[MATHEMATICAL MODELING OF OPTIONS FOR MOUNTING AN EXTERNAL FIXATION DEVICE ON THE TIBIA DURING ITS LENGTHENING].

The main problem in the treatment of axial deformities and limb shortening in pediatric orthopedics is the presence of incomplete growth of the patient, which gives an unfavorable basis for the achieved correction. The most advanced technology for the treatment of deformities and limb shortening is the use of external fixation devices (AVF). Target. To study the stress-strain state of the "tibia-AVF model with different variants of AVF mounting in conditions of tibia lengthening. Mathematical modeling of tibial osteosynthesis with external fixation devices in children with achondroplasia was c

Open technique for supra-acetabular pin placement in pelvic external fixation: a cadaveric study.

BACKGROUND: Standard supra-acetabular pin placement still needs fluoroscopic guidance, which is technically demanding for an untrained surgeon due to the unfamiliar fluoroscopic view and the risk of damaging some structures. The risks associated with the open technique without fluoroscopy have not yet been investigated, despite the palpable entry point and large bony corridor for rapid insertion in the limited time available for the management of unstable pelvic fracture patients. The aim of this study was to compare the open technique without fluoroscopy to the fluoroscopically assisted percu

The Use of External Fixator and Iliac Crest Bone Graft in Traumatic Metacarpal Bone Defect.

INTRODUCTION: Metacarpal bone fractures often lead to significant shortening in the phalanges; it can be caused by variable conditions, ranging from idiopathic, and infective to traumatic causes. There are very rare reports of metacarpals shortening in idiopathic primary hypoparathyroidism cases in the literature. Traumatic metacarpal shortening differs from congenital agenesis [2]. This condition leads to both functional and cosmetic limitations which affect the patient's quality of life. CASE REPORT: A 22-year-old male patient was admitted to our outpatient clinic with a traumatic left-hand

Post-operative complications of tibial plateau fractures treated with screws or hybrid external fixation.

PURPOSE: To assess the complications and second surgeries rates at 1 year follow-up in a group of patients underwent minimally invasive fixation with screws or hybrid external fixation (HEF) for tibial plateau fractures (TPF). The hypothesis was that low Schatzker (I-IV) TPF would have shown a lower complication rate with respect to high Schatzker (V-VI) TPF. METHODS: 148 patients who underwent minimally invasive surgery with screws or HEF for TPF were included and pooled in two groups: mono-condylar (Schatzker I-IV) and bi-condylar (Schatzker V-VI). The rate of second surgeries and complicat

Conventional bicortical pin substitution with a novel unicortical pin in external fixation: A biomechanical study.

INTRODUCTION: As most patients with polytrauma or open fractures are converted from temporary external fixation to definite stabilization, the prevention of complications such as infection is especially important. To overcome the high risk of infection associated with the use of the conventional bicortical pin for temporary external fixation, the authors developed a novel unicortical pin and analyzed it in a biomechanical study. METHODS: The unicortical pin consisted of an inner screw, purchasing the cortical bone, and an outer sleeve with 6 spikes. A bicortical pin was used for the purpose o

Ilizarov External Fixation for Correcting Complex Postpolio Foot and Ankle Deformity: A Case Study.

Given their severity and resultant loss of function, postpoliomyelitic foot and ankle deformities require a unique correction method. Correction with dynamic Ilizarov external fixation is one such modality, although it is technically demanding and requires precise preoperative planning. In our case study, a 40-year-old male with a severe postpoliomyelitic equinocavovarus right foot and ankle deformity was treated with gradual correction using dynamic, hinged Ilizarov external fixation. The external fixation was in place for a total of 103 days, with gradual correction performed in 2 successive

A Novel Inexpensive Technique to Seal Negative Pressure Wound Therapy on External Fixation Devices.

Negative pressure wound therapy, a tool widely applied to treat lower limb traumas, is useful in reconstructive procedures. However, obtaining an airtight vacuum seal when using a negative pressure dressing around an external fixation device can be complicated and time-consuming. The plastic drape seldom adheres to screws, pins, or wires and, as such, the vacuum seal is jeopardized. In surgical departments, colostomy paste is widely used, readily available, and applied where pins and wires make contact with the plastic drape. It is a fast, practical, and inexpensive method of preventing air le

A safe technique for insertion of heel wires for hindfoot external fixation frames: an anatomical study.

AIMS: Anatomical atlases document classical safe corridors for the placement of transosseous fine wires through the calcaneum during circular frame external fixation. During this process, the posterior tibial neurovascular bundle (PTNVB) is placed at risk, though this has not been previously quantified. We describe a cadaveric study to investigate a safe technique for posterolateral to anteromedial fine wire insertion through the body of the calcaneum. MATERIALS AND METHODS: A total of 20 embalmed cadaveric lower limbs were divided into two groups. Wires were inserted using two possible inser

Paediatric tibial shaft fractures treated by open reduction and stabilization with monolateral external fixation.

PURPOSE: Elastic stable intramedullary nailing is increasingly used for surgical treatment of tibial shaft fractures, but frequently requires immobilization and delayed full weight-bearing. Therefore, external fixation remains interesting. The aim was to report clinico-radiological outcomes of monolateral external fixation for displaced and unstable tibial shaft fractures in children. METHODS: All tibial fractures consecutively treated by monolateral external fixation between 2008 and 2013 were followed. Inclusion criteria included skeletal immaturity and closed and open Gustilo I fractures c

Femoral nonunion with segmental bone defect treated by distraction osteogenesis with monolateral external fixation.

BACKGROUND: Currently, the common treatment for femoral nonunion with large segmental bone defect is difficult and complex. The effective surgical methods are rare, include vascularized bone grafting, Masquelet technique and Ilizarov distraction osteogenesis. The objective of this study is to investigate the outcomes of segmental femoral defects treated with monolateral external fixation using the distraction osteogenesis. METHODS: We retrospectively analyzed patients with femoral nonunion with segmental bone defects (> 6 cm) between January 2010 and January 2014 in our single trauma center.

Prophylactic Postoperative Antibiotics May Not Reduce Pin Site Infections After External Fixation.

BACKGROUND: Pin infection continues to be a nuisance when using definitive external fixation. Prophylactic antibiotic treatment has been proposed in an effort to decrease pin complications. QUESTIONS/PURPOSES: We performed a prospective, randomized, single-blinded study to answer the following questions: (1) what was the effect of a 10-day course of oral prophylactic antibiotics administered immediately after external fixation surgery on the incidence of a subsequent pin infection, (2) what was the effect on the severity of a subsequent pin infection, and (3) what was the effect on the timing

Hinged external fixation for Regan-Morrey type I and II fractures and fracture-dislocations.

Elbow fracture-dislocation is always demanding to manage due to the considerable soft-tissue swelling or damage involved, which can make an early open approach and ligamentous reconstruction impossible. The purpose of this study was to evaluate the role of elbow hinged external fixation (HEF) as a definitive treatment in patients with elbow dislocations associated with Regan-Morrey (R-M) type I and II coronoid fractures and soft-tissue damage. We treated 11 patients between 2010 and 2012 with HEF. Instability tests and standard X-ray examinations were performed before surgery and 1-3 to 3-6 mo

Monoaxial external fixation of the calcaneus: An anatomical study assessing the safety of monoaxial pin insertion.

The use of external fixation for intra-articular calcaneal fractures is increasing in popularity. Studies have shown fine wire and monoaxial external fixation to be a viable surgical alternative to more invasive methods of open reduction and internal fixation of the calcaneus. However, there is an absence of literature that quantifies the risk of pin insertion for monoaxial fixation. This study aimed to determine the safety of inserting monoaxial pins within the calcaneus to house the Orthofix Calcaneal Mini-Fixator. Five formalin embalmed cadaveric ankle and lower leg specimens were inserted
