Biological and psychological correlates of intermittent dieting behavior in young women. A model for bulimia nervosa.

The eating disorder bulimia nervosa is characterized by alternating periods of strict dieting and overeating. Patients also report mood fluctuations, frequent eating related thoughts, fear of loss of control over eating, impairment of cognitive abilities such as concentration, and somatic complaints. The present study attempted to clarify to what extent these symptoms are consequences of the dieting behavior. Nine healthy young women, classified as unrestrained eaters, were set on a intermittent dieting schedule over 4 weeks. Four days each week (Tue, Wed, Thu, Fri) they had to reduce their in

[Analysis of etiologic models of disease development and continuation of bulimia nervosa using structural equation modeling].

In contrast to the great number of etiological models for bulimia nervosa, few conceptions determine the therapeutic practice. Two of these models, the ego-psychological model of reduced impulse-control and the behavioural model focussing the influence of "restrained eating" are empirically compared. Both models are not supposed to be of ubiquitous validity, but valid for subgroups of bulimic women. The theoretical assumptions are translated into structural equations and a sample of 127 bulimic women is divided up into 2 subsamples according to criteria which are presumed to define the validit

Self-induced abortion in a bulimic woman.

We report the case of a woman with bulimia nervosa, several personality disorders, and a past history of anorexia nervosa who deliberately induced an abortion via self-imposed starvation and vigorous exercise. Her history reveals severe obsessive-compulsive and narcissistic personality disorders as well as a lifelong pattern of denial of affect and illness.

Anti-ageing interventions and their neuroendocrine aspects in mammals.

A number of interventions for delaying or reversing declines in body functions due to ageing are critically reviewed here, including their relation to neuroendocrine function. Diets severely deficient in calories retard the ageing of body tissues, inhibit the development of disease and tumours, and significantly prolong the lifespan of rats and mice. Such diets also decrease hormone secretion, reduce the metabolism of the whole body, and lower gene expression. Administration of hormones, thymic peptides and other immune factors, and appropriate drugs can improve declining immune function in ol

Very low calorie diets and pre-fasting prolonged QT interval. A hidden potential danger.

The association of torsade de pointes and a prolonged electrocardiographic QT interval is well described. A prolonged QT interval may be congenital or acquired in several ways--by the use of anti-arrhythmic agents exemplified by quinidine, by the presence of hypocalcaemia or hypokalaemia, by the use of psychotropic drugs, and by the presence of intrinsic cardiac disease or bradycardias. Possibly less well appreciated is the potential for drastic weight loss to prolong the QT interval, as the present case report illustrates. A young woman weighing 244 pounds lost 24 pounds in two weeks with a c

Medical implications of obesity. Losing pounds, gaining years.

Through thousands of years of starvation and poor nutrition, the human body has become adept at storing scarce nutrients. Today, in the United States and Canada particularly, the combination of sedentary habits and excessive intake of calories is imposing a dual burden on a physiologic system that is ill-equipped to handle it. Unable to rid itself of calories, the body's only defense is to store them all. The end result is obesity, with all its deleterious effects on health and longevity.

Criterion-related validity of the Anorectic Cognitions Questionnaire.

Several authors have suggested core cognitive dysfunction as central to anorexia and bulimia nervosa. However, psychometrically sound assessment devices for these cognitions are notably lacking. Previous research with the Anorectic Cognitions Questionnaire (MAC) has suggested that it has good concurrent and criterion-related validity, and high internal consistency. As well, the factor structure has been supported. The purpose of this investigation was to further assess the criterion-related validity of the MAC. Male and female college students completed the MAC and a questionnaire about eating

Dietary treatments of obesity.

Dietary treatment of obesity is based on one or another of two premises: that the obese eat too much or that they eat the wrong things. The first is a tautology lacking explanatory power. The second is a meaningful and promising hypothesis but has yet to be effectively applied. At present, virtually all outpatient treatments of obesity, including behavior modification, are based on the first premise and consist of strategies for reducing the subject's caloric intake. Most such interventions produce short-term weight loss. Regain after the end of treatment remains the usual outcome. A survey of

Controlled-release diethylpropion hydrochloride used in a program for weight reduction.

In a double-blind, placebo-controlled evaluation in obese adults given comparable dietary and exercise recommendations, controlled-release diethylpropion hydrochloride promoted significantly more weight loss than matching placebo. The mean reduction in 12 weeks was 15.9 lb (average 1.32 lb/week) for 12 patients taking diethylpropion hydrochloride, 10.0 lb (0.84 lb/week) for 13 taking placebo, and 12.2 lb for 13 taking drug for two four-week periods separated by four weeks of placebo (1.38 lb/week on active drug and 0.30 lb/week on placebo). Amphetamine-like side effects were virtually absent.

[Multimodal obesity therapy-preconditioning neoadjuvant strategies prior to bariatric surgery].

BACKGROUND: Bariatric surgery is a key component of a multimodal treatment approach. In addition to surgery, conservative therapeutic options should be offered to promote preoperative weight loss and improve obesity-related comorbidities. OBJECTIVE: This review aims to present available preconditioning strategies and evaluate whether and to what extent they improve pre- and postoperative weight loss and reduce perioperative risks. MATERIALS AND METHODS: Analysis and discussion of current literature on the topic. RESULTS: The data available are highly heterogeneous. Preoperative weight loss

Effectiveness of lifestyle-modification approach (a randomized-controlled program of diet restriction and treadmill walking exercise) on elderly's metabolic syndrome-associated subjective tinnitus.

INTRODUCTION: Chronic subjective tinnitus complaint (CSTC) is highly prevalent in metabolic syndrome (MS) elderly. In 60 obese MS elderly with CSTC, studying the response of CSTC-related discomfort, MS components, and CSTC severity to lifestyle-modification approach was the aim of this study. METHODS: In this randomized/controlled lifestyle-modification trial, the recruitment of 60 obese MS elderly with CSTC from a local general hospital was randomly executed. The recruited elderly with MS and CSTC were randomly assigned to a study group (performed lifestyle-modification approach and the comp

Contribution of clinical factors and gut microbiota composition to weight loss in a pre-post dietary intervention pilot study in male children with obesity.

PURPOSE: Childhood obesity is a global health problem that may be linked to changes in the gut microbiota. The aim of this pilot study was to link the clinical parameters of children with obesity, who underwent a dietary intervention, with their gut microbiota profiles to better understand the factors associated with weight loss. METHODS: The study focused on 36 male children with obesity (aged 12-15 years) who were referred for treatment of primary obesity. Personal medical history information was collected at enrollment. Body composition was assessed before and after a three-week balanced h

Sesame Oil Ameliorates Alanine Aminotransferase, Aspartate Aminotransferase, and Fatty Liver Grade in Women with Nonalcoholic Fatty Liver Disease Undergoing Low-Calorie Diet: A Randomized Double-Blind Controlled Trial.

BACKGROUND: The type and amount of dietary fats play an important role in fat accumulation in the liver. Sesame oil (SO) is a good source of monounsaturated acids (MUFAs) and polyunsaturated fatty acids (PUFAs). OBJECTIVE: This trial aimed at examining the effect of SO consumption on the levels of liver enzymes and the severity of fatty liver in women with nonalcoholic fatty liver disease (NAFLD) undergoing a weight loss diet. METHODS: This randomized, double-blind, controlled trial was carried out on 60 women with NAFLD. Subjects were randomly assigned to the SO group (n = 30) and sunflower

Physiological adaptations to weight loss and factors favouring weight regain.

Obesity is a major global health problem and predisposes individuals to several comorbidities that can affect life expectancy. Interventions based on lifestyle modification (for example, improved diet and exercise) are integral components in the management of obesity. However, although weight loss can be achieved through dietary restriction and/or increased physical activity, over the long term many individuals regain weight. The aim of this article is to review the research into the processes and mechanisms that underpin weight regain after weight loss and comment on future strategies to addr

Western-style diet impairs stimulus control by food deprivation state cues: Implications for obesogenic environments.

In western and westernized societies, large portions of the population live in what are considered to be "obesogenic" environments. Among other things, obesogenic environments are characterized by a high prevalence of external cues that are associated with highly palatable, energy-dense foods. One prominent hypothesis suggests that these external cues become such powerful conditioned elicitors of appetitive and eating behavior that they overwhelm the internal, physiological mechanisms that serve to maintain energy balance. The present research investigated a learning mechanism that may underli
