Discomfort of noninvasive ventilation, high flow nasal cannula oxygen, and conventional oxygen implemented after extubation in non-COPD patients.

BACKGROUND: Discomfort of noninvasive respiratory supports remain controversial. In the present study, the discomfort of high flow nasal cannula oxygen, noninvasive ventilation, and conventional oxygen therapy were evaluated in the post-extubation period. METHODS: Noninvasive ventilation alternating with high flow nasal cannula oxygen or conventional oxygen therapy were implemented for 48 h following extubation in patients free of chronic obstructive pulmonary disease. Using a 10-cm visual analog scale, the discomfort of the interface (oxygen mask, nasal prongs or facial mask), was self-evalu…

Right Ventricular Mechanical Support With ProtekDuo Cannula as a Bridge to Recovery After Post-Infarction Ventricular Septal Defect Repair.

Post-myocardial infarction ventricular septal defect is a catastrophic mechanical complication frequently associated with acute right ventricular failure and high mortality. Veno-arterial extracorporeal membrane oxygenation (ECMO) is commonly used for temporary circulatory support; however, persistent left-to-right shunting and severe right ventricular dysfunction may limit successful myocardial recovery. We report the use of prolonged isolated right ventricular mechanical support with the ProtekDuo cannula in a patient with inferior myocardial infarction complicated by ventricular septal defe…

Kinked Femoral Veno-Venous Extracorporeal Membrane Oxygenation Cannula Successfully Removed Using a Balloon-Assisted Percutaneous Approach: A Case Report.

We report a case of safe percutaneous removal of a kinked venous access cannula from the inferior vena cava using a crossover balloon-assisted technique under fluoroscopic guidance in a patient receiving femoro-femoral veno-venous extracorporeal membrane oxygenation (ECMO) for severe acute respiratory distress syndrome following massive aspiration. After cessation of ECMO flow and clamping of both cannulas, a sheath-in-cannula technique was used by inserting 7F introducer sheaths into the access and return cannulas to maintain vascular access. A balloon catheter was advanced anterogradely into…

Use of High-Flow Nasal Cannula in Amyotrophic Lateral Sclerosis Patients Intolerant to Non-Invasive Ventilation: A Case Series.

Non-invasive ventilation (NIV) is standard respiratory support for amyotrophic lateral sclerosis (ALS), but intolerance may limit its use. High-flow nasal cannula (HFNC) has emerged as a potential alternative in selected patients, but evidence regarding its use remains scarce. We describe 18 ALS patients' management with HFNC between 2014 and 2024 after documented intolerance to NIV. Mean age at HFNC initiation was 65.1 ± 12.5 years, and 66.7% of patients were female. Partial NIV intolerance occurred in 38.9% of cases, while 33.3% tolerated intermittent combined NIV and HFNC. During follow-up,…

High-flow nasal cannula oxygenation reduces desaturation risk during diagnostic flexible bronchoscopy under deep sedation: a randomized controlled trial.

BACKGROUND: Deep sedation for flexible bronchoscopy (FB) improves procedural conditions but exacerbates desaturation risks. High-flow nasal cannula (HFNC) may mitigate this situation, yet efficacy under deep sedation remains unproven. METHODS: In this randomized trial (ChiCTR2400083597), 340 ASA I-II patients undergoing FB under deep sedation (MOAA/S ≤ 1) received conventional nasal cannula (5 L/min; NC) or HFNC (25/45/65 L/min). The primary outcomes were the incidence of intraoperative desaturation (SpO₂ < 90% > 10s) and intraoperative nadir SpO₂ value. The intraoperative VAS scores for coug…

Outcomes of early high-flow nasal cannula (HFNC) use in pediatric respiratory distress in acute settings: a meta-analysis.

With the increasing incidence of pneumonia and acute respiratory distress in pediatric populations, effective oxygen delivery techniques are crucial for improving clinical outcomes. However, a debate exists across current studies on the use of high-flow nasal cannula (HFNC), and whether its benefits outweight the overutilization in a hospital setting. This systematic review and meta-analysis evaluates HFNC therapy in comparison to conventional oxygen therapy and noninvasive ventilation (NIV). A literature search was conducted to identify studies comparing HFNC to other oxygen delivery modaliti…

Flow characteristics of the drainage cannula in venoarterial extracorporeal membrane oxygenation: a comparison between normal and collapsed vessel conditions.

Venoarterial extracorporeal membrane oxygenation (VA ECMO) is an advanced life-saving therapy for patients with severe cardiopulmonary failure. Understanding the performance of the drainage cannula is critical to minimizing complications such as thrombosis formation, platelet activation, and circuit failure. This study utilizes computational fluid dynamics (CFD) to analyze the flow characteristics within the drainage cannula under both normal vessel conditions and vessel collapse scenarios. The simulations focus on flow behavior, shear stress distribution, and regions prone to platelet accumul…

Minimally invasive, transapical dual-lumened cannula for short-term left ventricular support in a clinically relevant large animal model†.

OBJECTIVES: Cardiogenic shock is still a major clinical challenge, despite the available devices. We developed a minimally invasive, transapical dual-lumened cannula, which can provide antegrade circulatory support and unloading for the left ventricle (LV). After using 3D printing technology, we wanted to test whether our prototypes are haemodynamically competent and implantable in an experimental large porcine model as a proof of concept study. METHODS: We implanted our cannula prototypes to 7 healthy porcines via median sternotomy (n = 6) and via minimally invasive access (n = 1), transapic…

Aerosol Delivery to an Adult Model via High-Flow Nasal Cannula with Inspiration-Synchronized Small-Particle Vibrating Mesh Nebulizer.

Background: Aerosol delivery via high-flow nasal cannula (HFNC) with an inspiration-synchronized vibrating mesh nebulizer (VMN) yielded higher inhaled doses than with a continuous VMN. Recently, a prototype VMN generating aerosol particles <3 µm improved aerosol delivery during invasive ventilation in adult and pediatric models, outperforming the conventional VMN. However, the effects of inspiration-synchronized small-particle VMN during HFNC remain unknown. Methods: An adult manikin was connected to a dual-chamber lung model driven by a ventilator to simulate spontaneous breathing. An HFNC sy…

Home High-Flow Nasal Cannula in Patients with Chronic Respiratory Failure: A Literature Review and Suggestions for Clinical Practice.

High-flow nasal cannula (HFNC) is a noninvasive respiratory support system that delivers air that is heated at 31°C-38°C, humidified 100%, and oxygen-enriched at a constant high flow rate of 15-60 L/min. Because of its numerous physiological benefits, convenience, and minimal side effects, HFNC has been increasingly used over the past decade in patients with acute hypoxemic respiratory failure, yet the clinical benefits of long-term HFNC remain uncertain. Several studies have suggested its potential use as an alternative home oxygen therapy for patients with chronic stable lung diseases, such …
