We report two cases with severe skin reactions following oral terbinafine (Lamisil) therapy. The first case was a 49-year-old woman with ony … erbinafine 250 mg once daily. Despite this eruption she continued treatment for 6 days. Generalized erythema multiforme developed in the following days. Terbinafine is a recently introduced efficacious fungicidal drug. This is the first report of toxic epidermal necrolysis following terbinafine.

33 patients suffering from postherpetic erythema multiforme (PEM) were investigated with special reference to histocompatibility typing. In comparison with a control group (n = 54), we found strikingly high levels of HLA-DR1 and HLA-DR4 as well as a significantly elevated level of HLA-Cw3 in our patients (p less than 0.01).

Erythema multiforme (EM) is an inflammatory disorder of the skin, which may include mucous membrane involvement, that features target (iris) lesions. Mediators specifically involved in EM are not well characterized; its pathogenesis remains enigmatic. In this study, evidence for participation of kinins in the pathophysiology of inflammation in EM was investigated by assessing cleavage of high-molecular-weight kininogen (HMWK) in plasma. Eighty-six cases of erythema multiforme (EM) were studied based on the distribution of skin lesions. Twenty-nine patients had distinct facial skin lesions as well as lesions on the extremities (Group A) and 57 patients had skin lesions only on the extremities (Group B). Patients in Group A were younger than those in Group B; the average ages being 7.5 and 25.2 years, respectively. We have reported a case of severe erythema multiforme caused by outbreaks of recurrent HSV-2 infection. The EM was debilitating and responded to the treatment of HSV with oral acyclovir. On stopping the oral acyclovir, the HSV and the EM both recurred. We recommend a trial of oral acyclovir in cases of severe EM caused by recurrent HSV.

A patient who presented with erythema multiforme on clinical and histological examination but who subsequently developed fatal, fulminant systemic lupus erythematosus is reported. The prognostic significance of the development of erythema multiforme-type lesions in lupus erythematosus is discussed with reference to cases reported by Rowell et al. and Provost et al. The term 'Rowell's syndrome' is best avoided.
