Nonsteroidal anti-inflammatory drugs (NSAIDs) induce gastric ulcers due to inhibition of prostaglandin production. In view of increasingly accurate noninvasive diagnostic imaging modalities for pancreatic diseases, endoscopic retrograde cholangiopancreatography (ERCP) should be mainly restricted to therapeutic indications. Acute pancreatitis is still the most common complication of ERCP. Prevention measures should focus in particular on well-defined risk groups. Temporary pancreatic duct drainage, preferably using small-diameter endoprostheses, can reduce the incidence of post-ERCP pancreatitis in at-risk individuals. BACKGROUND: An ectopic opening of the common bile duct in the duodenal bulb is extremely rare, and the clinical significance of this anomaly has not been clarified. This study analyzed the clinical implications and cholangiographic findings of this anomaly.
METHODS: A total of 18 patients (15 men, 3 women; median age, 51 years) with an ectopic opening of the common bile duct in the duodenal bulb were studied. Medical records, endoscopic findings, and cholangiographic and other radiographic findings were reviewed.

A 60-year-old patient with a history of chronic pancreatitis and insulin-dependent diabetes was admitted to our hospital with a deeply excavated duodenal ulcer showing no signs of regression under 4-week parenteral nutrition and proton pump inhibitor therapy. Radiologic and endoscopic diagnostics could demonstrate a primary tumor of the pancreatic head penetrating into the duodenal lumen. After surgical treatment by pylorus-preserving pancreatoduodenectomy an abscessing intraductal papillary-mucinous neoplasm of the pancreas was established morphologically.

The last 105 endoscopic sphincterotomies performed between 1983 and 1987 are evaluated and compared with the recent data of the literature. The sphincterotomy has been successful in 98 cases (93%). The indications were: 95 cases of common bile duct stones and 10 inflammatory stenoses of the duodenal papilla. Four patients with recurrent episodes of acute pancreatitis, despite surgical sphincteroplasty in three and duodenoscopic sphincterotomy in one, are described. All patients were diagnosed by endoscopic pancreatic sphincter manometry confirming the diagnosis of a hypertensive pancreatic sphincter. Hydrostatic balloon dilation of the pancreatic sphincter was successfully employed as a single therapeutic modality. It is likely that more widespread use of these techniques will lead to an increase in the reported incidence and to a nonsurgical treatment in a condition previously thought to be rare.
