Using genetic epidemiology to study Rett syndrome: the design of a case-control study.

Rett syndrome is a neurological disorder that is seen almost exclusively in females. Although generally considered to have a genetic basis, the underlying mechanism remains obscure. One favoured hypothesis is that the syndrome is an X-linked dominant disorder, lethal or non-expressed in males. Genealogical research has also suggested that the mode of transmission in Rett syndrome may involve a premutation which over several generations is converted to a full mutation. Geographical clustering has been reported, and it has also been proposed that Rett syndrome is a clinically variable condition

Implementing a multi-centric cohort study during the SARS-CoV-2 pandemic in a resource-constrained setting: challenges, adaptive solutions, and lessons from Brazil.

BACKGROUND: Conducting multicenter, longitudinal epidemiological studies on emerging pathogens in real time during health emergencies presents significant challenges, especially in low- and middle-income countries, where substantial logistical and resource constraints hinder the rapid design and implementation of research protocols. This is particularly difficult when the disease spreads rapidly and its nature changes due to the onset of vaccination. METHODS: During the COVID-19 pandemic, a cohort study (AVISA) was conducted in eleven research sites to assess seroprevalence. The objective of t

Understanding occult hepatitis C infection.

BACKGROUND: Occult hepatitis C infection (OCI) is a type of hepatitis C virus (HCV) infection, defined as the presence of HCV RNA in hepatocytes or peripheral blood mononuclear cells (PBMCs) and the absence of HCV RNA in serum. STUDY DESIGN AND METHODS: A literature review was conducted to identify articles that characterized OCI as a disease, including its epidemiology, mode of transmission, pattern of infection, progression, and treatment. RESULTS: OCI patients experience a milder degree of inflammatory and cirrhotic changes than patients with chronic hepatitis C. OCI is transmissible pare

Update on the epidemiology and management of cryptococcal meningitis.

Despite recent improvements in the diagnosis and treatment of cryptococcosis, cryptococcal meningitis is responsible for > 600,000 deaths/year worldwide. The aim of this work is to provide an update on the developments in its epidemiology and management. Understanding the pathogenesis of Cryptococcus has improved, and new insights for the virulence of the fungus and the host response have enabled scientists to design new ways to confront this infection. Additionally, invertebrate model hosts have greatly facilitated the research in this field. Importantly, the epidemiology of Cryptococcus gatt

A potential hidden source of hepatitis C infection among noninjecting drug users.

Hepatitis C virus (HCV) is a major cause of chronic liver disease in the United States and worldwide. It is primarily transmitted through blood-to-blood contact with an infected individual. HCV is hyperendemic among injection drug users (IDUs), who contract the virus through contaminated syringes and drug preparation equipment shared with other IDUs. The prevalence of HCV is also high, to a lesser degree, among noninjection drug users, many of whom report no identifiable HCV risk exposures. This article reviews the epidemiological and virological evidence bearing on a potential hidden source o

[Tuberculosis focused through the molecular biology zoom].

The sequence of Mycobacterium tuberculosis genome set up a process of systematic research and improved the understanding of the microorganism biology, albeit the clues of its virulence and pathogenicity still remain rather cryptic. Genetic engineering did not succeed in designing more effective or convenient tools to accomplish the control of tuberculosis. Neither the bacillus variants created by mutagenesis and recombination nor the microorganism subunits (antigens, DNA) proved to be significantly better than the BCG vaccine as immunogens. Likewise, the search for novel bacterial targets and

A suitable drug structure for interaction with SARS-CoV-2 main protease between boceprevir, masitinib and rupintrivir; a molecular dynamics study.

In recent years, more than 200 countries of the world have faced a health crisis due to the epidemiological disease of COVID-19 caused by the SARS-CoV-2 virus. It had a huge impact on the world economy and the global health sector. Researchers are studying the design and discovery of drugs that can inhibit SARS-CoV-2. The main protease of SARS-CoV-2 is an attractive target for the study of antiviral drugs against coronavirus diseases. According to the docking results, binding energy for boceprevir, masitinib and rupintrivir with CMP are -10.80, -9.39, and -9.51 kcal/mol respectively. Also, for

One Health for fascioliasis control in human endemic areas.

Fasciola hepatica and F. gigantica are liver flukes causing fascioliasis, a worldwide zoonotic, complex disease. Human infection/reinfection occurs in endemic areas where preventive chemotherapy is applied, because of fasciolid transmission ensured by livestock and lymnaeid snail vectors. A One Health control action is the best complement to decrease infection risk. The multidisciplinary framework needs to focus on freshwater transmission foci and their environment, lymnaeids, mammal reservoirs, and inhabitant infection, ethnography and housing. Local epidemiological and transmission knowledge

The CAST (Childhood Asperger Syndrome Test): test accuracy.

The Childhood Asperger Syndrome Test (CAST) is a parental questionnaire to screen for autism spectrum conditions. In this validation study, the CAST was distributed to 1925 children aged 5-11 in mainstream Cambridgeshire schools. A sample of participants received a full diagnostic assessment, conducted blind to screen status. The sensitivity of the CAST, at a designated cut-point of 15, was 100 percent, the specificity was 97 percent and the positive predictive value was 50 percent, using the group's consensus diagnosis as the gold standard. The accuracy indices varied with the case definition

Impact of host genetics on susceptibility to human Chlamydia trachomatis disease.

Evidence that host genetic factors play a major role in susceptibility or resistance to many infectious diseases is increasing, due to major advances in genetic epidemiological methodology. Recent human genome mapping information and the identification of a large number of candidate genes provide the tools for such studies. The information obtained is important for understanding the pathogenesis of disease and for the design of preventive and therapeutic strategies. In the study of Chlamydia trachomatis disease pathogenesis, much research focuses on how bacterial factors modulate the immune re

Occupational heat exposure and male fertility: a review.

In humans, as in most mammals, spermatogenesis is temperature dependent. This temperature dependence has been clearly demonstrated by several experimental studies showing that artificial increases in scrotum or testicle temperature in fertile men reduce both sperm output and quality. Our knowledge of the effects of occupational heat exposure on male fertility comes mostly from a small number of epidemiological studies. We conducted an extensive review of these published reports, focusing on methodology and design (retrospective or prospective; reference group; number of subjects) and principal

Boosting understanding of Lassa Fever virus epidemiology: Field testing a novel assay to identify past Lassa Fever virus infection in blood and oral fluids of survivors and unexposed controls in Sierra Leone.

BACKGROUND: Despite identification 50 years ago, the true burden of Lassa Fever (LF) across Africa remains undefined for reasons including research focus on hospitalised patients, lack of validated field-feasible tools which reliably identify past infection, and the fact that all assays require blood samples making large-scale surveys difficult. Designated a priority pathogen of epidemic potential requiring urgent research by the World Health Organisation, a better understanding of LF sero-epidemiology is essential to developing and evaluating new interventions including vaccines. We describe

Persistent Endocrine-Disrupting Chemicals and Fatty Liver Disease.

PURPOSE OF REVIEW: Non-alcoholic fatty liver disease (NAFLD) is the most prominent chronic liver disease in Western countries, affecting approximately 25% of the population worldwide. Sex-specific differences in the development of NAFLD are apparent. While obesity and insulin resistance are major contributors to the increasing prevalence of NAFLD, a growing body of literature suggests that exposure to persistent endocrine-disrupting chemicals (pEDCs) may also play a role. This review summarizes recent (2011 and later) scientific literature investigating exposures to pEDCs, specifically persist

Injury epidemiology after the 2001 Gujarat earthquake in India: a retrospective analysis of injuries treated at a rural hospital in the Kutch district immediately after the disaster.

BACKGROUND: The number of injured far exceeds those dead and the average injury to mortality ratio in earthquakes stands at 3:1. Immediate effective medical response significantly influences injury outcomes and thus the overall health impact of earthquakes. Inadequate or mismanagement of injuries may lead to disabilities. The lack of precise data from immediate aftermath is seen as a remarkable weak point in disaster epidemiology and warrants evidence generation. OBJECTIVE: To analyze the epidemiology of injuries and the treatment imparted at a secondary rural hospital in the Kutch district,

Critical review of the epidemiological literature on occupational exposure to perchloroethylene and cancer.

OBJECTIVES: Of an estimated 500,000 workers in the USA potentially exposed to perchloroethylene (PCE), the largest share is employed in the dry-cleaning industry. PCE, a non-flammable solvent, has commercial applications as a chemical intermediate, metal degreaser and, since the 1950s, primary solvent in the dry-cleaning industry. The International Agency for Research on Cancer (IARC) currently finds sufficient evidence to designate PCE as carcinogenic in animals, with limited evidence in humans. With regard to occupational exposure through dry-cleaning, PCE is considered to be possibly carcin

Bridging disciplinary siloes: a scoping review on the inclusion and exclusion of pregnant and lactating populations in clinical research.

OBJECTIVES: Despite calls for including pregnant and lactating people in clinical research, exclusionary practices persist. Current arguments for inclusion are fragmented and discipline-specific. This scoping review synthesised how inclusion and exclusion of pregnant and lactating populations in clinical research is framed across fields, and identified recommendations for responsible inclusion. DESIGN: Scoping review. DATA SOURCES: We searched eight databases from inception to 14 February 2024: MEDLINE, CINAHL, Family & Society Studies Worldwide, SocINDEX, Scopus, Web of Science, Embase and

Comparing National Institutes of Health funding for cancer survivorship: a spotlight on breast and gynecologic cancer.

BACKGROUND: Cancer survivorship in the United States is rapidly rising, with a projected 50% increase by 2050, driven by improvements in treatment and increased incidence of cancers such as endometrial cancer. Along with this growth, long-term side effects create significant burdens, yet survivorship research, particularly for female-organ cancers, remains limited and imbalanced - with breast cancer studies far outnumber those for gynecologic cancers. To understand these disparities, funding patterns for survivorship research between disease sites, while accounting for differences in disease p
