Colo-ovarian fistula is a rare entity. The case of a 54-year-old female with a colo-ovarian fistula is presented. We describe our experience in managing this complication of diverticulitis and propose a workup and treatment plan. Initial imaging and diagnostic studies are described. En-bloc resection of the sigmoid colon and ovary was performed. A review of the literature is presented.

The authors reports a case of cervical and abdominal subcutaneous emphysema, associated to pneumoretroperitoneum and pneumomediastinum, with no free air in the abdominal cavity, secondary to perforated diverticulitis of the sigmoid colon. We comment the rarity of this clinical presentation, the physiopathologic mechanisms of subcutaneous air presence in cases of non instrumental perforation of abdominal viscera and the ominous significance of the subcutaneous emphysema in such perforations.

Colovesical fistulas were identified in 42 patients. Diverticulitis was the underlying cause in 40 per cent of the patients. Carcinoma of the colon or cervix caused 33 per cent of the fistulas. Cystoscopy was the most effective procedure in diagnosing the fistulas. Surgical therapy had to be individualized to the patient's condition. Fistulas secondary to radiation were associated with a high complication rate.

BACKGROUND: Extramedullary plasmacytoma is an uncommon tumor that most often involves the nasopharynx or upper respiratory tract. Extramedullary plasmacytoma is a type of plasma cell neoplasm that can present as a primary tumor or secondary to another plasma cell neoplasm, such as multiple myeloma. Secondary extramedullary plasmacytoma is usually noted in the advanced stages of the disease. Involvement of the gastrointestinal tract occurs in approximately 10% of cases.
CASE PRESENTATION: A 71-year-old Caucasian woman with known diverticular disease of the colon and multiple myeloma diagnosed

63 colonic occlusions were operated on from 1975 to 1988. Cancer and stenotic diverticular sigmoiditis were found to be the main causes of colonic obstruction in respectively 36 cases (55%) and 8 cases (13%). Other etiologies were represented by neoplasic gynecologic diseases (9.5%). The rectosigmoid was involved in 50% of the cases (31) while transverse colon and caecum were respectively concerned in 25% and 17%. The 2 main therapeutical procedures used were colic resection (35 cases-55%) with immediate anastomosis in 1 case out of 3 and derivation (26 cases 41%) mostly in the proper from of

BACKGROUND: Transanal hybrid rectal and colon resection have been introduced in recent years at dedicated surgical centers. The anus is used as a natural orifice for large size access. The use of transanal hybrid colectomy techniques is still in its infancy with outcomes and unique complications being identified. The purpose of this work is the evaluation of outcomes for transanal hybrid colon resections (ta-CR), including intra operative and postoperative complications, results, and advantages.
METHODS: A prospectively maintained database was analyzed. Inclusion criteria were any patient who

OBJECTIVE: Aim of our study was to identij5 the risk factors for operative morbility and mortality after urgent surgery for complicated sigmoid diverticulitis. A further end point was define the adequate surgical approach in these patients.
METHODS: Data fJom 118 patients who were admitted for emergency surgery between 2000 and 2009 for non-haemorrhagic complicated diverticulitis of the sigmoid colon were retrospectively evaluated and analysed. Operative options included resection with primary anastomoses (PA), Hartmann's procedure (HP) and colostomy. All operative complications were noted an

The aim of this work is a critical analysis of our results with primary laparoscopic resection and anastomosis of the colon for diverticulitis. From October 1993 to October 1997, 171 patients with a mean age of 60 years (97 males, 74 females) have been operated laparoscopically. 95 patients were operated electively after many episodes of acute diverticulitis and 76 patients in the acute phase, of whom 11 patients presented a colovesical fistula. 6 left hemicolectomies and 165 sigmoid resections were performed. Among the sigmoid resections 11 patients with simultaneous resection of a colovesica

Point-of-care ultrasound (POCUS) is essential for diagnosing acute abdominal conditions, especially in resource-limited settings. This report discusses a 51-year-old male presenting with left lower quadrant pain, pallor, and low-grade fever. Ultrasound performed at the Italian Navy's Divers and Riders Group with a Mindray DC-60 system revealed bowel wall thickening, increased echogenicity of surrounding fat, and a small fluid collection, suggestive of acute diverticulitis. A contrast-enhanced CT scan at a civilian hospital confirmed the diagnosis. This case highlights the value of POCUS in dia

BACKGROUND: Up to 50% of patients with acute complicated diverticulitis require operative management on their index admission. There is ongoing debate as to whether primary anastomosis with diverting ileostomy versus a Hartmann's procedure is the optimal surgical approach for these patients. This study aims to compare postoperative complications in patients undergoing either Hartmann's procedure or primary anastomosis and diverting ileostomy for perforated diverticulitis using recent National Inpatient Sample data.
METHODS: Patients who underwent either primary anastomosis with diverting ileo

BACKGROUND: Functional dependence (FD) is associated with poor postoperative outcomes. We investigated the influence of FD on the postoperative outcomes of colectomy.
METHODS: The 2012-2020 National Surgical Quality Improvement Program was queried for patients who had undergone colectomy for diverticulitis. The patients were analyzed based on FD or functionally independent (FI) status.
RESULTS: Of the 62,409 patients 991 (1.6%) were FD. Compared to FI patients, those with FD were older (mean age, 72.7 vs. 59.1 years, p < 0.001), with higher comorbidities and more unplanned open procedures (7

We report a case of complete descending colon obstruction due to diverticular disease that was initially managed by endoscopic stent placement followed by single-stage left colectomy with primary anastomosis. Traditional management of complete large bowel obstruction, whether due to benign or malignant disease, most often requires a temporary colostomy because of unprepared colon. In this case, preparation of the colon was accomplished by successful stenting of the benign colonic obstruction. We believe that endoscopic colonic stenting is an effective way of avoiding a temporary colostomy in p

The sigmoid colon is the part of the large intestine which is most commonly involved in diverticular disease due to its anatomical properties. True diverticula containing all layers of the colonic wall are rare. The majority of diverticula found are pseudodiverticula. They are herniations of mucosa and submucosa through clefts in the colonic muscle layers as a result of morphologic changes of the colonic wall and an imbalance between the intraluminal pressure and the pressure in the abdominal cavity. Microperforations of the tip of the diverticula are an important factor leading to inflammatio

BACKGROUND: Single-incision laparoscopic colectomy is evolving as a safe and feasible approach for the management of various diseases of the colon and rectum. The modality affords completion of "scarless" surgery through a transumbilical incision; however, this approach is associated with several limitations when performing colorectal procedures involving the pelvis. Collinear alignment of the camera and instruments through a single umbilical incision results in restricted visualization, inadequate dissection and mobilization, and the potential for inadvertent injury. We have developed an inno

Acute epiploic appendagitis most commonly manifests with acute lower quadrant pain. Its clinical features are similar to those of acute diverticulitis or, less commonly, acute appendicitis. The conditions that may mimic acute epiploic appendagitis at computed tomography (CT) include acute omental infarction, mesenteric panniculitis, fat-containing tumor, and primary and secondary acute inflammatory processes in the large bowel (eg, diverticulitis and appendicitis). Whereas the location of acute epiploic appendagitis is most commonly adjacent to the sigmoid colon, acute omental infarction is ty

Actinomycosis is an uncommon entity, caused by an anaerobic bacterium, Actinomyces israelii, which is a component of the human oral and gastrointestinal flora. The cervicofacial region is the commonest site of disease, and the abdomen is the second commonest. In this situation the disease is almost always unifocal and restricted to the right colon, especially to the cecum. We report here the case of a patient with a very rare form of this entity, characterized by multiple foci of abdominal involvement with the most severe lesions localized in the transverse and sigmoid colon. The clinical pres

Interleukin-6 blockade can blunt inflammatory biomarkers and mask serious infection. A 49-year-old woman with neuromyelitis optica spectrum disorder receiving satralizumab presented with mild abdominal pain; C-reactive protein levels were normal at the time of perforation despite a transient elevation early during hospitalization. Computed tomography revealed colonic diverticular perforation with a localized abscess. She improved with percutaneous drainage, targeted antimicrobials, and withdrawal of satralizumab, followed by transition to ravulizumab. This case suggests that satralizumab may o

We have reported the cases of two patients who had acute pylephlebitis associated with portal vein thrombosis and septic hepatic emboli as a result of right colonic diverticulitis. Although rare, pylephlebitis is a treatable but often lethal complication of intra-abdominal sepsis. Several bacterial pathogens, especially Escherichia coli are associated with pylephlebitis. Early suspicion and prompt antibiotic therapy can lead to resolution of portal vein thrombosis and hepatic abscess formation, resulting in full recovery for the patient. Surgery may not be required. Our two patients received a
