Comparison of cervical vestibular evoked potentials evoked by air-conducted sound and bone-conducted vibration in vestibular Schwannoma patients.

OBJECTIVE: The vestibular evoked myogenic potential (VEMP) is associated with otolithic afferents and can be used to evaluate the function of the saccule and utricle. In this study, we compared cervical VEMP evoked by stimulation with Air-conducted sound (ACS) and bone-conducted vibration (BCV) to the forehead and investigated whether BCV can be used as a substitute for ACS. METHODS: Data were obtained from 33 patients with vestibular schwannoma. Vestibular examinations were performed preoperatively. VEMP was obtained upon stimulation with ACS (ACS cVEMP) and BCV to the forehead using a minis

Combined recordings of compound nerve action potentials and spinal cord evoked potentials in differential diagnosis of spinal root lesions.

Three cases are presented to demonstrate the diagnostic value of sensory neurography in combination with somatosensory evoked potentials in the diagnosis of proximally located neuropathy and its differentiation to centrally located demyelinating processes. Simultaneous recording of cortical and spinal evoked potentials, as well as peripheral nerve action potentials, revealed in two cases (herpes zoster, Guillain-Barré syndrome) a site of lesion at the spinal roots suggesting radiculitis and in one case (tick bite) a site of lesion central to the source of lumbar evoked potentials suggesting my…

Effective connectivity of the insula as measured by cortico-cortical evoked potentials.

Effective connectivity of the human insula, mainly assessed at rest using cortico-cortical evoked potentials (CCEPs), is not yet fully characterized at high-resolution. Here, we significantly extend prior CCEP studies of the insula by leveraging an extensive multicenter CCEP database and fine-grained anatomical atlases of the insula. We analyzed CCEP datasets from 897 patients with refractory focal epilepsy (459 females, age: 26±14 years) explored by stereo electroencephalography and with at least one electrode contact in the insula. Efferent and afferent effective connectivity measures of nin…

Optical Cortico-Cortical Evoked Potentials Reveal Structural and State-Dependent Cortical Communication.

BACKGROUND: Cortico-cortical evoked potentials probe causal cortical communication in humans; however, establishing a preclinical methodological counterpart that combines cortex-wide coverage, high temporal resolution, minimal invasiveness, and awake-state recording remains challenging. OBJECTIVE: To map causal cortical communication and determine how local excitability, anatomical connectivity, and brain state jointly shape signal propagation across the cortex. METHODS: We used an all-optical platform that combines single-pulse transcranial optogenetic stimulation with widefield calcium ima…

Separation of luminance and contrast modulation in steady-state visual evoked potentials.

Neurons in the retina and early visual cortex respond primarily to local luminance contrast rather than overall luminance energy. The distinction between luminance and contrast processing is revealed in its most striking form by the contrast asynchrony paradigm: two discs with bright and dark surrounds modulate in luminance. When the discs modulate at 3-6 Hz, there is a percept of antiphase flicker even though the luminance modulation of the patches is in phase. To establish the neural basis of this perceptual phenomenon, we conducted a study using steady-state visual evoked potentials (SSVEPs…

Motor evoked potentials as a side effect biomarker for deep brain stimulation.

OBJECTIVES: To determine if motor evoked potentials (MEP) - stimulation-induced muscle activation measured using electromyography - can serve as a biomarker of corticobulbar (CBT) and corticospinal (CST) tract activation induced by deep brain stimulation (DBS). METHODS: In 12 patients with Parkinson's disease and subthalamic (n = 10) or pallidal (n = 2) DBS, contact mapping determined clinical motor side effect thresholds. For equivalent stimulation parameters (except lower frequency), EMG was recorded from cranial and arm muscles to determine the presence, peak amplitudes and latencies of ME…

Validating genuine changes in heartbeat-evoked potentials using pseudotrials and surrogate procedures.

The brain continuously receives interoceptive information about the state and function of our internal organs. For instance, each time the heart beats, the brain responds by generating time-locked activity, known as heartbeat-evoked potentials (HEP). When investigating HEPs, it is essential to adequately control for heartbeat-independent confounding activity to avoid false interpretations. In the present study, we highlight the pitfalls of uncontrolled analyses and advocate for the use of surrogate heartbeat analysis and pseudotrial correction, which are promising tools to control for spurious…

Cortical hyperexcitability in corticobasal syndrome as revealed by giant somatosensory evoked potentials.

OBJECTIVE: Corticobasal syndrome (CBS) is characterized by asymmetric symptoms, including myoclonus. Few studies have reported the observation of giant somatosensory evoked potentials (SEPs) in CBS patients. We aimed to clarify the origin and characteristics of myoclonus by examining the relationships among clinical symptoms and electrophysiological and brain imaging findings. METHODS: We conducted a cross-sectional study of 23 patients with CBS who underwent SEPs at Kyoto University Hospital from 2008 to 2022. We compared SEPs, cortical reflex (C-reflex), and cerebral perfusion in the precen…

Frequency-dependent modulation of evoked potentials during GPi-DBS in pediatric dystonia.

Deep brain stimulation (DBS) of the globus pallidus internus (GPi) is an established treatment for dystonia, yet the neurophysiological mechanisms underlying frequency-dependent effects remain poorly understood. In this study, we investigate how different GPi-DBS frequencies modulate evoked potential (EP) characteristics within the basal ganglia-thalamo-cortical network in pediatric dystonia patients. We evaluated how increasing stimulation frequency alters EP morphology, focusing on peak-to-peak amplitude (PPa), peak-to-peak duration (PPd), and time to peak (TP). This analysis was motivated b

Chromatic visual evoked potentials: A review of physiology, methods and clinical applications.

Objective assessment of the visual system can be performed electrophysiologically using the visual evoked potential (VEP). In many clinical circumstances, this is performed using high contrast achromatic patterns or diffuse flash stimuli. These methods are clinically valuable but they may only assess a subset of possible physiological circuitries within the visual system, particularly those involved in achromatic (luminance) processing. The use of chromatic VEPs (cVEPs) in addition to standard VEPs can inform us of the function or dysfunction of chromatic pathways. The chromatic VEP has been w

Single-trial visually evoked potentials predict both individual choice and market outcomes.

A central assumption in the behavioral sciences is that choice behavior generalizes enough across individuals that measurements from a sampled group can predict the behavior of the population. Following from this assumption, the unit of behavioral sampling or measurement for most neuroimaging studies is the individual; however, cognitive neuroscience is increasingly acknowledging a dissociation between neural activity that predicts individual behavior and that which predicts the average or aggregate behavior of the population suggesting a greater importance of individual differences than is ty

TMS-evoked EEG potentials demonstrate altered cortical excitability in migraine with aura.

Migraine is associated with altered sensory processing, that may be evident as changes in cortical responsivity due to altered excitability, especially in migraine with aura. Cortical excitability can be directly assessed by combining transcranial magnetic stimulation with electroencephalography (TMS-EEG). We measured TMS evoked potential (TEP) amplitude and response consistency as these measures have been linked to cortical excitability but were not yet reported in migraine. We recorded 64-channel EEG during single-pulse TMS on the vertex interictally in 10 people with migraine with aura and 1

A Novel Approach to Screen for Somatosensory Evoked Potentials in Critical Care.

BACKGROUND: Somatosensory evoked potentials (SSEPs) help prognostication, particularly in patients with diffuse brain injury. However, use of SSEP is limited in critical care. We propose a novel, low-cost approach allowing acquisition of screening SSEP using widely available intensive care unit (ICU) equipment, specifically a peripheral "train-of-four" stimulator and standard electroencephalograph. METHODS: The median nerve was stimulated using a train-of-four stimulator, and a standard 21-channel electroencephalograph was recorded to generate the screening SSEP. Generation of the SSEP was su…

Electroacupuncture Promotes Neuroplasticity of Central Auditory Pathway: An Auditory Evoked Potentials Study.

Our previous studies found that electroacupuncture at the right Zhongzhu acupoint (TE3) can enhance auditory recovery in rats with noise-induced hearing loss. Here, we investigated the changes in auditory brainstem response (ABR) and long late latency (LLR) evoked potential to explain the mechanisms of electroacupuncture at TE3. The auditory evoked potentials were recorded, including ABR and LLR, at baseline and on day 3 (D3), D5, and D8 after baseline. The 2-Hz electroacupuncture at the right TE3 was applied on D3, D4, and D5 in the electroacupuncture group but not in the control group. In AB…

Respiratory-related evoked potentials in chronic obstructive pulmonary disease and healthy aging.

Altered neural processing and increased respiratory sensations have been reported in chronic obstructive pulmonary disease (COPD) as larger respiratory-related evoked potentials (RREPs), but the effect of healthy-aging has not been considered adequately. We tested RREPs evoked by brief airway occlusions in 10 participants with moderate-to-severe COPD, 11 age-matched controls (AMC) and 14 young controls (YC), with similar airway occlusion pressure stimuli across groups. Mean age was 76 years for COPD and AMC groups, and 30 years for the YC group. Occlusion intensity and unpleasantness was rated…

Evoked potentials as biomarkers of hereditary spastic paraplegias: A case-control study.

INTRODUCTION: The Hereditary Spastic Paraplegias (HSP) are a group of genetic diseases that lead to slow deterioration of locomotion. Clinical scales seem to have low sensitivity in detecting disease progression, making the search for additional biomarkers a paramount task. This study aims to evaluate the role of evoked potentials (EPs) as disease biomarkers of HSPs. METHODS: A single center cross-sectional case-control study was performed, in which 18 individuals with genetic diagnosis of HSP and 21 healthy controls were evaluated. Motor evoked potentials (MEP) obtained with transcranial mag…

Electrophysiological correlates of gist perception: a steady-state visually evoked potentials study.

Gist perception refers to perceiving the substance or general meaning of a scene. To investigate its neuronal mechanisms, we used the steady-state visually evoked potential (SSVEP) method-an evoked oscillatory cortical response at the same frequency as a visual stimulus flickered at this frequency. Two neighboring stimuli were flickered at different frequencies f1 and f2, for example, a drawing of a sun on the left side of the screen flickering at 8.6 Hz and the drawing of a parasol on the right side of the screen flickering at 12 Hz. SSVEPs enabled us to separate the responses to the two dist

Evoked Potentials Reveal Noise Exposure-Related Central Auditory Changes Despite Normal Audiograms.

Purpose Complaints of auditory perceptual deficits, such as tinnitus and difficulty understanding speech in background noise, among individuals with clinically normal audiograms present a perplexing problem for audiologists. One potential explanation for these "hidden" auditory deficits is loss of the synaptic connections between the inner hair cells and their afferent auditory nerve fiber targets, a condition that has been termed cochlear synaptopathy. In animal models, cochlear synaptopathy can occur due to aging or exposure to noise or ototoxic drugs and is associated with reduced auditory
