Campylobacter jejuni-Associated Lumbar Vertebral Osteomyelitis with Cauda Equina Syndrome in the Absence of Gastrointes Symptoms: A Rare Case Report.

BACKGROUND: Campylobacter jejuni (C. jejuni) is a common cause of self-limiting gastroenteritis but rarely causes extraintestinal infections. Vertebral osteomyelitis due to this pathogen is exceptionally uncommon, and cases presenting without gastrointestinal symptoms are even rarer. To our knowledge, no previous case has described lumbar osteomyelitis with a spinal extradural abscess leading to cauda equina syndrome in the absence of enteric symptoms. Diagnosing and managing this infection remains clinically challenging. CASE PRESENTATION: A 65-year-old man presented with a two-month history…

How to assess the long-term recovery outcomes of patients with cauda equina syndrome before surgery: a retrospective cohort study.

BACKGROUND: Factors influencing recovery after decompression surgery for cauda equina syndrome (CES) are not completely identified. The authors aimed to investigate the most valuable predictors (MVPs) of poor postoperative recovery (PPR) in patients with CES and construct a nomogram for discerning those who will experience PPR. METHODS: Three hundred fifty-six patients with CES secondary to lumbar degenerative diseases treated at Xijing Hospital were randomly divided into training ( N =238) and validation ( N =118) cohorts at a 2:1 ratio. Moreover, 92 patients from the 970 th Hospital compose…

Prediction of the outcome of bladder dysfunction based on electrically induced reflex findings in patients with cauda equina syndrome: A retrospective study.

The evaluation of the electrically induced bulbocavernosus reflex (BCR) using electromyography (EMG) is a useful tool for evaluating the integrity of sacral spinal segments 2 to 4, and that of their afferent and efferent connections in the urogenital region. In the current retrospective study, the value of this technique in predicting the outcome of bladder dysfunction was investigated in patients with cauda equina syndrome (CES). Electrically induced BCR (E-BCR) was evaluated in 40 patients with bladder dysfunction due to CES at the subacute stage (7-90 days). Manually examined BCR, perianal …

Post-void bladder ultrasound in suspected cauda equina syndrome-data from medicolegal cases and relevance to magnetic resonance imaging scanning.

OBJECTIVE: Post-void residual (PVR) scans of less than 200 ml are increasingly being used to rule out the likelihood of cauda equina syndrome (CES) and to delay emergency MRI scanning in suspected cases. This study was done to review a series of 50 MRI confirmed cases of CES and to test the hypothesis that a PVR of less than 200 ml was unlikely to be present. METHODS: Fifty consecutive medicolegal cases involving CES were audited. Records were reviewed to see if PVR scans were done. MRI scans were reviewed, clinical and radiological diagnosis reviewed, and treatment recorded. RESULTS: Out of…

Spinal Cauda Equina Stimulation for Alternative Location of Spinal Cord Stimulation in Intractable Phantom Limb Pain Syndrome: A Case Report.

Phantom limb pain is a phenomenon in which patients experience pain in a part of the body that no longer exists. In several treatment modalities, spinal cord stimulation (SCS) has been introduced for the management of intractable post-amputation pain. A 46-year-old male patient complained of severe ankle and foot pain, following above-the-knee amputation surgery on the right side amputation surgery three years earlier. Despite undergoing treatment with multiple modalities for pain management involving numerous oral and intravenous medications, nerve blocks, and pulsed radiofrequency (RF) treat

Does Early Surgical Decompression in Cauda Equina Syndrome Improve Bladder Outcome?: Comments on a Recent Study by Srikandarajah et al.

STUDY DESIGN AND OBJECTIVES: This study by Srikandarajah et al is a retrospective cohort study of 200 CES patients. Although parts of the study appear to be well done we identified serious problems that impacted their conclusions. SUMMARY OF BACKGROUND DATA: There is strong consensus that patients with incomplete cauda equina syndrome (CESI) should be operated upon expeditiously to prevent progression to CES with urinary retention (CESR). There is controversy concerning optimal timing of surgery in patients who have entered CESR. Some studies conclude urgent surgery for CESR does nothing to i…

[Cauda equina compression syndrome (CECS): retrospective study of surgical treatment with partial dorsal laminectomy in 86 dogs with lumbosacral stenosis].

We report our results of partial lumbosacral laminectomy for treatment of canine Cauda equina Compression Syndrome due to a lumbosacral stenosis. Opposite to conventional techniques of dorsal laminectomy, only widening of the Spatium interarcuale is performed. This is achieved by exstirpation of the Lig. flavum and partial dorsal laminectomy of the first sacral segment. The Proc. spinosi and integrity of facet joints are fully maintained by this technique. In 96.5% of 86 dogs treated with this method relief of dorsal pressure and permanent rapid regression of clinical symptoms was achieved. In

Acute Cauda Equina Syndrome Following Transpedicular Biopsy of a Lumbar Vertebral Lesion: A Case Report of Spinal Non-Hodgkin's Lymphoma.

INTRODUCTION: Spinal involvement in non-Hodgkin's lymphoma (NHL) is uncommon and often presents with non-specific clinical and radiological features, frequently mimicking infective or degenerative spinal pathology. Tissue diagnosis using transpedicular biopsy is commonly performed in such cases. However, acute neurological deterioration occurring after biopsy in a pathological vertebra is rare. We report a unique case of acute cauda equina syndrome (CES) developing in close temporal association with transpedicular biopsy of a lumbar vertebral lesion, later diagnosed as NHL. CASE REPORT: A 60-…

Case Report: A rare but critical complication in patients with lumbar infection combined with cauda equina syndrome-sacrococcygeal pressure sores.

This report presents a complex case of a 65-year-old male patient with a lumbar spine infection (T12-L1) caused by Staphylococcus aureus, leading to cauda equina syndrome, and subsequently developing a massive infectious pressure sore in the sacrococcygeal region. Back pain due to spinal infections, analgesic use, neurological deficits, and systemic infections masked the early signs of the pressure sore. Prolonged immobilization and neurogenic bladder and bowel incontinence led to a 5 cm × 5 cm deep pressure sore in the sacrococcygeal area. Involving a single-stage surgery for debriding the sp…

From hospital discharge to long-term care: Unmet rehabilitation needs in cauda equina syndrome patients from a national UK cohort.

OBJECTIVES: Although long term disability may be a consequence of cauda equina syndrome (CES), the evidence base for the effect of rehabilitation or different rehabilitation strategies is lacking. Our aim was to understand long term neurological deficits (understood as rehabilitation needs) and current rehabilitation provision for patients with CES. METHODS: We retrospectively analysed data from a large UK wide cohort of CES patients presenting between 1st June 2018 and 31st May 2019. Rehabilitation referrals and attendance were described, and symptoms at discharge and one year were identifie…

Can clinical presentation predict radiologically confirmed cauda equina syndrome: retrospective case review of 530 cases at a tertiary emergency department.

PURPOSE: Cauda equina syndrome (CES) may have significant individual consequences if diagnostic delays occur. Our aim was to evaluate the presenting subjective and objective features of patients with suspected CES in comparison to those with radiologically confirmed cauda equina compression (CEC).. METHODS: This was a retrospective analysis of all cases presenting with suspected CES to a tertiary emergency care unit over a two-year period. CEC was defined as radiological confirmation of CEC by Consultant Musculoskeletal (MSK) Radiologist report (MSK-CEC) and by measured canal occupancy due to…

Comparison of capsule and posterior lumbar interbody fusion in cauda equina syndrome with retention: a 24-month follow-up study.

BACKGROUND AND OBJECTIVES: Cauda equina syndrome with retention (CESR) is a severe lumbar condition characterized by painless urine retention due to cauda equina nerve injury. The standard treatment, posterior lumbar interbody fusion (PLIF), often yields suboptimal results. This study aims to compare the clinical safety and efficacy of a novel technique, capsule lumbar interbody fusion (CLIF), with PLIF in CESR patients, hypothesizing that CLIF can enhance neurological recovery by reducing nerve tension. METHODS: A single-center, retrospective study was conducted on 83 patients with CESR due …

Large central lumbar disc herniation causing acute cauda equina syndrome with loss of evoked potentials during prone positioning for surgery.

BACKGROUND: Few studies in the literature discuss operative positioning for lumbar surgery precipitating acute cauda equina syndromes (CES). CASE DESCRIPTION: A 56-year-old male with a large L2-3-disc herniation was placed prone on a Jackson table. He immediately lost all motor and sensory evoked potentials. Signals returned to the baseline when surgery was aborted, and he was returned to the supine position. However, potentials were again lost when he was repositioned prone, following which the surgeons proceeded with surgical decompression with a good outcome. CONCLUSION: This case highlig…

Epidural venous plexus engorgement due to inferior vena cava thrombosis resulting in cauda equina syndrome: Case report and literature review.

BACKGROUND: Epidural venous plexus congestion at L5-S1 due to inferior vena cava (IVC) thrombosis led to an acute cauda equina syndrome (CES). Laminectomy to rule out an epidural abscess, allowing for resection of the dilated veins, led to immediate symptom resolution. CASE DESCRIPTION: A 47-year-old male presented with acute urinary retention and left greater than right lower extremity paresis of 2 weeks duration. Magnetic resonance imaging (MRI) revealed a contrast-enhancing space-occupying anterior epidural L5-S1 level lesion resulting in cauda equina compression. As the patient was septic…

Cauda Equina Syndrome Due To Dural Sac Shift with Engorgement of the Epidural Venous Plexus: Rare Complication After Lumbar Microdiscectomy.

BACKGROUND: Several postoperative complications related to lumbar microdiscectomy have been reported, including cauda equina syndrome. However, to the best of our knowledge, postoperative cauda equina syndrome resulting from dural sac shift with engorgement of the epidural venous plexus is yet to be reported. CASE DESCRIPTION: A 71-year-old male patient was referred to our hospital with a chief complaint of pain and sensory disturbance due to the lumbar disc herniation of L5-S1. Microdiscectomy was performed to treat the lumbar disc herniation, and his sensory disturbance improved. However, f…
