Available is pilot experience in the use of extracorporeal shock--wave lithotripsy for acute--stage management of calculous cholecystitis running with obturation of the cystic duct in the absence of complications. Crushing of the stones resulted in elimination of the duct obturation and compression of the gallbladder in 95% of the patients, the acute episode was arrested and 50% of those treated got free of the stones. These therapeutic results are promising and evidence extracorporeal lithotripsy efficiency in acute obstructive cholecystitis.

The increased incidence of gallbladder diseases after gastrectomy is discussed with regard to contractile motility of the gallbladder. Ultrasonographic findings and contraction of the gallbladder in response to egg yolk or caerulein were studied before and after gastrectomy at intervals ranging from 2 weeks to 6 months. Enlargement of the gallbladder with accumulation of biliary sludge and hypomotility were frequently observed within a month of operation for gastric cancer, suggesting that biliary stasis is an important contributing factor in postoperative acute cholecystitis. Within 3 months …

110 transduodenal interventions were performed on patients with acute cholecystitis complicated by choledocholithiasis and stenosis of the major duodenal papilla in 1981-85. Mechanical jaundice was diagnosed in 51 of them. Most of the patients were operated upon in the "cold" period. Papillosphincteroplasty was performed on 73 patients, papillosphincterotomy--on 17, transduodenal ++supra-papillary choledochoduodenoanastomosis--on 20 patients. A special probe was used during operation for the detection and dissection of the major duodenal papilla. Lethality after operation was 2.7%.

Six cases of schistosomal cholecystitis are reported; five also had calculi. Histopathological studies revealed extensive fibrocalcific reaction of the walls and around the cystic ducts. We hypothesise that this fibrosis produced strictures of the cystic duct and stagnation of bile, predisposing to cholelithiasis. Surgeons practising in endemic areas should be aware of this rare complication of chronic schistosomiasis.

Ninety patients undergoing Tc-99m disofenin hepatobiliary scintigraphy for suspected acute cholecystitis were assessed for enterogastric reflux. Seventy-seven cases showed bowel activity by one hour and were included in the study. Twenty-six percent (20/77) showed definite enterogastric reflux. The gastric activity tended to clear rapidly, even though patients remained supine during examination. Six of 20 patients (30%) with enterogastric reflux had gallbladder visualization. Of these six, one had acute cholecystitis and one had resolving acute cholecystitis with gallstone pancreatitis. There …

To test the hypothesis that there is an early stage of cholesterol gallstone formation in man characterized by symptoms of chronic cholecystitis, poor gallbladder emptying, and biliary cholesterol crystals, we studied cholecystokinin-stimulated gallbladder emptying by DISIDA scintigraphy and examined bile for cholesterol crystals in symptomatic patients with normal oral cholecystography and gallbladder sonography. Of 36 patients studied, 16 had biliary cholesterol crystals; their mean 30-min gallbladder ejection fraction was 25.9 +/- 14.8%. Among the 20 patients without crystals, the mean ejec…

The entity of postoperative acute cholecystitis has striking features that demand special attention. The process may follow intra- and extra-abdominal procedures, and the diagnosis may be especially difficult after recent abdominal operations. The course of the disease is frequently obscure and fulminant, progressing rapidly to gangrene and perforation of the gallbladder, with a high mortality rate. Six such patients, aged 69 to 83 years, were managed in our department, with one death. The cause of this complication is probably multifactorial and includes: stasis of bile of high viscosity indu…

Acute acalculous cholecystitis in burned patients develops due to general disturbances of microcirculation of polyetiological origin. Early laparotomy can facilitate diagnosis of acute acalculous cholecystitis in burned patients. Rapidly developing destruction of the gallbladder walls requires an active surgical tactics.

Acute cholecystitis is initially a chemical inflammation, but regularly complicated by bacterial invasion from the gut. Escherichia coli, Klebsiella and Streptococcus faecalis dominate among aerobic bacteria, whereas Bacteroides fragilis and clostridia are commonly encountered anaerobes. Mixed infections are prevalent. Bactibilia occurs in at least 60% of the early stage of acute cholecystitis and is particularly prevalent in the elderly. Also, bactibilia is very common in recurrent cholecystitis. A close connection is found between the presence of bactibilia and infectious complications. Alth…

Correlations between anatomic and clinical findings were absent in a series of 214 cases of acute cholecystitis, and there was a high percentage of severe forms (58 p. 100). Early operation (between 48 and 72 hours) is therefore the most logical treatment and demonstrates comparable morbidity and mortality, if not lower figures, than with delayed surgery. Diagnosis can usually be made on clinical grounds and results of ultra-sound imaging and allows surgery to be performed after intensive care and preoperative antibiotic therapy. Total surgery with perioperative cholangiography is usually poss…

Two cases are described of acute gangrenous cholecystitis following urologic operations. The subject is reviewed. The predominance of urologic procedures as the preceding factor for this postoperative complication is pointed out. The possible causes, the presenting features, the diagnosis, management, and serious prognosis are discussed. Awareness of the condition, early diagnosis, and operation are recommended. Its importance to the urologist is stressed.
