Ocular injuries caused by airgun pellets: an analysis of 105 cases.

One hundred and five cases of airgun pellet injuries of the eye have been analysed. The average age was 14 and the male to female ratio 7.5:1. Forty-five patients had final visual acuities of 6/18 or less in the injured eye and 19 of these had the eye removed. The two main causes for poor vision were retinal damage and cataract. One patient became completely blind after sympathetic ophthalmitis arising 11 years after the injury. The pellet lodged in the orbit in 14 cases and three of these, together with one who had an intraethmoidal pellet, were investigated for abnormal lead levels in blood

Hyperconnective and hypoconnective cortical and subcortical functional networks in multiple system atrophy.

INTRODUCTION: In multiple system atrophy (MSA), the organization of the functional brain connectivity within cortical and subcortical networks and its clinical correlates remains to be investigated. METHODS: Whole-brain based 'resting-state' fMRI data were obtained from 22 MSA patients (11 MSA-C, 11 MSA-P) and 22 matched healthy controls, together with standardized clinical assessment and video-oculographic recordings (EyeLink®). RESULTS: MSA patients vs. controls showed significantly higher ponto-cerebellar functional connectivity and lower default mode network connectivity (p < .05, correc

Mild hypohydration increases the frequency of driver errors during a prolonged, monotonous driving task.

UNLABELLED: The aim of the present study was to examine the effect of mild hypohydration on performance during a prolonged, monotonous driving task. METHODS: Eleven healthy males (age 22±4y) were instructed to consume a volume of fluid in line with published guidelines (HYD trial) or 25% of this intake (FR trial) in a crossover manner. Participants came to the laboratory the following morning after an overnight fast. One hour following a standard breakfast, a 120min driving simulation task began. Driver errors, including instances of lane drifting or late breaking, EEG and heart rate were rec

Gamma activity modulated by picture and auditory naming tasks: intracranial recording in patients with focal epilepsy.

OBJECTIVE: We measured the spatial, temporal and developmental patterns of gamma activity augmented by picture- and auditory-naming tasks and determined the clinical significance of naming-related gamma-augmentation. METHODS: We studied 56 epileptic patients (age: 4-56 years) who underwent extraoperative electrocorticography. The picture-naming task consisted of naming of a visually-presented object; the auditory-naming task consisted of answering an auditorily-presented sentence question. RESULTS: Naming-related gamma-augmentation at 50-120 Hz involved the modality-specific sensory cortices

Saccade induced cortical activation in patients with post-stroke visual field defects.

Substantial disability in patients with hemianopia results from reduced visual perception. Several studies have shown that these patients have impaired saccades but may improve search strategies with appropriate training of saccades. We used fMRI to study the representation of saccades in patients with post-stroke hemianopia to the left. Brain activation during visually guided saccades was measured in 10 patients with a pure occipital cortical lesion causing homonymous hemianopia and in 10 healthy control subjects. Differences in activation between rest and saccades and between controls and pa

Effects of periodic leg movements during sleep in middle-aged subjects without sleep complaints.

Recent reports have called into question the relevance of periodic leg movements during sleep disorder (PLMSD) as a specific clinical entity. Because periodic leg movement in sleep index (PLMSI) increases with age, it has become an important exclusion criterion in research on aging. However, it is unknown if PLMSI is related to sleep quality in middle-aged subjects without sleep complaints. The sleep of 70 healthy, middle-aged subjects (age 40 to 60 years) without sleep complaints was evaluated. Subjects were divided into two groups according to their PLMSI severity: (1) 43 subjects (28 women,

A report of two cases of unilateral vestibulopathie after systemic ototoxic treatment.

PURPOSE OF THE STUDY: To present the cases of two patients that developed unilateral vestibular hypofunction after systemic treatment with ototoxic agents, although they manifested symptoms more typical of bilateral vestibular hypofunction. MATERIAL AND METHODS: The patients were seen after having recovered from their initial illnesses, and both of them denied suffering any spells of vertigo, loss of hearing during the treatment or tinnitus. RESULTS: In both patients, oscillopsia and vestibular ataxia were of varying intensity. Bedside vestibular examination, caloric and rotatory chair testi

The utility of cephalometry with the Muller maneuver in evaluating the upper airway and its surrounding structures in Chinese patients with sleep-disordered breathing.

OBJECTIVE: The objective was to simultaneously measure the size and collapsibility of the upper airway in Chinese patients with sleep-disordered breathing using cephalometry with the Muller maneuver. STUDY DESIGN: Prospective study of 100 consecutive Chinese patients evaluated for sleep-disordered breathing at a sleep center. METHODS: Each patient received overnight polysomnography and two lateral cephalograms (at the end-expiration phase and the Muller maneuver, respectively) to evaluate the upper airway and its surrounding structures (soft palate, tongue, and hyoid bone). After excluding 1

Cataplexy-related neurons in the amygdala of the narcoleptic dog.

The amygdala plays an important role in the interpretation of emotionally significant stimuli and has strong projections to brainstem regions regulating muscle tone and sleep. Cataplexy, a symptom of narcolepsy, is a loss of muscle tone usually triggered by sudden, strong emotions. Extracellular single-unit recordings were carried out in the amygdala of narcoleptic dogs to test the hypothesis that abnormal activity of a subpopulation of amygdala neurons is linked to cataplexy. Of the 218 cells recorded, 31 were sleep active, 78 were active in both waking and rapid-eye-movement sleep, 88 were m

Sigma smooth pursuit eye tracking: constant k values revisited.

Effective sigma tracking, i.e., apparent movement perception when slow eye movements are made across a stationary repetitive pattern under stroboscopic illumination, has been shown to be a function of the distance between sequential stimuli (P(s)) and the flash frequency (f(s)). The relationship between these factors and eye velocity ( V (e)) has been formally specified as V (e)= k P(s)f(s)[deg s(-1)], where it has been argued that the value of k, which defines the rate limit for eye velocity, is normally 1, or exceptionally 2 or 3. However, theoretically the limitations on the maximum value f

Eye movement disorders after frontal eye field lesions in humans.

Eye movements were recorded electro-oculographically in three patients with a small ischemic lesion affecting the left frontal eye field (FEF) and in 12 control subjects. Reflexive visually guided saccades (gap and overlap tasks), antisaccades, predictive saccades, memory-guided saccades, smooth pursuit and optokinetic nystagmus (OKN) were studied in the three patients. Staircase saccades and double step saccades were also studied in one of the three patients. For both leftward and rightward saccades, latency in the overlap task (but not in the gap task) and that of correct antisaccades and of

Ophthalmologic assessment of young patients with Alport syndrome.

BACKGROUND: Alport syndrome is an X-linked disease affecting basement membrane collagen. It is characterized by nephritis associated with high-tone sensorineural hearing impairment and ophthalmic signs. Although ocular changes have been described in adults, few data exist regarding the incidence of abnormal ocular features in adolescence and childhood. METHODS: Fifteen male and five female patients with Alport syndrome underwent ophthalmologic, audiologic, and nephrologic assessments. All patients studied had hematuria and a positive family history of Alport syndrome. Thirteen patients had a

Infant apnea: polygraphic studies and follow-up monitoring.

Twenty-six infants with unexplained and apparently life-threatening apnea were evaluated clinically, underwent eight- or 12-hour polygraphic recordings during sleep, and were then observed on home monitors. The one exception was an infant who was treated with theophylline and not monitored at home. Clinical evaluation revealed a history of vomiting in 19 infants, gastroesophageal reflux (GER) on esophagram in 19, and subtle neurologic abnormalities in ten. Polygraphic studies revealed questionable EEG abnormalities in nine infants, abnormal rapid eye movement (REM) time in four, prolonged apne

Fundus flavimaculatus and Stargardt's disease.

Of 42 patients studied who had fundus flavimaculatus and Stargardt's disease, most had reduced visual acuity due to an atrophic macular lesion. Family histories were consistent with autosomal-recessive inheritance. In some young patients, the yellow-white flecks developed with time, while in some older patients the flecks faded, pari passau with increasing retinal pigment epithelial atrophy. Fluorescein angiography showed that most white flecks were largely or totally nonfluorescent. Even after the flecks had faded from view, the sites where they had been usually remained nonfluorescent. In pa
