FH Genetic Testing in Preventive Cardiology: Unpacking Challenges and Enablers Beyond Cost.

IMPORTANCE: Familial hypercholesterolemia (FH) is a common genetic condition associated with premature cardiovascular disease. Genetic testing enables definitive diagnosis and cascade screening, yet its uptake remains limited even in cost-free settings. As Singapore implements a national subsidized genetic testing service for FH, understanding persisting barriers to testing is critical to improving service design and implementation. OBJECTIVE: This qualitative study examined individual-level barriers and facilitators of genetic testing (GT) uptake and assessed experiences with genetic testing

Cutting-Edge Cardiology: Individualizing Antiplatelet Therapy for Optimal Acute Coronary Syndrome Outcomes.

Dual antiplatelet therapy (DAPT), consisting of aspirin and a P2Y12 inhibitor, is fundamental after percutaneous coronary intervention (PCI) in acute coronary syndromes (ACSs). Recent evidence supports moving from a uniform to an individualized approach, tailoring therapy duration and intensity to patient-specific ischemic and bleeding risks. A targeted literature review was conducted in PubMed, MEDLINE, Scopus, and Google Scholar from inception to June 2025. Relevant MeSH and free-text terms included "acute coronary syndromes, dual antiplatelet therapy, percutaneous coronary intervention, tic

Bridging the gaps in interventional cardiology disparities-socioeconomic, geographic, and political inequalities.

Despite significant advancements in interventional cardiology, including PCI, TAVI, and other structural heart interventions, access to these life-saving procedures remains uneven across the globe. This viewpoint highlights how socioeconomic, geographic, and political disparities impact clinical decision-making, outcomes, and professional well-being. Drawing from real-world experience and health systems analysis, the article explores the multifaceted barriers that hinder equitable care-ranging from health literacy and rural access to workforce shortages and regulatory or reimbursement challeng…

VO2 Max in Clinical Cardiology: Clinical Applications, Evidence Gaps, and Future Directions.

PURPOSE OF REVIEW: VO₂ max is a fundamental marker of cardiorespiratory fitness with substantial prognostic and diagnostic value within the field of cardiology. This review analyzes current and emerging evidence regarding its clinical uses, highlights key evidence gaps, and explores emerging developments poised to broaden its clinical application. RECENT FINDINGS: Evidence supports VO2 max as a powerful independent predictor for heart failure, coronary artery disease, hypertrophic cardiomyopathy, and cardiac amyloidosis, supporting it use in identifying high-risk patients for advanced interve…

Hazardous alcohol use: a cross-sectional study of cardiology patients in Sweden.

BACKGROUND: Alcohol use is understudied in cardiology settings. We investigated the prevalence of hazardous alcohol use and probable dependence among cardiology patients. METHODS: Cross-sectional study in three regions of Sweden. Alcohol use was assessed using the AUDIT-10 questionnaire. We defined hazardous alcohol use as: AUDIT-10 ≥ 6 for women or ≥ 8 for men (primary definition) and probable dependence as AUDIT-10 ≥ 13 for women or ≥ 15 for men. We examined associations using logistic regression. RESULTS: We included 1107 participants (median age = 73 years; range = 18-102; 66% men). The …

Palliative Care Education in Pediatric Cardiology Fellowships: A Survey of Program Directors.

With advances in treatment options for severe congenital heart disease, there is a growing population of children and adults living with advanced heart disease, many of whom experience significant long-term comorbidities and uncertain disease trajectories. Although palliative care (PC) plays an integral role in the care of children with advanced heart disease, there is a lack of PC education in pediatric cardiology fellowship training. We distributed a cross-sectional survey to pediatric cardiology fellowship program directors (PDs) nationally (n = 58). Survey response rate was 48.3% (28/58). …

Comparison of Pretreatment in European Society of Cardiology Acute Coronary Syndrome Guidelines.

INTRODUCTION: Most patients with acute coronary syndrome (ACS) die before hospitalization. Early diagnosis and effective interventions can prevent the disease from worsening. In this single-center, retrospective study we aimed to investigate the appropriateness of the pretreatment of patients referred to the emergency department of our hospital, a percutaneous cardiac intervention (PCI) center, with a prediagnosis of ACS under the previously published European Society of Cardiology guidelines (2017 and 2020) and the new guidelines published in 2023. METHODS: Based on the date of publication o…

American Society of Nuclear Cardiology quality metrics for cardiac amyloid radionuclide imaging.

This document from the American Society of Nuclear Cardiology develops metrics for the assessment of quality for laboratories that perform cardiac amyloid radionuclide imaging. These metrics are based on clinical guidelines, appropriate use criteria, information and position statements, and expert opinion. The document introduces 15 quality metrics that address current gaps in care organized around 4 axes: A) Appropriate indications; B) Patient experience and workflow; C) Instrumentation and protocols; and D) Interpretation and reporting. With the increasing use of imaging for cardiac amyloid,…

Preconception counselling in women of reproductive age attending cardiology clinics in Scotland.

BACKGROUND: Guidelines for the management of cardiovascular disease (CVD) recommend preconception risk stratification and counselling in all women of childbearing age. We assessed the provision of preconception counselling (PCC) among women of reproductive age attending general cardiology outpatient clinics over a 12-month period in two large health boards in Scotland. METHODS AND RESULTS: Electronic health records were reviewed and data on patient demographics, cardiac diagnoses, medication use and the content of documented discussions regarding PCC were recorded. Women were classified accor…

Cardiovascular Implications of Gynecological Disorders: Bridging the Gap Between Gynecology and Cardiology.

Gynecological disorders such as endometriosis, polycystic ovary syndrome, and gynecological cancers are increasingly recognized as potential risk factors for cardiovascular disease (CVD). Endometriosis, a chronic inflammatory condition, exhibits shared pathogenic mechanisms with CVD, including endothelial dysfunction and an atherogenic lipid profile. Emerging evidence suggests a link between endometriosis and an elevated risk of cardiovascular events such as myocardial infarction, ischemic heart disease, and hypertension. Polycystic ovary syndrome, characterized by hormonal imbalances and meta

Vicious cycle of vitamin B1 insufficiency and heart failure in cardiology outpatients.

Heart failure is a major manifestation of vitamin B1 deficiency; beriberi. We have previously reported that even vitamin B1 insufficiency, milder than deficiency, is a risk for heart failure in the institutionalized elderly. Then in this cross-sectional study, sixty-eight cardiology outpatients were evaluated for their whole blood vitamin B1 and plasma brain natriuretic peptide (BNP) concentrations, a sensitive marker of heart failure, as well as their dietary intake. Whole blood vitamin B1 concentration was significantly correlated with plasma BNP level in vitamin B1-deficient/insufficient pa…

New electrocardiographic aspects of the P wave: Its value in clinical cardiology.

In this article, we will comment on new aspects of P-wave morphology that help us to better diagnose atrial blocks and atrial enlargement, and their clinical implications. These include: (1) Atypical ECG patterns of advanced interatrial block; (2) The ECG diagnosis of left atrial enlargement versus interatrial block; (3) Atrial fibrillation and advanced interatrial block: The two sides of the same coin; and (4) P-wave parameters: Clinical implications.
