemia in young infants. This open-label study assessed the safety and pharmacokinetics (PK) of micafungin in 13 young infants (>48 h and <120 days of life) with suspected candidemia or invasive candidiasis. Infants of body weight > or =1,000 and <1,000 g received 7 and 10 mg/kg/day, respectively, for a minimum of 4-5 days. In the 7-mg/kg/day group, the mean baseline weight and gestational age were 2,101 g and 30 week

Extracellular proteases are key factors contributing to the virulence of pathogenic fungi from the genus Candida. Their proteolytic activities are crucial for extracting nutrients from the external environment, degrading host defenses, and destabilizing the internal balance of the human organism. Currently, the enzymes most frequently described in this context are secreted aspartic proteases (Saps). This review comprehensively explores the multifaceted roles of Saps, highlighting their importance in biofilm formation, tissue invasion through the degradation of extracellular matrix proteins and

Disseminated candidiasis is a life-threatening disease and remains the most common bloodstream infection in hospitalized patients in the United States. Despite the availability of modern antifungal therapy, crude mortality in the last decade has remained unacceptably high. In particular, Candida auris is a multidrug-resistant, health care-associated fungal pathogen and has recently emerged as the first fungal pathogen to cause a global public health threat. A reliable animal model for disseminated C. auris candidiasis is therefore needed to study the unique aspects of this little-known host-pa

Invasive fungal infections are difficult to eradicate especially in immuno-compromised host. Amphotericin B and voriconazole have been the mainstay of treatment but both have significant toxicity. Caspofungin belongs to a new class of antifungal agents, the echinocandins. It acts on the fungal cell wall by selective inhibition of beta-(1,3)-D-glucan syntheses, which is not present in mammalian cells. In vitro data and experimental studies have demonstrated that it has antifungal activity against yeasts of the genus Candida (including those resistant to amphotericin B and azoles), severe specie

Candida infections, particularly invasive candidiasis, pose a serious global health threat. Candida albicans is the most prevalent species causing candidiasis, and resistance to key antifungal drugs, such as azoles, echinocandins, polyenes, and fluoropyrimidines, has emerged. This growing multidrug resistance (MDR) complicates treatment options, highlighting the need for novel therapeutic approaches. Antifungal peptides (AFPs) are gaining recognition for their potential as new antifungal agents due to their diverse structures and functions. These natural or recombinant peptides can effectively

Candida albicans is the most common cause of invasive fungal infections in humans. It is unclear how C. albicans escapes from phagocytic attack and survives in the hostile blood environment during life-threatening systemic infections. Using a conditional overexpression or suppression genetic strategy, we discovered that HYR1 gene reduced phagocytic killing activity of C. albicans in vitro and increased tissue fungal burden in vivo. Concordant with its positive regulation by the transcription factor Bcr1p, autonomous expression of HYR1 complemented the hypersusceptibility to phagocyte-mediated

Amphotericin B colloidal dispersion (ABCD) is an equimolar mixture of amphotericin B and cholesteryl sulphate with desirable preparation and stability characteristics. It allows the intravenous delivery of amphotericin B in doses up to 7 mg/kg daily. Peak serum concentrations of amphotericin B, given as ABCD, are lower, AUC0-infinity similar and half-life longer than deoxycholate amphotericin B. In vitro activity may be altered with respect to the deoxycholate preparation, some isolates being more resistant and others more susceptible. Preclinical toxicology with ABCD revealed a safety factor

The lungs from 27 patients who died from acute radiation sickness in Chernobyl accident were studied histologically. Pulmonary infectious complications were found caused by invasion of viral, bacterial and fungal agents. 2 cases with cytomegaloviral pneumonitis, 3 with focal bronchopneumonia and 3 with pulmonary candidiasis are described. Being dependent on hematopoietic function, the inflammatory reactions were not pronounced in postirradiation aplasia and became evident in the recovery.

INTRODUCTION: Primary subcutaneous candidiasis is a rare, severe opportunistic infection. While Candida albicans is typically a commensal, it can become invasive in immunocompromised hosts, often mimicking other necrotic skin conditions.
CASE PRESENTATION: We describe an 83-year-old male with high-risk myelodysplastic syndrome (MDS) and acute myeloid leukemia who developed a nonhealing eschar-covered ulcer on his left shin following a minor gardening injury. Despite multiple courses of broad-spectrum antibiotics, the lesion persisted. Initial superficial cultures were dismissed as colonizatio
