A first-in-human, phase I/IIa trial of the T cell immunotherapy personalised tumour trained lymphocytes in patients with advanced colorectal cancer: Study protocol.

BACKGROUND: Adoptive T cell therapy is a promising alternative to conventional therapies for certain solid tumours. Personalized tumour-trained lymphocytes (pTTL) is an autologous T cell therapy derived from tumour-draining regional lymph nodes (RLNs), trained to target patient-specific neoantigens arising from tumour-specific mutations. The protocol for an ongoing phase I/IIa first-in-human trial of pTTL in Stage IV Colorectal Cancer (CRC), NEOGAP-CRC-01, is presented here. METHODS: pTTL is produced by in vitro expansion of T cells from RLNs using EpiTCer® technology. Tumour-specific neoanti

Safety of nadofaragene firadenovec-vncg: review of data from phase 2 and phase 3 studies.

INTRODUCTION: Non-muscle-invasive bladder cancer (NMIBC) is a common malignancy worldwide. While Bacillus Calmette-Guérin (BCG) is standard of care for treatment for most patients with high-risk NMIBC, many will either not respond to BCG initially or will eventually develop BCG-unresponsive disease. A treatment option in BCG-unresponsive disease is nadofaragene firadenovec-vncg (Adstiladrin), a nonreplicating adenoviral vector-based gene therapy approved by the US Food and Drug Administration (FDA) for the treatment of adults with high-risk BCG-unresponsive NMIBC with carcinoma in situ with or

Emerging drugs for the treatment of progressive familial intrahepatic cholestasis: a focus on phase II and III trials.

INTRODUCTION: Progressive familial intrahepatic cholestasis (PFIC) is a group of disorders characterized by inappropriate bile formation, causing hepatic accumulation of bile acids and, subsequently, liver injury. Until recently, no approved treatments were available for these patients. AREAS COVERED: Recent clinical trials for PFIC treatment have focused on intestine-restricted ileal bile acid transporter (IBAT) inhibitors. These compounds aim to reduce the pool size of bile acids by interrupting their enterohepatic circulation. Other emerging treatments in the pipeline include systemic IBAT

Design and pilot implementation for the BETTER CARE-HF trial: A pragmatic cluster-randomized controlled trial comparing two targeted approaches to ambulatory clinical decision support for cardiologists.

BACKGROUND: Beart failure with reduced ejection fraction (HFrEF) is a leading cause of morbidity and mortality. However, shortfalls in prescribing of proven therapies, particularly mineralocorticoid receptor antagonist (MRA) therapy, account for several thousand preventable deaths per year nationwide. Electronic clinical decision support (CDS) is a potential low-cost and scalable solution to improve prescribing of therapies. However, the optimal timing and format of CDS tools is unknown. METHODS AND RESULTS: We developed two targeted CDS tools to inform cardiologists of gaps in MRA therapy fo

Immune checkpoint blockers in patients with unresectable or metastatic thymic epithelial tumours: A meta-analysis.

BACKGROUND: For patients with advanced thymic epithelial tumours (TET), there is no standard second-line treatment after platinum-based chemotherapy. Although immune checkpoint blockers (ICB) are a potential treatment strategy, their efficacy seems limited with an increased risk of immune-related adverse events (ir-AEs), thus hampering their application in daily clinical practice. METHODS: We performed a meta-analysis to better evaluate the existing evidence about the activity and safety of ICB in the setting of unresectable or metastatic advanced TET previously treated with platinum-based ch

Transient Permeation Enhancer® (TPE®) technology for oral delivery of octreotide: a technological evaluation.

INTRODUCTION: The FDA approval of oral semaglutide for type 2 diabetes (2019) and oral octreotide for acromegaly (2020) is evidence that selected niche peptides can be administered orally if formulated with selected intestinal permeation enhancers. AREAS COVERED: We evaluated the oral octreotide formulation, MYCAPSSA® (Chiasma Pharmaceuticals, Needham, MA, USA). An outline of the current standard of care in acromegaly and the benefits of oral octreotide versus depot injections is provided. We discuss the Transient Permeation Enhancer (TPE®) technology used and detail the safety and efficacy d

Impact of target-mediated drug disposition on hetrombopag pharmacokinetics and pharmacodynamics in Chinese healthy subjects and patients with chronic idiopathic thrombocytopenic purpura.

AIMS: The pharmacokinetics (PK) of hetrombopag were found to be nonlinear across evaluated dose ranges. The aim of this study was to develop a mechanism-based population pharmacokinetic/pharmacodynamic (PopPK/PD) model and to provide a reasonable expected therapeutic dose for a future confirmatory clinical study of hetrombopag. METHODS: Nonlinear mixed-effects modelling was performed using pooled 2168 hetrombopag concentrations and 1526 platelet counts from 72 healthy subjects and 32 chronic idiopathic thrombocytopenic purpura (ITP) patients from two phase I studies and one phase II study. Strengths and pitfalls of NNT and NNH in early breast cancer escalation trials.

BACKGROUND: The management of patients with high-risk early breast cancer (eBC) requires balancing between clinical benefit and toxicity. Number Needed to Treat (NNT) and Number Needed to Harm (NNH) may provide a clinically intuitive framework to assess benefit-risk profiles, but their use in eBC has not been systematically explored. METHODS: We analysed 10 phase II-III randomized clinical trials (RCTs) of treatment escalation strategies in patients with mid to high-risk eBC. To ensure consistency and cross-trial comparison, invasive disease-free survival (IDFS), disease-free survival (DFS),

Emerging drugs for antipsychotic-induced tardive dyskinesia: investigational drugs in Phase II and Phase III clinical trials.

INTRODUCTION: Antipsychotic drugs (APs) represent the mainstay of treatment for schizophrenia and other forms of psychosis. Tardive dyskinesia (TD) is a motor disorder associated with the ongoing use of APs and is characterized by involuntary, repetitive movements that are potentially irreversible. Current treatment is wanting, due in part to our limited understanding of the mechanisms underlying TD. AREAS COVERED: Risk of TD associated with APs appears linked to continuous blockade of dopamine D2 receptors in the basal ganglia. Proposed mechanisms include increased dopamine activation of D2
