Cochlear implant electrode array misplaced in Hyrtl's fissure.

Hyrtl's fissure is a cleft that is present in the developing fetal petrous temporal bone and extends from the area inferior to the round window to the meninges of the posterior fossa. Persistent Hyrtl's fissure, due to incomplete ossification, is considered a rare temporal bone malformation, and is a known cause of perilabyrinthine cerebrospinal fluid fistula. Very few cases are reported as being at risk of complication of cochlear implant surgery. Here we report the case of an 8-year-old boy with misplacement of an electrode array in Hyrtl's fissure. The diagnosis was made postoperatively, si

Perception of environmental sounds by experienced cochlear implant patients.

OBJECTIVES: Environmental sound perception serves an important ecological function by providing listeners with information about objects and events in their immediate environment. Environmental sounds such as car horns, baby cries, or chirping birds can alert listeners to imminent dangers as well as contribute to one's sense of awareness and well being. Perception of environmental sounds as acoustically and semantically complex stimuli may also involve some factors common to the processing of speech. However, very limited research has investigated the abilities of cochlear implant (CI) patient

Language performance in children with cochlear implants and additional disabilities.

OBJECTIVES/HYPOTHESIS: Quantify post-cochlear implant (CI) language among children with disabilities and determine the role of nonverbal cognitive quotients (NVCQ) in predicting language. STUDY DESIGN: Small cohort study in pediatric tertiary care center. METHODS: Children (n = 20) with CIs and developmental disabilities were enrolled. Receptive and expressive language was reported as language quotients (LQs). Pre- and post-CI LQs were compared using the signed-rank test. Multiple regression models analyzed language while controlling for possible confounders. RESULTS: Five subjects had symp

Users' perspectives on the benefits of FM systems with cochlear implants.

This study explored: (1) the benefits of an FM system in real-world environments from the perspective of adults with coch-lear implants, and (2) the factors and barriers to using an FM system with a cochlear implant. Using a qualitative research design, 14 adults with unilateral cochlear implants recorded their experiences during a two-month trial period with a personal FM system and responded to a questionnaire at the end of the trial. A detailed analysis of 169 journal entries (230 hours of FM use) permitted a description of the benefits and negative aspects associated with FM use in everyda

Phonological patterns in the conversational speech of children with cochlear implants.

UNLABELLED: In this descriptive, longitudinal study, phonological patterns (i.e., natural phonological processes) were examined in a set of conversational speech samples obtained from six young children fitted with cochlear implants. Both developmental and non-developmental patterns were observed. This is consistent with findings from previous studies of the speech of children with hearing loss who wear hearing aids. Several of the patterns found were also the same as those reported in previous studies of children with cochlear implants. One developmental pattern and two non-developmental patt

The acceptance of background noise in adult cochlear implant users.

PURPOSE: The purpose of this study was to determine (a) if acceptable noise levels (ANLs) are different in cochlear implant (CI) users than in listeners with normal hearing, (b) if ANLs are related to sentence reception thresholds in noise in CI users, and (c) if ANLs and subjective outcome measures are related in CI users. METHOD: ANLs and the Hearing in Noise Test (HINT; M. Nilsson, S. Soli, & J. Sullivan, 1994) were examined in 9 adult CI users and 15 adult listeners with normal hearing. In addition, the Abbreviated Profile of Hearing Aid Benefit (APHAB; R. M. Cox & G. C. Alexander, 1995)

Are cochlear implant patients suffering from perceptual dissonance?

Cochlear implants provide functional hearing to the majority of recipients and have gained widespread acceptance clinically, but the range of performance remains great and largely unexplained. Designs for implanted electrodes and electronics have converged, whereas novel speech processing strategies have proliferated. For each patient, the fitting audiologist must sort empirically through options that produce large but idiosyncratic differences in both objective performance and subjective preference. This review and analysis suggests that the place-pitch and rate-pitch theories on which cochle

[Initial information on adjustment of speech processor of the cochlear implant].

Adjustment of a speech processor of the cochlear implant is the first procedure in rehabilitation of patients after cochlear implantation. Influence of setting understated and overstated threshold levels of current perception on processing of a speech signal was studied. The results of the study show that only precise estimation of current perception threshold and comfortable loudness allow transmitting maximal acoustic information for each patient.

Evaluating training courses for parents of children with cochlear implants.

The purpose of this study was to investigate the change in effective communication between parents and children with cochlear implants when the parents attended a modified Hanen training course (Watson 1995). The study demonstrates that the course led to improvement in the effectiveness of the parent-child interaction, as rated by naïve independent observers. The Erber rating scale (Erber et al. 1995) was implemented as a tool for evaluating changes before and after the courses. Using video samples taken at the above intervals, the ratings showed a significant increase in effective communicati
