[Centro-peripheral etiopathogenesis of primary headache triggered nasally].

The presence of endonasal pathological conditions or headache accompanying symptoms involving the nose, show a bi-directional link between t … tic, parasympathetic) and explain both pain and attendant accompanying symptoms during migraine and cluster headache attacks. Peripheral triggering or aggravating factors may, therefore, be responsible for or, at least, contribute to pathophysiological mechanisms of certain headaches.

[Headache in the elderly].

Chronic headache is still a frequent problem in old age, affecting about 10% of all women and 5% of all men older than 70 years. The incidence of primary headache decreases with advancing age, while that of secondary headache increases. The clinical characteristics of migraine can also change with age; for example, vegetative symptoms are less prominent, and less intense migrainous pain localized predominantly in the neck is frequently reported. Migraine aura can also be experienced more frequently in isolation, without a headache. Hypnic headache is a rare primary headache syndrome that occur

Primary Headache and Helicobacter Pylori.

H. pylori infection has recently been associated with various vascular disorders. The aim of this study was to investigate its role in primary headache, a pathology strictly associated with vascular alterations. A total of 200 subjects affected by primary headache were evaluated. H. pylori infection was diagnosed by the 13C urea breath test. Headache was classified in tension-type headache, cluster headache, and migraine with or without aura. Prevalence of H. pylori infection and gastrointestinal (GI) symptoms were evaluated. H. pylori infection was found in 40% of the patients; prevalence of …

Peripheral autonomic potentials in primary headache and drug-induced headache.

Autonomic functions of different primary headache types have been investigated in several studies, most of them analyzing cardiovascular reflex mechanisms or biochemical changes. The results are contradictory; only in tension-type headache and in cluster headache has a sympathetic hypofunction been shown in a preponderance of studies. We analyzed the peripheral autonomous potentials (PAPs) in different primary headache types and in drug-induced headache and compared the results with those of healthy subjects and of patients with low back pain. Latencies of PAPs were significantly increased in …

Texture analysis on muscle T2 maps can reveal changes in upper trapezius muscles related to primary headache disorders.

Migraine and tension-type headache (TTH) are highly prevalent primary headache disorders. Neck pain is common among patients with migraine or TTH, but objective biomarkers for the neck's myofascial involvement within the trigemino-cervical complex (TCC) are lacking. This study aimed (1) to apply texture analysis (TA) to magnetic resonance imaging (MRI) covering the upper trapezius muscles with T2 maps, which are particularly sensitive to subtle edematous changes such as those following inflammation; and (2) to associate demographics, characteristics of sleep and sports, headache, and neck pain

[A rare headache syndrome. SUNCT syndrome, hemicrania continua and red ear syndrome].

Rare headache syndromes that are so fare not yet admitted to the classification system of the International Headache Society are the SUNCT syndrome (short-lasting, unilateral, neuralgiform headache attacks with conjunctival injection and tearing), the hemicrania continua and the red ear syndrome. The clinical characteristics, differential diagnoses and therapeutic strategies of these pathophysiologically unclarified diseases are presented with three casuistic reports.

Brain structure and function related to headache: Brainstem structure and function in headache.

OBJECTIVE: To review and discuss the literature relevant to the role of brainstem structure and function in headache. BACKGROUND: Primary headache disorders, such as migraine and cluster headache, are considered disorders of the brain. As well as head-related pain, these headache disorders are also associated with other neurological symptoms, such as those related to sensory, homeostatic, autonomic, cognitive and affective processing that can all occur before, during or even after headache has ceased. Many imaging studies demonstrate activation in brainstem areas that appear specifically asso…

Chronic daily headache in the elderly.

Disabling headache disorders are ubiquitous in all age groups, including the elderly, yet they are under-recognized, underdiagnosed and unde … sistent headache, and hemicrania continua. Rarer short-duration (<4 hours) forms of CDH are chronic cluster headache, chronic paroxysmal hemicrania, SUNCT, and hypnic headache. Accurate diagnosis, management, and relief of the burden of CDH in the elderly population present numerous unique challenges as the "aging world" continues to grow. In order to implement ap …

Serotonin syndrome in patients with headache disorders.

Serotonin syndrome (SS) is an iatrogenic, drug-induced syndrome caused by serotoninergic agent. Various serotonergic drugs are used in different headache disorders. Therefore, a possibility of developing SS exists in patients with headache. Herein, we are reporting two patients with headache disorders who developed SS.Case 1: a 49-year-old man had a 6-year history of episodic cluster headache (CH). However, he had never been diagnosed with CH before reporting to us. He had been receiving amitriptyline, tramadol/acetaminophen combination and flunarizine. Lithium was started for CH. He developed…

Common headache misdiagnoses.

The authors have a personal philosophy that it is worthwhile for a physician on occasion to be the patient. When this particular physician consults another physician for a medical problem, he is seeking treatment to eradicate that particular problem. It is difficult to afford the appropriate therapy for that problem if the correct diagnosis is not made. It is exactly the same situation with patients that come to us with complaints of headache. If we cannot make the proper diagnosis, it is unlikely we will be able to render the appropriate therapy. We need to keep this in mind when we evaluate

Short-lasting unilateral neuralgiform headache attacks with ispilateral facial flushing is a new variant of paroxysmal extreme pain disorder.

BACKGROUND: We encountered a 5-year-old girl who had short-lasting, severe, unilateral temporal headaches with ipsilateral lacrimation, nasal congestion and rhinorrhoea, and facial flushing after severe attacks. Family history revealed similar short-lasting, severe headaches in an older brother, younger sister, mother, maternal aunt, and maternal grandfather's brother. METHODS: We performed routine laboratory examinations and electrophysiological and radiological studies for three children, and whole-exome sequencing to determine the genetic causality in this family. RESULTS: Focal hyperperf

Abnormalities of masseter inhibitory reflex in patients with episodic tension-type headache.

OBJECTIVE: To investigate the masseter inhibitory reflex (MIR) and its eventual changes in patients with episodic tension-type headache (TTH). METHODS: MIR was studied in 21 patients with episodic TTH and 30 healthy subjects, with age and sex matched to the study cohort. Median age of patients was 17.0 years (ranged 16~49 years), median duration of disease 12 months (1~5 years), and median frequency of headache 7.5 d per month. RESULTS: The second period of suppression (S2) of MIR was reduced in intensity and duration in 10% of controls and 66.7% (confidence interval (CI)=45.3%~85%; P<0.05)

[Idiopathic pulsating headache].

Idiopathic stabbing headache is described as a primary, benign headache seen mainly in women. It is characterized by ultra-brief attacks of pain lasting one second or less and having either single or multiple foci. The orbital region is by far the most frequently affected. Hardly any triggering factors or accompanying symptoms have been identified. The frequency of paroxysms is highly variable, ranging from one attack per year to more than 50 per day. Under extreme circumstances, stabbing pains can come as often as one attack per minute. The timing is erratic, with paroxysms that can be either
