Does the Expiratory Disproportion Index Remain Predictive of Airway Stenosis in Obese Patients?

OBJECTIVES/HYPOTHESIS: The expiratory disproportion index (EDI) is the ratio of forced expiratory volume in 1 second divided by peak expiratory flow rate multiplied by 100. An elevated EDI (>50) can help differentiate upper airway stenosis from other dyspnea etiologies, but this has not been verified when considering body habitus. We hypothesize that the predictive value of elevated EDI in diagnosing airway stenosis will be lower in obese patients as compared to nonobese patients. STUDY DESIGN: Retrospective cohort study. METHODS: Patients >18 years old with recorded pulmonary function test

Differentiating benign from malignant mediastinal lymph nodes visible at EBUS using grey-scale textural analysis.

BACKGROUND AND OBJECTIVE: Recent data suggest that grey-scale textural analysis on endobronchial ultrasound (EBUS) imaging can differentiate benign from malignant lymphadenopathy. The objective of studies was to evaluate grey-scale textural analysis and examine its clinical utility. METHODS: Images from 135 consecutive clinically indicated EBUS procedures were evaluated retrospectively using MATLAB software (MathWorks, Natick, MA, USA). Manual node mapping was performed to obtain a region of interest and grey-scale textural features (range of pixel values and entropy) were analysed. The initi

Role of the kurtosis statistic in evaluating complex noise exposures for the protection of hearing.

OBJECTIVE: To highlight a selection of data that illustrate the need for better descriptors of complex industrial noise environments for use in the protection of hearing. DESIGN: The data were derived using a chinchilla model. All noise exposures had the same total energy and the same spectrum; that is, they were equal energy exposures presented at an overall 100 dB(A) SPL that differed only in the scheduling of the exposure and the value of the kurtosis, beta(t), a statistical metric. Hearing thresholds were determined before and after noise exposure using the auditory-evoked potential measu

Is 18 F-fluoride PET/CT an Accurate Tool to Diagnose Loosening After Total Joint Arthroplasty?

BACKGROUND: Several studies using positron emission tomography (PET) show highly elevated periprosthetic bone uptake of fluorine-18 sodium fluoride ( 18 F-fluoride), suggestive of implant loosening after arthroplasty. Focus so far has been on qualitative but not on quantitative assessment. There is also a lack of intraoperative confirmation of preoperative 18 F-fluoride PET findings. Although the method seems to have acceptable accuracy and high sensitivity, an attempt to improve the specificity and an overall validation of the method appear warranted. QUESTIONS/PURPOSES: (1) Is there a diffe

Pure-tone audiometry without bone-conduction thresholds: using the digits-in-noise test to detect conductive hearing loss.

Objective: COVID-19 has been prohibitive to traditional audiological services. No- or low-touch audiological assessment outside a sound-booth precludes test batteries including bone conduction audiometry. This study investigated whether conductive hearing loss (CHL) can be differentiated from sensorineural hearing loss (SNHL) using pure-tone air conduction audiometry and a digits-in-noise (DIN) test.Design: A retrospective sample was analysed using binomial logistic regressions, which determined the effects of pure tone thresholds or averages, speech recognition threshold (SRT), and age on the

IL4I1 Accelerates the Expansion of Effector CD8+ T Cells at the Expense of Memory Precursors by Increasing the Threshold of T-Cell Activation.

IL4I1 is an immunoregulatory enzyme that inhibits CD8 T-cell proliferation in vitro and in the tumoral context. Here, we dissected the effect of IL4I1 on CD8 T-cell priming by studying the differentiation of a transgenic CD8 T-cell clone and the endogenous repertoire in a mouse model of acute lymphocytic choriomeningitis virus (LCMV) infection. Unexpectedly, we show that IL4I1 accelerates the expansion of functional effector CD8 T cells during the first several days after infection and increases the average affinity of the elicited repertoire, supporting more efficient LCMV clearance in WT mic

Diagnostic utility of tubomanometry for patulous eustachian tube: A comparative study.

PURPOSE: The current diagnosis of patulous Eustachian tube (PET) has limitations and cannot accurately assess its functional status. This study aims to validate tubomanometry (TMM) as an objective diagnostic tool for PET by analyzing pressure-specific R-values. METHODS: A retrospective cohort study was conducted involving 95 ears from 64 patients which were stratified into three groups: Definite PET (n = 26), Possible PET (n = 41), and Controls (n = 28). Diagnostic performance was evaluated by receiver operator characteristic (ROC) curve analysis to determine optimal R-value thresholds. RESU

Effect of amniotic suspension allograft in a rat destabilization of medial meniscus osteoarthritis model.

Placental-derived allografts have been of interest as a potential nonsurgical treatment to reduce pain and improve function in knee osteoarthritis (OA). The purpose of this study was to evaluate the effect of single and repeat injection of amniotic suspension allograft (ASA) on pain, function, and cytokine levels using a destabilization of the medial meniscus (DMM) rat model of OA. Post-DMM surgery, animals were treated with a single injection of either ASA, vehicle, or triamcinolone, or repeated injection of either ASA or vehicle. Behavioral testing including knee swelling, pain threshold, dy

Opportunistic screening for osteopathy with routine abdominal computed tomography scan in chronic pancreatitis.

BACKGROUND AND AIMS: Lumbar vertebral bone attenuation, measured in Hounsfield units (HU) can indirectly indicate the bone mineral density (BMD). The aim of this study is to determine the optimal HU threshold on abdominal computed tomography (CT) scans to detect osteopathy in patients with chronic pancreatitis (CP). METHODS: This cross-sectional study included patients with CP who underwent CT scans to measure HU at L1 to L4 vertebrae. The mean lumbar vertebral attenuation of female renal transplant donors, aged 20-30 years was utilized to calculate the T-scoreHU of all patients at each verte

Tactile Angle Discrimination Decreases due to Subjective Cognitive Decline in Alzheimer's Disease.

BACKGROUND: Subjective Cognitive Decline (SCD) is the early preclinical stage of Alzheimer's Disease (AD). Previous study provided an invaluable contribution by showing that a tactile angle discrimination system can be used to distinguish between healthy older individuals and patients with mild cognitive impairment and AD. However, that study paid little attention to the relationship between tactile angle discrimination and SCD. Therefore, a means of differentiating Normal Controls (NCs), elderly subjects with SCD, patients with amnestic Mild Cognitive Impairment (aMCI), and AD is urgently nee

The effect of mild traumatic brain injury on the visual processing of global form and motion.

Cortical visual processing involves the ventral stream (form perception) and the dorsal stream (motion perception). We assessed whether mild traumatic brain injury (TBI) differentially affects these two streams. Eleven adults with mild TBI (28 ± 9 yrs, 17 ± 5 months post injury) and 25 controls (25 ± 5 yrs) participated. Participants completed tests of global processing involving Glass patterns (form) and random dot kinematograms (motion), measurement of contrast thresholds for motion direction discrimination, a comprehensive vision screening and the Post-Concussion Symptom Inventory (PCSI). Early discrimination of sensorimotor Guillain-Barré syndrome into demyelinating or axonal subtype by automated nerve excitability testing.

In the early stage of disease, differentiating acute inflammatory demyelinating polyneuropathy (AIDP) and acute motor sensory axonal neuropathy (AMSAN) using only a conventional nerve conduction studies (NCS) may be difficult. We evaluated the differences in the motor axonal excitability properties of 16 cases of sensorimotor Guillain-Barré syndrome by nerve excitability testing (NET). The antiganglioside antibody assay and follow-up NCS resulted in 12 patients diagnosed as AIDP and 4 patients as AMSAN. Clinical and excitability parameters in each group were compared with those in 30 normal co

Altered thymic differentiation and modulation of arthritis by invariant NKT cells expressing mutant ZAP70.

Various subsets of invariant natural killer T (iNKT) cells with different cytokine productions develop in the mouse thymus, but the factors driving their differentiation remain unclear. Here we show that hypomorphic alleles of Zap70 or chemical inhibition of Zap70 catalysis leads to an increase of IFN-γ-producing iNKT cells (NKT1 cells), suggesting that NKT1 cells may require a lower TCR signal threshold. Zap70 mutant mice develop IL-17-dependent arthritis. In a mouse experimental arthritis model, NKT17 cells are increased as the disease progresses, while NKT1 numbers negatively correlates wit

Objective method for differentiating between drug-induced vestibulotoxicity and cochleotoxicity.

HYPOTHESIS: An objective direct method is proposed to differentiate between drug-induced functional vestibulotoxicity and cochleotoxicity. BACKGROUND: Many substances are ototoxic. Although there are objective methods to directly evaluate functional cochlear toxicity (auditory nerve brainstem responses [ABR]), it is more difficult to assess direct functional ototoxicity to the various vestibular end organs. METHODS: Short-latency vestibular evoked potentials (VsEP) from different vestibular end organs and ABR, were used to assess functional impairment of the vestibular and cochlear end organ

Sharp cell type boundaries emerge from coordinated morphogen signaling.

Classic models of the French flag problem depict sharp cell type boundaries emerging from threshold responses to morphogen gradients. How discrete cell type boundaries arise from morphogen signals that vary continuously across developing tissues remains incompletely understood. We use hair follicle dermal condensate (DC) formation to study a sharp developmental transition in which proliferative progenitors undergo cell cycle exit concurrent with molecular differentiation. Using genetic and genomic approaches, we show that Wnt and Hedgehog signaling coordinate separable cellular events during D

Neural stem cell-loaded biohybrid hydrogel improves cochlear implants by electrode-neural coupling and neural regeneration.

Background: Contemporary cochlear implants (CIs) face unresolved dual challenges: biomechanical-electrochemical mismatch at the electrode-tissue interface and progressive spiral ganglion neuron (SGN) degeneration, severely limiting long-term auditory restoration. Integrating regenerative medicine with bioelectronic engineering offers promise to overcome these bottlenecks. Methods: A biohybrid neural interface was developed by embedding neural stem cells (NSCs) in photopolymerized poly(3,4-ethylenedioxythiophene):poly(styrenesulfonate) (PEDOT:PSS)/collagen hydrogel.
