E-health systems for management of MDR-TB in resource-poor environments: a decade of experience and recommendations for future work.

INTRODUCTION: Multi-drug resistant TB (MDR-TB) is a complex infectious disease that is a growing threat to global health. It requires lengthy treatment with multiple drugs and specialized laboratory testing. To effectively scale up treatment to thousands of patients requires good information systems to support clinical care, reporting, drug forecasting, supply chain management and monitoring. METHODS: Over the last decade we have developed the PIH-EMR electronic medical record system, and subsequently OpenMRS-TB, to support the treatment of MDR-TB in Peru, Haiti, Pakistan, and other resource-

A retrospective single-site data-linkage study comparing manual to electronic data abstraction for routine post-operative nausea and vomiting audit.

BACKGROUND: Post-operative nausea and vomiting (PONV) is a common cause of patient dissatisfaction following anaesthesia. Audit of adherence to PONV prevention guidelines is resource intensive when performed by manual chart extraction. Electronic audit can require costly anaesthetic and medical records. OBJECTIVE: In our single-site study we sought to compare manual and electronic PONV audits by utilizing existing non-anaesthetic electronic medical records to avoid expensive additional software. METHODS: The audits were performed from 13 January 2020 to 1 February 2020 for surgical inpatient

Promoting Re-engagement in HIV Care after Emergency Department Visit by Leveraging Clinical Informatics at a Southern Academic Medical Center.

Retaining persons with HIV (PWH) in HIV care and ensuring access to antiretroviral therapy are crucial for reducing HIV transmission and enhancing health outcomes. HIV care engagement rates in the United States have plateaued over the last decade, indicating the need for innovative re-engagement strategies. We developed an automated electronic health record-based alert system to identify out-of-care (OOC) PWH presenting to any emergency department (ED) within the Duke University Health System. OOC was defined as no HIV care clinical visit in over 12 months. Automated alerts were processed by t

[Electronic fetal monitoring and management of adverse outcomes: how to perform and improve a training program for clinicians?].

Electronic fetal monitoring during labor is the most commonly used method to evaluate the fetal status, but it remains exposed to some criticism. By comparison with intermittent auscultation and in the light of the results of the great studies in the last 30 years, it may be accused its failure to improve the neonatal outcome and its responsibility in the increase on operative deliveries. Actually, the electronic fetal monitoring is a tool whose effectiveness is linked to the accuracy of the analysis developed by the clinician. Studies on assessment of the tracing interpretation indicate that

A prospective study of electrical cardioversion for sustained tachycardias by emergency unit personnel.

Acute symptomatic tachyarrhythmias are commonly seen by emergency unit personnel. Electrical cardioversion is often used at Groote Schuur Hospital to treat such patients because of concerns about the safety and efficacy of intravenous anti-arrhythmic agents. All patients presenting with acute symptomatic tachyarrhythmias who were managed only by the staff of the Emergency Unit were entered into the study to assess the efficacy and safety of direct current (DC) cardioversion. Those with sinus tachycardia or atrial fibrillation of more than 24 hours' duration were excluded. Staff, on joining the

Medication Exposure in Highly Adherent Psychiatry Patients.

Medication exposure is dependent upon many factors, the single most important being if the patient took the prescribed medication as indicated. To assess medication exposure for psychotropic and other medication classes, we enrolled 115 highly adherent psychiatry patients prescribed five or more medications. In these patients, we measured 21 psychotropic and 38 nonpsychotropic medications comprising a 59 medication multiplex assay panel. Strict enrollment criteria and reconciliation of the electronic health record medication list prior to study initiation produced a patient cohort that was adh

Implementation and Evaluation of an Alternative Electronic Health Record Tool for Ordering Blood Products in Pediatric Oncology and Stem Cell Transplantation: Mixed Methods Analysis.

BACKGROUND: Repeated blood product ordering is associated with order entry errors and potential patient harm. Traditional electronic health record order sets require repeated re-entry for recurrent transfusions, creating inefficiencies and opportunities for error, and contributing to physician burnout. Historically, we have used order sets to order blood products, which must be re-entered each time a transfusion is needed. Reusable transfusion therapy plans may address these challenges by standardizing and streamlining transfusion workflows. We conducted a pre-post study at a single pediatric

Design of a mechanical clutch-based needle-insertion device.

Insertion of trocars, needles, and catheters into unintended tissues or tissue compartments results in hundreds of thousands of complications annually. Current methods for blood vessel cannulation or epidural, chest tube, and initial trocar placement often involve the blind pass of a needle through several layers of tissue and generally rely on distinguishable anatomic landmarks and a high degree of clinical skill. To address this simply and without the use of electronics, a purely mechanical clutch system was developed for use in medical devices that access tissue and tissue compartments. Thi

MUSiK: An Open Source Simulation Library for 3D Multi-View Ultrasound.

Diagnostic ultrasound has long filled a crucial niche in medical imaging thanks to its portability, affordability, and favorable safety profile. Now, multi-view hardware and deep-learning-based image reconstruction algorithms promise to extend this niche to increasingly sophisticated applications, such as volume rendering and long-term organ monitoring. However, progress on these fronts is impeded by the complexities of ultrasound electronics and by the scarcity of high-fidelity radiofrequency data. Evidently, there is a critical need for tools that enable rapid ultrasound prototyping and gene

An In-Silico Testbed for Fast and Accurate MR Labeling of Orthopaedic Implants.

One limitation on the ability to monitor health in older adults using Magnetic Resonance (MR) imaging is the presence of implants, where the prevalence of implantable devices (orthopedic, cardiac, neuromodulation) increases in the population, as does the pervasiveness of conditions requiring MRI studies for diagnosis (musculoskeletal diseases, infections, or cancer). The present study describes a novel multiphysics implant modeling testbed using the following approaches with two examples: - an in-silico human model based on the widely available Visible Human Project (VHP) cryo-section dataset;
