Resection margins for colorectal metastases to the liver: do they make a difference?

Objective: The authors determined an appropriate surgical treatment for liver metastases from colorectal cancers. Clinicopathologic featuresof metastatic lesions of colorectal cancers were studied. Summary Background Data: Major hepatic resection is the usual procedure for treatment of hepatic metastases from colorectal cancers. Methods: Forty consecutive patients who underwent hepatic resections were prospectively studied, for a total of 89 metastatic liver tumors. Results: Metastatic tumor often extended along Glisson’s capsule, including invasion to the portal vein (9 cases), the hepatic ve

Colorectal cancer and comorbidities - implications for surgical practice.

Colorectal cancer (CRC) is one of the most common malignant diseases worldwide. Its etiology and prognosis are significantly influenced by numerous comorbidities that may affect both the risk of tumor development and the course of the disease, including treatment response. The aim of this review article is to summarize current knowledge regarding the relationship between CRC and selected diseases and comorbid conditions, drawing on evidence from systematic reviews, meta-analyses, and cohort studies. Type 2 diabetes mellitus, inflammatory bowel disease, metabolic syndrome, obesity, hypertension

Exosomes in Colorectal Cancer: From Tumor Biology to Diagnostic and Therapeutic Applications.

Colorectal cancer (CRC) remains a leading cause of cancer-related morbidity and mortality worldwide despite advances in screening, surgical techniques, and systemic therapies. In recent years, exosomes-nanoscale extracellular vesicles involved in intercellular communication-have emerged as critical regulators of CRC biology. Exosomes mediate tumor progression, metastatic dissemination, immune evasion, and therapeutic resistance through the transfer of bioactive molecules including proteins, lipids, and non-coding RNAs. Moreover, exosomes have gained increasing attention as promising liquid bio

Tryptophan metabolism in colorectal cancer: From mechanistic insights to novel therapeutic strategies.

BACKGROUND: Tryptophan metabolism drives colorectal cancer (CRC) pathogenesis through three interconnected cascades: the kynurenine pathway, the serotonin pathway, and the gut-microbiota-dominated indole pathway. These pathways exert context-dependent dual activities determined by disease stage, metabolite concentration, cell type and tumor microenvironment. Substrate competition and metabolic flux redistribution occur across the three branches, while extra-intestinal tryptophan metabolites mediate multi-organ crosstalk. Despite substantial preclinical findings, clinical translation is hindere

Safe investigation of isolated change in bowel habit with a flexible sigmoidoscopy? A systematic review and meta-analysis.

INTRODUCTION: Public awareness campaigns have led to increasing referrals of patients to colorectal surgery for possible cancer. Change in bowel habit, is traditionally described as a symptom of a left sided bowel cancer. If this is the case in practice, it raises the potentially attractive option of investigating such patients with flexible sigmoidoscopy only. This study sought to systematically review the literature describing tumour location of patients with bowel cancer presenting with left-sided symptoms to establish the safety of potential investigation of these patients with flexible si

Polyp detection at colonoscopy: Endoscopist and technical factors.

The adenoma detection rate (ADR) has emerged as the most important quality measure in colonoscopy, as it predicts the risk of interval cancer after colonoscopy. Measuring and improving ADR is the central focus of the current quality movement in colonoscopy. High ADRs can be achieved by a colonoscopist with a thorough understanding of the wide range of endoscopic appearances of precancerous lesions in the colorectum, effective bowel preparation, and meticulous technique using high definition colonoscopes. The knowledgeable and effective examiner needs no adjunctive devices or techniques to achi

Cutaneous metastasis as a primary presentation of a pulmonary enteric adenocarcinoma.

BACKGROUND: Primary pulmonary enteric adenocarcinoma (PEAC) is a rare non-small cell lung cancer subtype sharing morphologic and immunohistochemical features with colorectal adenocarcinoma. Given the frequency of lung metastases in colorectal cancer, the differential diagnosis of PEAC according to routine morphological and immunohistochemical findings may be difficult. Genome sequence by next-generation sequencing has recently introduced new perspectives to better define the diagnosis and tumor sensitivity to treatments, while the rarity of this subtype of cancer still limits the current knowl

Combination of sigmoidoscopy and a fecal immunochemical test to detect proximal colon neoplasia.

BACKGROUND & AIMS: The combination of sigmoidoscopy and a sensitive fecal occult blood test was recommended as one strategy for colorectal cancer screening by the US Preventive Services Task Force in 2008. However, there have been no studies to evaluate the sensitivity of a one-time screen that uses both flexible sigmoidoscopy and a fecal immunochemical test (FIT) to detect advanced colorectal neoplasia. METHODS: We analyzed data from 21,794 asymptomatic persons who had undergone colonoscopy and a FIT. Analyses were performed with the following assumptions: colonoscopy would be performed for

[Expert consensus on the diagnosis and whole-course management of early-onset colorectal cancer in China (2026 version)].

The incidence of early-onset colorectal cancer (EOCRC) has been rising globally in recent years, with an especially rapid increase in disease burden in China. Compared with late-onset colorectal cancer, EOCRC is more often diagnosed following symptomatic presentation, with greater diagnostic delay and more aggressive biological behavior. Patients are typically at critical life stages regarding career development and family building, and have urgent needs for fertility and sexual function preservation, and long-term quality of life. However, current domestic and international guidelines mostly

Prospective Assessment of Carboxyhemoglobin and Methemoglobin Levels in Operating Room Personnel During HIPEC/Peritonectomy Compared with Standard Colorectal Surgery.

Background/Objectives: In the operating room, surgical staff are exposed to inhalation of smoke from the combustion of tissues when using electrocautery. By introducing HIPEC/peritonectomy in the treatment of malignant neoplasms of the peritoneum, exposure to smoke is significantly increased. To adequately perform this procedure, high-power monopolar cautery is used. The aim of this study was to assess carbon monoxide exposure associated with surgical smoke generated during peritonectomy procedures and to compare it with exposure during standard colorectal resections. Methods: In this prospect

[Colonic perforations after self-expandable metallic stenting: two case reports].

The use of self-expandable metallic stents in colorectal stenoses, both benign and malignant, is a recently born treatment, quickly widespread because of its simplicity and therapeutic efficacy. It is particulary useful, like emergency temporany treatment, in malignant colonic obstruction; in these cases the temporary endoprosthesis positioning allows the intestinal transit and to overcame the emergengy phase. After patient conditions improvement, he can be operated and the endoprosthesis removed within operating specimen. In some particularly serious cases, when only a derivative intervention

[Hereditary colorectal cancer : An update on genetics and entities in terms of differential diagnosis].

The pathologist can contribute to recognizing hereditary causes of colorectal cancer via morphology. By identifying so-called index patients, it is possible to take preventive measures in affected families. The precise definition of the clinical presentation and the histopathological phenotype help to narrow the spectrum of expected genetic alterations. Novelties within Lynch syndrome include the recognition of EPCAM as a fifth gene locus, as well as the newly defined Lynch-like syndrome with evidence of somatic mismatch repair (MMR) mutations. With regard to polyposis-associated syndromes, th
