Enhanced Recovery After Surgery Protocols in Craniofacial Surgery.

Enhanced Recovery after Surgery (ERAS) refers to a patient centered, multidisciplinary team developed pathway aimed at reducing the surgical stress response and facilitating expedited patient postoperative recovery. These protocols have been largely developed in the general surgery literature and have led to vast improvements in the patient experience. ERAS protocols are generally substantiated on 3 phases along the continuum of surgical care: preadmission optimization, intraoperative treatment, and postoperative management. In this article, the evidence for ERAS development in craniomaxillofa

Giant Lacrimal Gland Cyst in a Pediatric Patient: Case Report and Review of the Literature.

Lacrimal gland cysts are rare clinical entities in the pediatric population. Herein is described a 6-year-old male patient presenting with progressive left upper lid ptosis, found to have a large ipsilateral superotemporal orbital mass. Diagnosis of a giant lacrimal gland cyst was confirmed excisional biopsy. Despite the resolution of mechanical blepharoptosis, the patient maintained visual acuity limitation due to suspected deprivation amblyopia. The pathophysiology and clinical manifestations of lacrimal gland cysts in the pediatric population are reviewed to emphasize the importance of expe

Rapid Diagnosis of Invasive Aspergillosis and Active Hepatitis-B Virus Co-Infection in a HIV-1 Infected Patient Using Cell Free DNA Sequencing.

Multiple simultaneous opportunistic infections in Human Immunodeficiency virus-1/Acquired immunodeficiency syndrome (HIV-1/AIDS) is a known and dreaded occurrence that often leads to poor outcomes. We present a case of disseminated aspergillosis and active Hepatitis-B virus (HBV) infection in such a host, where cell free DNA (cfDNA) next generation sequencing (NGS) of plasma was used to expedite diagnosis. Bronchoscopy was avoided and treatment was started expeditiously. In this case report we discuss the interpretation of the cfDNA NGS, and its potential role for early diagnosis and avoidance

An Unusual Case of Cavitary Lung Lesion and a Brief Review of Literature.

Large cavitary pulmonary infarctions are extremely rare. Here, we report an unusual case of large pulmonary infarction due to pulmonary embolism (PE). This was complicated by secondary infection with Actinomyces leading to cavitary abscess formation. The patient had minimal non-specific symptoms despite extensive involvement and was treated with lobectomy, antibiotics, and anticoagulation, with expedited recovery. This case depicts a rare association between PE/pulmonary infarction and pulmonary actinomycosis. Furthermore, it highlights a high index of suspicion needed to diagnose these two co

Transthoracic Echocardiographic Findings of Stanford Type A Aortic Dissection: A Case Report.

Aortic dissection is a well-known, but relatively uncommon diagnosis in the emergency department (ED). With a mortality rate as high as 30 percent, it is important to be able to diagnose quickly and accurately. Definitive diagnosis with imaging studies such as computed tomography angiogram (CTA) can be expensive and time-consuming and may not always be available in the community. Herein, we discuss a case of a 59-year-old man presenting with severe chest pain, hypotension, and bradycardia who was diagnosed with aortic dissection first by bedside ultrasound. This expedited the CTA and a cardiot

Subacute Encephalopathy With Seizures in Alcoholics Syndrome: A Subtype of Nonconvulsive Status Epilepticus.

A recent assessment of the classification of nonconvulsive status epilepticus (NCSE) has incorporated the specific electroencephalographic (EEG) patterns on a syndromic basis. Such a clinical EEG syndromic approach may enable more accurate and expedited diagnosis of particular subtypes of NCSE so as to improve therapy. Herein, we review the characteristics of subacute encephalopathy with seizures in alcoholics syndrome, a subtype of focal NCSE occurring in chronic alcoholism with specific features, including encephalopathy, lateralized periodic discharges on the EEG, chronic microvascular isch

Phacoemulsification with intraocular pinhole implantation associated with Descemet membrane endothelial keratoplasty to treat failed full-thickness graft with dense cataract.

We present a case in which a different approach was used to treat an eye with dense cataract associated with an irregular failed full-thickness corneal graft. After cataract surgery, a foldable intraocular pinhole was implanted in the capsular bag to treat the irregular corneal astigmatism. Next, a Descemet membrane endothelial keratoplasty graft was used to improve transparency in the failed penetrating keratoplasty. This approach addressed the cataract, irregular cornea, and failed graft through a standard 2.2 mm clear corneal phacoemulsification incision, thus avoiding open-sky surgical tim

The Changing Landscape of Alopecia Areata: The Translational Landscape.

Recent genetic and preclinical studies have increased our understanding of the immunopathogenesis of alopecia areata (AA). This has allowed expedited development of targeted therapies for the treatment of AA, and a paradigm shift in our approach and understanding of autoimmunity and the hair follicle. The synergy between preclinical studies, animal models, and translational studies has led to unprecedented advances in the treatment options for AA, ultimately benefiting patients who have had little recourse. In this review, we summarize the scientific field of contemporary AA research, and look

An audit of a novel electronic messaging treatment service for Chlamydia trachomatis at a community pharmacy.

In 2016, NHS Lothian regional sexual and reproductive health services introduced an electronic pharmacy treatment voucher that could be sent to the mobile phones of patients with Chlamydia trachomatis and their sexual contacts. A retrospective audit of the electronic treatment voucher database was conducted for vouchers issued between April and December 2016. Five hundred and forty-nine vouchers were issued and 56% were redeemed at a pharmacy within one week of issue. This suggests that electronic vouchers are a popular and effective means of providing expedited treatment for uncomplicated C.

Successful management of acute interstitial nephritis in two cases of disseminated tuberculosis.

Acute interstitial nephritis (AIN) is characterized by an inflammatory infiltrate in the kidney interstitium and a decline in the creatinine clearance. Medications used for the treatment of tuberculosis have been implicated in the development of AIN, but there is limited data on how to manage AIN in this setting and which medications and dosages should be used to treat tuberculosis once AIN occurs. We describe two cases of AIN in the setting of disseminated tuberculosis in which AIN was successfully managed. It is recommended that Infectious Diseases and Nephrology be involved early in the car

Surgical perspectives in craniofacial trauma.

Knowledge of relevant anatomy and underlying mechanisms of traumatic injury is essential for understanding the radiologic findings in craniofacial trauma and their clinical importance. Craniofacial anatomy is diverse, and as a result of this anatomic diversity, physicians from numerous different specialties scrutinize similar imaging sets, looking for different pathologic abnormalities within the same anatomic regions. Radiologists familiar with the chief concerns of this anatomically diverse region can help expedite the decision-making process by keeping those concerns in mind when they repor

Choosing an animal model for the study of Huntington's disease.

Since the identification of the causative gene in Huntington's disease (HD), a number of animal models of this disorder have been developed. A frequently asked question is: which of these models most closely recapitulates the human disease? In this Review, we provide an overview of the currently available animal models of HD in the context of the clinical features of the disease. In doing so, we highlight their strengths and limitations for modelling specific symptoms of the disease. This should highlight the animal model that is best suited to address a particular question of interest and, ul

Gorham's disease: the disappearing bone.

Gorham's disease is a rare disorder characterized by proliferation of vascular channels. The clinical presentation of Gorham's disease is variable and depends on the site of involvement. A high index of clinical suspicion is needed for accurate diagnosis, and recognition of distinctive radiologic and histopathologic features can help facilitate and expedite diagnosis. The subsequent images were obtained from a 31-year-old male patient. Nuclear scintigraphy and computed tomography identified active bone destruction in the lower thoracic and upper lumbar spine in addition to the absence of the l

Fulminant herpes simplex viral hepatitis: ultrasound and CT imaging appearance and a review of the imaging literature.

We report the case of a previously healthy 21-year-old woman who presented 6 days post-partum in acute fulminant hepatic failure. A liver ultrasound demonstrated normal echogenicity without discrete nodules while an enhanced computed tomography (CT) demonstrated innumerable 1- to 3-mm hypodense nodules throughout the liver with greater involvement of the left lobe. Liver biopsy confirmed herpes simplex virus infection. We believe this disease has a characteristic appearance on CT and prompt recognition can expedite diagnosis and therapy.

Pulmonary actinomycosis.

Pulmonary actinomycosis is a rare but important and challenging diagnosis to make. Even when the clinical suspicion is high, the disease is commonly confused with other chronic suppurative lung diseases and with malignancy. An early, accurate diagnosis will prevent the considerable psychological and physical morbidity, including unwarranted surgery, associated with delayed diagnosis. The clinical, radiological and therapeutic characteristics of the infection are reviewed here. Respiratory physicians should be aware of this important differential when investigating patients for persistent pulmo

Treatment of congenital ectodermal dysplasia with zygomatic implants: a case report.

Dental abnormalities associated with ectodermal dysplasia (ED) can result in severe functional and esthetic problems. To correct these problems, dental implants have increasingly become the treatment of choice. This patient study illustrates the use of implants to rehabilitate a 20-year-old ED patient who initially presented with only 2 permanent and 6 primary teeth in the maxilla. Along with conventional endosseous implants, 2 specially designed zygomatic implants were utilized to avoid the need for bone grafting in the patient's severely resorbed maxilla. This expedited achievement of the fi

Classification of origin and proximal epicardial course of coronary arteries in transposition of the great arteries.

The complexity of the variability in aortic origin and epicardial course of the coronary arteries relative to the pedicles of the great arteries in hearts with transposition of the great arteries is evident in that there is no simple classification to include all the possible variations of the coronary artery patterns. Therefore, a simplified and yet all-inclusive classification is proposed in which the origin of the coronary arteries is defined as seen by an observer looking from the pulmonary artery toward the aorta. This promote and classification should expedite and promote the internation

[Fibrous dysplasia and ossifying fibroma. 2 unusual fibro-osseous lesions of the paranasal sinuses].

Fibrous dysplasia and ossifying fibroma are locally destructive and deforming benign lesions that can occur anywhere in the craniofacial skeleton, including the paranasal sinuses. Differentiation between these two diseases is essential because of their differing natural histories, and distinctions must be made by correlating clinical information with radiological and histological findings. The cases of fibrous dysplasia and ossifying fibroma described in this report illustrate the subtle differences between these two fibroosseous lesions. Emphasis is placed on the differing natural history and

Granulocyte-macrophage colony stimulating factor for the treatment of drug induced agranulocytosis.

Drug induced agranulocytosis is an uncommon but potentially fatal complication. In some cases, it may be associated with hypoplasia and depletion of granulocytic precursors in the marrow, leading to prolonged neutropenia. We report on the use of granulocytic-macrophage colony stimulating factor (GM-CSF) in two such cases, at a dose of 10 micrograms per kilogram per day subcutaneously. The absolute neutrophil count rose above 500/mm3 in 3 days in both cases. We believe that GM-CSF expedited the recovery of granulocyte counts in our patients and warrants further study in the management of drug i

Urineprint of high-altitude: Insights from analyses of urinary biomarkers and bio-physical-chemical features of extracellular vesicles.

Humans exposed to altitude hypoxia experience dysfunctions of the urinary system. As a non-invasive, easily manageable and informative biological sample, urine represents a relevant matrix for detecting clinical impairments of urinary system, as well as alterations of other systems and extracellular vesicles (EVs) biology during high-altitude expeditions. Nevertheless, gaps exist in the comprehensive assessment of dysfunction, molecular burden and EVs biology due to high-altitude acute exposure. This study aimed to find a biophysical and biochemical signature of urinary EVs for hypoxia-induced…
