Rituximab in a desensitization protocol as a rescue therapy in severe nephrotic syndrome-a case report.

Idiopathic nephrotic syndrome is the most common glomerulopathy in children, and minimal change disease (MCD) is the most common histological pattern. First-line treatment involves glucocorticosteroids, but frequent relapses and steroid dependence may lead to steroid-related complications. Rituximab (RTX) is recommended in cases of lack of response or treatment side effects, reducing relapse rates and limiting the need for other medications. The mechanism of RTX involves B-cell depletion through various cytotoxic mechanisms. Its efficacy in nephrotic syndrome (NS) may stem from its influence o

Induction of regulatory T cells by an aluminum-CpG combination adjuvant for therapeutic and preventive vaccination against pollen allergy.

Pollen allergy is a prevalent allergic disorder affecting a substantial proportion of the global population. Allergen-specific immunotherapy (ASIT) is the only disease-modifying treatment, acting by inducing regulatory T cells (Tregs) and Th1-based immunity. However, current ASIT approaches often elicit insufficient allergen-specific tolerance and fail to correct underlying immune dysregulation, thereby limiting clinical efficacy. Here, a combination adjuvant composed of aluminum oxyhydroxide and CpG oligodeoxynucleotides (Al-CpG) is developed and formulated with the T-cell epitope derived fro

Animal models of allergen-specific immunotherapy in food allergy: Overview and opportunities.

Food allergy is an adverse reaction to otherwise harmless proteins in food. The disease is a major health problem of growing concern, affecting approximately 5%-8% of young children and 2%-4% of adults. No accepted strategy exists for prevention and treatment of food allergy, and strict avoidance of the offending food is presently the only viable management option. Living with food avoidance may have a huge impact on the quality of life of food allergic patients, with daily fear of serious or even fatal reactions. The urgent need for safe and efficient food allergy treatment options has led to

Allergic rhinitis: impact of the disease and considerations for management.

AR is a common condition affecting individuals of all ages. Those afflicted with AR often suffer from associated inflammatory conditions of the mucosa, such as AC, rhinosinusitis, asthma, otitis media with effusion, and other atopic conditions, such as eczema and food allergies. Lack of treatment or treatment with suboptimal therapy may result in reduced quality of life and compromise productivity at work or school. Although environmental controls may prove difficult to implement, and not all controls appear adequately to mitigate symptoms of AR, they continue to represent a foundation for trea

Oral administration of CpG-ODNs suppresses antigen-induced asthma in mice.

Oligodeoxynucleotides containing CpG motifs (CpG-ODNs) can protect against eosinophilic airway inflammation in asthma. Previously we have found that parenteral or mucosal administration of CpG-ODNs is effective in preventing (as well as reversing established) disease. In this study, we examined the effect of oral CpG-ODNs on the development of immune tolerance. Using an ovalbumin (OVA)-induced murine model of asthma, we found that CpG-ODNs, administered orally around the time of sensitization, prevented eosinophilic airway inflammation in a dose-dependent manner. Although oral co-administratio

Differential effects of subcutaneous and sublingual immunotherapy on timothy grass-specific TH2 CD4+ T-cell subsets.

BACKGROUND: Allergen-specific CD4+ T cells are a highly heterogenous population. Depletion of these cells has been proposed as essential to achieve allergen desensitization in allergen immunotherapy. OBJECTIVE: The overall aim of this study was to characterize the heterogeneity of timothy grass (Phleum pratense) allergen-specific CD4+ T cells and determine how the frequency and phenotype of these cells change in response to sublingual (SLIT) and subcutaneous (SCIT) immunotherapy. Correlations between frequencies of these cells with Total Nasal Symptom Score and grass-specific serum immunoglob

Current approaches to treatment of antibody-mediated rejection.

Antibody-mediated rejection (AMR) has recently been recognized as a significant and unique form of rejection that is not amenable to treatment with standard immunosuppressive medications aimed at modification of T-cell function. Recent interest in AMR and the role of B cells in rejection has been aided by the concomitant discovery that C4d staining of renal biopsy tissue is strongly associated with AMR and a poor prognosis, and the emergence of desensitization protocols for treatment of highly human leukocyte antigen (HLA)-sensitized patients. Treatment options include: (i) the use of high-dos

Management of allergy to penicillins and other beta-lactams.

The Standards of Care Committee of the British Society for Allergy and Clinical Immunology (BSACI) and an expert panel have prepared this guidance for the management of immediate and non-immediate allergic reactions to penicillins and other beta-lactams. The guideline is intended for UK specialists in both adult and paediatric allergy and for other clinicians practising allergy in secondary and tertiary care. The recommendations are evidence based, but where evidence is lacking, the panel reached consensus. During the development of the guideline, all BSACI members were consulted using a Web-b

A focused report on progestogen hypersensitivity.

INTRODUCTION: Progestogen Hypersensitivity (PH) is caused by increased sensitivity to either exogenous or endogenous progestogens. It is characterized by recurrent cutaneous eruptions including erythema multiforme, eczema, urticaria, and angioedema, which may be associated with systemic symptoms including asthma and anaphylaxis. AREAS COVERED: Symptoms may be persistent or cyclical, coinciding with progestogen levels. With increased use of oral contraceptives and hormonal treatments for fertility, the prevalence of PH is expected to continuously increase. Several proposed immunological mechan

Short-term beneficial effect of aspirin in patient with chronic rhinosinusitis and tolerant to acetylsalicylic acid.

It is well known that desensitization treatment with aspirin can significantly improve symptoms and quality of life in patient with aspirin-exacerbated respiratory disease. However, its mechanism has not been clearly understood yet. In this case report, 41-year-old male patient was referred to our allergy and immunology department with complaints of chronic rhinosinusitis including postnasal discharge, sneezing, facial pain/pressure, waking up tired, nasal obstruction, smell loss for a long time. According to the patient, the complaints were controlled partially with nasal steroid and antihist

Systemic immediate allergic reactions to arthropod stings and bites.

Most of the encounters with biting and stinging insects result in more or less pronounced localized reactions. Typically, urticarial wheals and papular reactions are observed. Less often local bullous or hemorrhagic or disseminated papular reactions, particularly in children and immunologically naive adults, may be seen. With the exception of bee and wasp venom allergies, immediate-type allergic reactions to arthropod stings and bites are rare. Systemic IgE-mediated hypersensitivity has also been reported from additional hymenoptera species, e.g. hornets, bumble bees and ants. Rare are systemi

Sublingual immunotherapy for allergy to shrimp: the nine-year clinical experience of a Midwest Allergy-Immunology practice.

BACKGROUND: Diet restrictions and fear of adverse reactions put a significant burden on the nutrition, growth and life style of children and adults with food allergies. While various disease-modifying options are pursued, there are so far no published clinical data on immunotherapy for crustaceans. The efficacy and safety of desensitization to crustaceans by means of sublingual immunotherapy is assessed for the first time in this study with a view of validating it as a clinical-practice modality. METHODS: Charts of a Midwest Allergy-Immunology practice from the period January 2014-June 2023 w

Immunological Dynamics and Outcomes of ABO-Incompatible Kidney Transplants: Insights From a 5+-Year Follow-Up at a Transplant-Heavy Tertiary Center in Saudi Arabia.

BACKGROUND: ABO-incompatible (ABOi) kidney transplantation is an essential strategy to expand the donor pool for patients with end-stage kidney disease (ESKD). However, it poses significant immunological challenges because of the risk of graft rejection. In Saudi Arabia, where ABOi transplants are limited, there is a paucity of long-term locoregional data on the impact of immunological factors on graft outcomes. This study evaluated ABOi transplant outcomes in patients with more than 5 years of follow-up, with a primary focus on immunological factors and their role in graft survival and reject

Dose-response studies of methylated and nonmethylated CpG ODNs from Bifidobacterium longum subsp. infantis for optimizing Treg cell stimulation.

Background: Allergen immunotherapy (AIT) is the most effective treatment for atopic allergic diseases, aiming to induce regulatory T cells (Treg) that modify the immune response to specific allergens, which leads to long-term tolerance and reduced symptoms. Enhancing Treg activity is crucial for improving immunotherapy outcomes. In a previous murine model study, we examined the effects of a synthetic methylated DNA oligodeoxynucleotide (ODN) from the Bl-T2 m5C motif of Bifidobacterium longum subsp. infantis. The ODN that contains the methylated BI-T2 m5C motif (methylated ODNA) sequence conjug

[Specific hyposensitization of microbial allergy by using a synthetic artificial polyelectrolyte].

In this work the effectiveness of complex immunotherapy, including specific inhalation hyposensitization with the introduction of artificial synthetic polyelectrolite, was studied. Specific allergen produced a good effect (75%) in the inhalation hyposensitization of hemophilic allergy. The intramuscular injection of artificial synthetic polyelectrolite, made in addition to the inhalation of allergen, produced a better desensitizing effect than the separate administration of allergen or polyelectrolite. The multiple administration of polyelectrolite produced a desensitizing effect on allergic r

Immunotherapy with grass pollen major allergens.

Perennial hyposensitization with a partially purified timothy extract resulted in a statistically significantly higher degree of clinical protection than treatment with the two timothy major allergens (Nos. 19 and 25) and protected better from the second--than during the first grass pollen season. The extracts were standardized biologically and adsorbed to aluminium hydroxide for administration. The therapy had a more beneficial influence on sneezing than on rhinorrhoea and blockage of nasal airways, and an excellent effect on grass pollen asthma was obtained with the partially purified timoth
