Cost comparison of school-based mass drug administration of albendazole and ivermectin versus albendazole alone for soil-transmitted helminth control in Uganda.

The co-administration of albendazole and ivermectin (ALB-IVM) is recommended for the treatment of soil-transmitted helminth (STH) infections, especially where the prevalence of Trichuris trichiura is high. Before large-scale implementation can be considered, feasibility, acceptability and required resources and investments for the mass drug administration (MDA) of ALB-IVM compared with albendazole (ALB) alone should be assessed. This study, conducted in two districts in south-western Uganda, aimed to assess and compare the costs of school-based MDA with ALB-IVM (MDA-ALB-IVM) versus the routine

The Power of ECHO Autism: Improving Confidence and Holistic Care for Autistic Adults.

BACKGROUND: Autistic adults describe difficulties accessing high-quality primary care services. Primary care practitioners (PCPs) report a lack of confidence and knowledge in working with autistic adults. The Extension for Community Healthcare Outcomes (ECHO) model is a longitudinal clinician education program that utilises a 'hub and spoke' model, community-building strategies and peer case-based learning to improve the care of patients with medical complexity. METHODS: The present study utilised the ECHO model ('ECHO Autism Adult Healthcare') to train PCPs in how to best care for autistic a

Participant perspectives related to individual chemical exposure report-back approaches in three environmental health studies.

Returning results to participants of environmental exposure studies has become more common in recent years. Despite evidence of benefits for study participants, there are challenges in communicating results to people with limited resources or capacity to mitigate chemical exposures. We interviewed N = 54 participants and compared exposure report-back conducted in 2010-2013 across three susceptible study populations: 1) low-income pregnant individuals in the Chemicals in Our Bodies (CIOB) study; 2) the Center for the Health Assessment of Mothers and Children of Salinas (CHAMACOS) cohort; and 3)

Determinants of Health Service Responsiveness in Community-Based Vector Surveillance for Chagas Disease in Guatemala, El Salvador, and Honduras.

BACKGROUND: Central American countries face a major challenge in the control of Triatoma dimidiata, a widespread vector of Chagas disease that cannot be eliminated. The key to maintaining the risk of transmission of Trypanosoma cruzi at lowest levels is to sustain surveillance throughout endemic areas. Guatemala, El Salvador, and Honduras integrated community-based vector surveillance into local health systems. Community participation was effective in detection of the vector, but some health services had difficulty sustaining their response to reports of vectors from the population. To date, n

Uptake, benefits of and barriers to safer crack use kit (SCUK) distribution programmes in Victoria, Canada--a qualitative exploration.

BACKGROUND: Crack use is prevalent amongst street drug users in Canadian cities, and associated with severe drug use, health and social problems. Whilst few targeted interventions are available for crack use, the common use and sharing of hazardous makeshift paraphernalia are a key concern, as these risks may be associated with oral injury and blood-borne virus (BBV)--e.g., hepatitis C virus (HCV)--transmission amongst users. Recently, distribution programmes of so-called 'safer crack use kits' (SCUKs) have been initiated in select Canadian cities, primarily to reduce the use of unsafe materia

Diagnostic Diplopia: Isolated Oculomotor Nerve Palsy With Normal Imaging.

A 51-year-old man presented with progressive, subacute diplopia and ophthalmoplegia. Examination by the hospitalist demonstrated right ophthalmoplegia on left lateral gaze. No neurologist or neuro-ophthalmologist was available. Imaging and laboratory work were largely unremarkable. Although a microvascular etiology was suspected, the unremarkable workup expanded the differential diagnosis, highlighting the diagnostic and management challenges of cranial nerve III palsy when imaging is unremarkable in a community hospital with limited resources.

A process to address disparities in rates of sudden infant death syndrome.

Fetal and Infant Mortality Review (FIMR) is a continuous quality improvement program that leads to improvements in services and resources for families and, ultimately, a decrease in infant mortality. It is an action-oriented process that combines medical data with the mother's report of experiences during the life and death of her infant. The FIMR has proven to be especially important in addressing community issues associated with infant deaths related to sudden infant death syndrome.

Catheter blockage in community patients.

Research has shown that over 50 per cent of long-term catheterised patients experience recurrent blockage. For patients in the community this presents particular problems but also places increased demand on resources and nursing time. Kathryn Getliffe describes the findings of a project which examined a number of issues relating to catheter blockage in patients at home. The efficiency of nursing services can, she argues, be enhanced by effective individualised care planning.

The use of nominal group technique in identifying community health priorities in Moshi rural district, northern Tanzania.

This article highlights issues pertaining to identification of community health priorities in a resource poor setting. Community involvement is discussed by drawing experience of involving lay people in identifying priorities in health care through the use of Nominal Group Technique. The identified health problems are compared using four selected village communities of Moshi district in Kilimanjaro region, Tanzania. We conducted this study to trace the experience and knowledge of lay people as a supplement to using 'health experts' in priority setting using malaria as a tracer condition. The p

Expand the community care system: deinstitutionalization can work.

The author argues that most of the mentally ill should be treated in the community, where they can develop the skills necessary to function in society. The state hospitals should be used only to treat the most unmanageable patients. The author blames the presently inadequate community care system on insufficient funding and on the tendency of states to divide newly available funds between community services and state hospitals so that neither system is adequate. But whereas community care's problems can be resolved through greater commitment of funds and other measures, the author contends, th

Current state of mental health and substance use in Toledo, Belize: A mixed methods study.

Mental health and substance use are increasingly pressing issues in communities across low-and-middle income countries, including Belize, particularly Toledo, the country's most rural and resource-limited district. Using community-based participatory research methods, this preliminary mixed methods study (quantitative n = 163; qualitative n = 10) aims to (1) investigate mental health symptoms and substance use patterns in a non-randomized sample of individuals from southern Belize and (2) explore community perspectives on mental health among community stakeholders. Findings show high levels of

Assessing client needs in community veterinary care: a case study from WisCARES.

As community veterinary clinics expand across the United States, there is a growing recognition that services need to be aligned with the specific needs of clients and their companion animals, which may vary from one community to the next. WisCARES Community Clinic, which has served low-income pet owners in Dane County, Wisconsin for over a decade, conducted a comprehensive needs assessment in the summer of 2024 to re-evaluate the support required by its clientele. Through an interviewer-administered survey of 51 clients, conducted either in person or by phone, the study identified key areas f
