Revisiting ischemia and reperfusion injury as a possible cause of necrotizing enterocolitis: Role of nitric oxide and superoxide dismutase.

BACKGROUND/PURPOSE: The pathogenesis of necrotizing enterocolitis (NEC) is unknown. Ischemia and reperfusion (I/R) injury has been considered a major contributing factor. Nitric oxide (NO) and superoxide dismutases (SODs) have been shown to protect bowel from I/R injury. This study aims to assess (1) the ability of premature intestine to resist I/R injury compared with mature intestine and (2) the possible role of NO and SODs in modulating such response. METHODS: Intestines from 5 groups of rats (n = 6 for each study group) were studied: (1) premature, gestational age 20 days; (2) premature,

Plasma concentration of intestinal- and liver-FABP in neonates suffering from necrotizing enterocolitis and in healthy preterm neonates.

Both early diagnostic and prognostic assessment of the acute abdomen in preterm infants are hampered by the lack of a sensitive and specific parameter for intestinal injury. In this prospective clinical study we wanted to estimate the value of intestinal (I-) and liver (L-) fatty acid binding protein (FABP) in diagnosing necrotizing enterocolitis (NEC). Using highly sensitive and specific sandwich ELISAs which employ recombinant human I- and L-FABP as standard proteins (limit of detection 0.1 ng/ml plasma), the L-FABP concentration (median 7.6 ng/ml) was determined to be about 3 fold that of I

Fulminant necrotizing enterocolitis associated with severe RSV infection in a preterm infant following Nirsevimab prophylaxis: a case report.

Respiratory syncytial virus (RSV) prophylaxis with the monoclonal antibody Nirsevimab has shown high efficacy in preventing severe lower respiratory tract infections in infants. However, the sequences of immunopathological phenomena leading to breakthrough infections and the role of RSV in causing extrapulmonary disease remain poorly understood. We report on a former preterm infant who developed an unusually severe form of necrotizing enterocolitis (NEC) requiring multiple emergency laparotomies and prolonged intensive care treatment in the context of severe RSV-bronchiolitis 22 days after pas

Epidemiology of necrotizing enterocolitis in multi-birth preterm infants: A 22-year retrospective study in Switzerland (2000-2021).

BACKGROUND: Necrotizing enterocolitis (NEC) is a life-threatening gastrointestinal condition predominantly affecting preterm infants. Although multiple births are increasingly common, their specific contribution to NEC risk remains unclear. OBJECTIVE: To assess the incidence and mortality of NEC in preterm multiple-birth infants in Switzerland over two decades (2000-2021), and to determine whether multiplicity contributes to NEC risk. METHODS: We conducted a retrospective cohort study of 15,635 infants born before 32 weeks' gestation, using prospectively collected data from the Swiss Neonata

Sodium butyrate protects against necrotizing enterocolitis through a mechanism involving YB-1-dependent regulation of intestinal epithelial apoptosis.

Due to a lack of complete mechanistic understanding, there are no specific therapeutics for necrotizing enterocolitis (NEC), a deadly gastrointestinal disease affecting premature newborns. While sodium butyrate offers intestinal protection, its efficacy is strictly dose-dependent, and the underlying mechanisms regulating intestinal epithelial cell (IEC) survival remain elusive. Here, using neonatal murine NEC models and human IECs, we demonstrate that moderate-dose butyrate (50 mg/kg) significantly attenuates mucosal injury and improves survival, whereas high-dose butyrate (250 mg/kg) exacerba

The impact of early stoma closure on infants after surgery for necrotizing enterocolitis (NEC): real-world data analysis.

To evaluate the impact of early (< 8 weeks) versus late (≥ 8 weeks) stoma closure on postoperative recovery and complications in preterm infants following surgery for necrotizing enterocolitis (NEC). In this multicenter retrospective cohort study, we analyzed data from preterm infants who underwent stoma closure between 2011 and 2022. Propensity score matching (PSM) was applied in a 1:2 ratio to balance baseline characteristics between early and late closure groups. Outcomes assessed included stoma-related complications, operative characteristics, gastrointestinal recovery, and postoperative c

Empowering parents in neonatal decision making using Q-methodology: Development of a decision guidance framework for necrotizing enterocolitis.

BACKGROUND/OBJECTIVES: Given the high risk of peri-operative mortality and postoperative morbidities in surgical necrotizing enterocolitis (NEC), parents are faced with an extraordinarily difficult and emotionally charged decision. Together with healthcare professionals they must weigh whether pursuing surgery is in the infant's best interest or if palliative care would be more appropriate. The purpose of this study is to explore how parents make this complex decision and to investigate whether Q-methodology could serve as a decision guidance framework for the empowerment of parents. METHODS:

Vitamin E and GPX4 cooperatively protect treg cells from ferroptosis and alleviate intestinal inflammatory damage in necrotizing enterocolitis.

BACKGROUND: The notable decline in the number of Tregs within Necrotizing enterocolitis (NEC) intestinal tissues, contribute to excessive inflammation and necrosis, yet the precise underlying factors remain enigmatic. Ferroptosis, a novel cell death stemming from a disrupted lipid redox metabolism, is the focus of this investigation. Specifically, this study delves into the ferroptosis of Treg cells in the context of NEC and observes the protective effects exerted by vitamin E intervention, which aims to mitigate ferroptosis of Treg cells. METHODS: To investigate the reduction of Treg cells in

Acquired Zinc Deficiency in Preterm Infant Post-Surgery for Necrotizing Enterocolitis (NEC) on Prolonged Total Parenteral Nutrition (TPN).

Zinc (Zn) is a vital trace element that plays a pivotal role in protein synthesis, cellular growth, and differentiation and is involved as a cofactor of metalloenzymes, performing a wide variety of metabolic, immune, and synthesis roles. Zn is required at all stages of an infant's and child's development, and severe Zn deficiency has been reported to lead to slower physical, cognitive, and sexual growth. Preterm neonates are at a higher risk of developing zinc deficiency for a variety of reasons, including low Zn intake from enteral feeds containing breast milk, relative malabsorption due to i

Metagenomic Sequencing with Strain-Level Resolution Implicates Uropathogenic E. coli in Necrotizing Enterocolitis and Mortality in Preterm Infants.

Necrotizing enterocolitis (NEC) afflicts approximately 10% of extremely preterm infants with high fatality. Inappropriate bacterial colonization with Enterobacteriaceae is implicated, but no specific pathogen has been identified. We identify uropathogenic E. coli (UPEC) colonization as a significant risk factor for the development of NEC and subsequent mortality. We describe a large-scale deep shotgun metagenomic sequence analysis of the early intestinal microbiome of 144 preterm and 22 term infants. Using a pan-genomic approach to functionally subtype the E. coli, we identify genes associated

Heme oxygenase-1 deficiency promotes the development of necrotizing enterocolitis-like intestinal injury in a newborn mouse model.

Necrotizing enterocolitis (NEC) is typified by mucosal destruction, which subsequently can lead to intestinal necrosis. Prematurity, enteral feeding, and bacterial colonization are the main risk factors and, combined with other stressors, can cause increased intestinal permeability, injury, and an exaggerated inflammatory response. Heme oxygenase-1 (HO-1) mediates intestinal protection due to anti-inflammatory, antioxidative, and antiapoptotic effects of its products carbon monoxide, biliverdin, and bilirubin. This study investigates a possible role of HO-1 in the pathogenesis of NEC using a n

Feed intolerance and necrotizing enterocolitis in preterm small-for-gestational age neonates with normal umbilical artery Doppler flow.

This was a prospective observational study conducted in a level III neonatal unit in North India to measure the incidence of feed intolerance and necrotizing enterocolitis (NEC) in preterm small for gestational age (SGA) neonates with normal umbilical artery Doppler flow in comparison with gestation matched appropriate for gestational age (AGA) neonates. Fifty consecutive singleton SGA preterms between 28 and 34 weeks gestation with normal Doppler were enrolled and 50 gestation matched AGA served as controls. There was a trend toward more feed intolerance (22% vs. 12%, p = 0.183), NEC (12% vs.

Necrotizing enterocolitis in a 850 gram infant receiving sorbitol-free sodium polystyrene sulfonate (Kayexalate): clinical and histopathologic findings.

We report a 27-week, 850 g infant with severe Streptococcus group B sepsis and life-threatening hyperkalemia due to progressive anuria. On the fourth day of life, after he failed treatment with diuretics, salbutamol, insulin, calcium gluconate and sodium bicarbonate, he was treated with sorbitol-free Kayexalate enemas. Potassium level slowly decreased from 9.2 mmol/l to normal level along with a recovery of normal urine output. On the 11th day of life, clinical and radiological signs of a perforated necrotizing enterocolitis (NEC) occurred and the patient required surgical intestinal resection

Spontaneous, isolated intestinal perforations in neonates with birth weight less than 1,000 g not associated with necrotizing enterocolitis.

From January 1986 through December 1988, we have seen 7 cases of isolated intestinal perforation in 250 infants with birth weights less than 1,000 g (3% incidence) without histological or clinical evidence of necrotizing enterocolitis (NEC). Patients had a mean birth weight of 670 g, gestational age of 25.1 weeks, and sustained a perforation at a chronological age of 10.4 days. No infants had been fed. A definite, blue-discolored abdomen was the only consistent clinical sign (n = 7). Free intraperitoneal air on radiograms was rarely observed (n = 1). Abdominal ultrasounds (n = 3) and metrizami

Transcriptomics-guided serum podoplanin and interpretable machine learning for early risk stratification of necrotizing enterocolitis in preterm infants.

BACKGROUND: To evaluate podoplanin (PDPN) as a circulating biomarker for early necrotizing enterocolitis (NEC) risk stratification and to determine whether an interpretable machine-learning framework integrating PDPN with prediagnostic clinical and inflammatory variables could improve individualized risk estimation. METHODS: PDPN expression was examined in two public NEC intestinal transcriptomic datasets. Serum PDPN was then measured by enzyme-linked immunosorbent assay in a nested case-control cohort of 132 preterm infants (66 with NEC and 66 gestational-age- and birth-weight-matched contro

Mesenchymal stromal cell therapy restores intestinal integrity and attentuates inflammation in a preterm piglet model of necrotizing enterocolitis.

PURPOSE: Necrotizing enterocolitis (NEC) is a life-threatening gastrointestinal disease of prematurity characterized by inflammation, necrosis, and high morbidity. Current therapies are limited, necessitating the development of novel treatments. Mesenchymal stromal cells (MSCs) have shown promise in murine NEC models. Given the anatomical and physiological similarities between premature piglets and human infants, we employed a preterm piglet model to evaluate MSC efficacy. We hypothesized that intraperitoneal MSC administration would reduce intestinal injury in NEC. METHODS: Preterm piglets w

DS0384 Alleviates Necrotizing Enterocolitis: Secretes N-carbamyl glutamic Acid and Participates in Lipid Metabolism and Lipid Peroxidation Processes.

Necrotizing enterocolitis (NEC) is a life-threatening inflammatory bowel disease linked to gut microbiome dysbiosis. This study evaluates the efficacy of Limosilactobacillus reuteri strain DS0384, which secretes N-carbamyl glutamic acid (NCG), in modulating lipid peroxidation and inflammatory pathways in NEC. After pretreatment with DS0384, NEC mouse model was induced by gavage with bacteria-containing formula. NCG levels in the ileum were measured via CE-TOFMS metabolomic analysis. Additionally, rat small intestinal epithelial IEC-6 cells were exposed to lipopolysaccharide (LPS), treated with
