Temporal behavior of exercise oscillatory ventilation and respiratory muscle performance in patients with heart failure.

BACKGROUND: Exercise oscillatory ventilation (EOV) is a negative prognostic marker in patients with heart failure (HF). EOV can either disappear (D-EOV) or persist (P-EOV) during exercise, with each showing different clinical implications. The relationship between respiratory muscle weakness and EOV persistence is not well understood, and its impact on exercise performance and muscle function in HF patients needs further exploration. METHODS: This study included 98 clinically stable HF patients with reduced left ventricular ejection fraction (LVEF), all undergoing cardiopulmonary exercise tes

Long-term effects of physical training on cardiopulmonary exercise parameters in young patients with congenital heart diseases.

INTRODUCTION: Physical activity is recognised as an important intervention in patients with CHD. However, more data on the actual magnitude of physical training impact on functional capacity in this group of patients are still warranted. We aim to assess effort tolerance in a contemporary cohort of patients with congenital heart disease, regularly following a training programme, in comparison with a matched control group. METHODS: Patients with CHD followed at the sports medicine department, who had undergone cardiopulmonary exercise test between 2011 and 2019, were included. Variables record

Clinical study of exercise on improvement of β-cell function and insulin resistance in non-diabetic young offsprings of diabetic patients.

BACKGROUND: Previous studies have shown that exercise could improve β-cell function in humans or animal models of type 2 diabetes. However, whether it can prevent the progression of pre-diabetes to diabetes remains unclear. AIM: To study the effects of exercise on glycolipid metabolism, β-cell function, and insulin resistance in the non-diabetic young offsprings of diabetic patients. METHODS: One hundred and eighty-two normal glucose tolerance young offsprings of type 2 diabetic parents were enrolled. Individuals with fasting insulin ≥ 12.0 mU/L were assigned to hyperinsulinemia group (n = 7

Impaired cardiac reserve in asymptomatic patients with moderate pulmonary restenosis late after relief of severe pulmonary stenosis: evidence for diastolic dysfunction.

BACKGROUND: Patients with moderate pulmonary valve restenosis late after relief of severe pulmonary stenosis (PS) may show decreased exercise tolerance. To elucidate the mechanism of decreased exercise tolerance, we evaluated cardiac response to physical and pharmacological stress in these patients and compared results with those of patients with native moderate PS. METHODS: Twenty asymptomatic patients with moderate PS were divided into 2 groups: Group I (late after relief of severe PS, n=9), and Group II (no previous intervention, n=11). All patients underwent an exercise test, dobutamine s…

A novel XPNPEP3 gene variant manifesting as rhabdomyolysis and exercise intolerance.

Biallelic mutations in XPNPEP3 gene, encoding a mitochondrial peptidase, mainly cause nephronophthisis, but associated muscle involvement remains poorly described. We report here a 44-year-old male presenting since childhood with exercise intolerance and recurrent rhabdomyolysis. Electroneuromyography revealed a sensory axonal neuropathy and brain MRI showed white matter lesions in the posterior cranial fossa. Muscle biopsy revealed ragged-red fibers, COX negative fibers and abnormal mitochondria in electron microscopy. Whole genome sequencing identified a homozygous frameshift variant in the

The physiology of ice hockey performance: An update.

Ice hockey is an intense team sport characterized by repeated bursts of fast-paced skating, rapid changes in speed and direction and frequen … bated fatigue during the final stages of a game, whereas multiple factors likely interact to impair exercise tolerance during each shift. This includes pronounced PCr degradation, with potential inadequate resynthesis in a proportion of fast-twitch fibers in situations of repeated intense actions. Finally, the recovery pattern is inadequately described, but seems le …

Exercise Intolerance in Older Adults With Heart Failure With Preserved Ejection Fraction: JACC State-of-the-Art Review.

Exercise intolerance (EI) is the primary manifestation of chronic heart failure with preserved ejection fraction (HFpEF), the most common form of heart failure among older individuals. The recent recognition that HFpEF is likely a systemic, multiorgan disorder that shares characteristics with other common, difficult-to-treat, aging-related disorders suggests that novel insights may be gained from combining knowledge and concepts from aging and cardiovascular disease disciplines. This state-of-the-art review is based on the outcomes of a National Institute of Aging-sponsored working group meeti

[Value and limitations of sport in cardiac patients].

Regular physical exercise increases effort capacity, the quality of life, and improves symptoms in all cardiac patients. In addition, and much more importantly, all studies show a close correlation between exercise and the reduction of mortality and morbidity with a beneficial effect on the progression of atherosclerosis. This relationship is particularly well documented in patients with coronary artery disease. Exercise has a beneficial effect due to its action on all the principal coronary risk factors, an endothelial dysfunction, coagulability and prevention of thrombosis and an autonomic n

[Exercise intolerance caused by muscular phosphorylase kinase deficiency. Contribution of in vivo metabolic studies].

A 33 year old man has been presenting since childhood an exertional muscle pain syndrome without myoglobinuria. Muscle biopsy revealed a vacuolar myopathy with glycogen excess in subsarcolemmal and intermyofibrillar spaces which was confirmed by electron microscopy. Plasma production of ammonia was abnormally high during exercise on a bicycle ergometer while the raise of lactate was normal. NMR spectroscopy showed an increased muscle glycogen content, with a slight and delayed drop of the pH during exercise. Phosphorylase b kinase activity was undetectable in muscle specimen whereas activities

Effects of exercise therapy on chemotherapy delivery and response in primary breast cancer: A secondary analysis of a randomized trial.

BACKGROUND: Whether structured exercise therapy improves chemotherapy delivery, tolerability, and tumor response is unclear. METHODS: This was a secondary analysis of a phase 2 trial investigating exercise therapy (n = 72) versus usual care (n = 72) in patients with primary breast cancer. Exercise therapy comprised individualized treadmill walking three times weekly for 20-50 minutes per session at 55%-100% of pretreatment exercise capacity. Chemotherapy delivery was assessed according to the relative dose intensity (RDI), tolerability was assessed according to patient-reported outcomes and b

Supraspinal fatigue after normoxic and hypoxic exercise in humans.

Inadequate cerebral O₂ availability has been proposed to be an important contributing factor to the development of central fatigue during strenuous exercise. Here we tested the hypothesis that supraspinal processes of fatigue would be increased after locomotor exercise in acute hypoxia compared to normoxia, and that such change would be related to reductions in cerebral O₂ delivery and tissue oxygenation. Nine endurance-trained cyclists completed three constant-load cycling exercise trials at ∼80% of maximal work rate: (1) to the limit of tolerance in acute hypoxia; (2) for the same duration b

Cardiopulmonary exercise testing in male adolescent patients with thalassemia major after blood transfusion.

BACKGROUND: Thalassemia major (TM) is characterized by anemia, and a synthesis defect occurs in both beta globin genes on the short arm of chromosome 11. Thalassemia patients suffer fatigue, and decreases in VO2max and aerobic exercise capacity are observed. Cardiopulmonary exercise test (CPET) data are valuable for interpreting the responses of the cardiovascular, respiratory, and locomotor systems to exercise. The main purpose of our study was to reveal the effects of transfusion on respiratory parameters, oxygen uptake, the ventilatory threshold, and maximal exercise capacity in the adolesc
