Presentation of one case of renoduodenal fistula due to pyonephrosis in an oligophrenic female patient. In view of the patient's characteristics, the clinical picture was highly developed at presentation which resulted in failure of the conservative treatment. We emphasize the diagnostic and therapeutical methodology followed. Also, we include a literature review on this disease.

A 77 year old man with an eight month history of polymyalgia rheumatica treated by steroids underwent left lower lobectomy for a non-small cell carcinoma. Following discharge he developed a pneumothorax, empyema, septicaemia, and acute renal failure. Reinvestigation and reoperation showed this to be caused by a non-malignant non-traumatic direct gastropleural fistula.

A patient is described who underwent closed mitral valvotomy and presented 21 years later with left ventricular failure. Coronary angiography revealed a coronary artery to pulmonary vein arteriovenous fistula. This is the first report of an acquired fistula of this type developing secondary to trauma associated with cardiac surgery. Diagnosis and treatment implications are discussed.

The association between diving, barotrauma, and the production of perilymphatic fistula has been known for almost 20 years. Forty-eight cases of round and oval window fistulas following diving have been reviewed and essentially corroborate previous findings. We report an unusual case of an enterovesical fistula caused by leiomyosarcoma infiltrating the intestinal wall and urinary bladder. The patient had previously presented recurrent urinary tract infection for one year. Although he has residual tumor, so far this elderly patient has survived for three years post-operatively without further antineoplastic treatment.

A retrospective series is presented of 51 cases operated on for suspected perilymph fistula. In 26 ears a fistula was identified at surgery. A positive fistula test was found to strongly indicate a perilymph fistula but was more often negative than positive in surgically demonstrated fistula ears. Other vestibular tests were found to be of little value in the pre-operative diagnosis. We report on a case of vesicouterine fistula following cesarean section in a 26-year-old patient. This rare complication can be initially treated noninvasively with contraceptive steroids combined with other conservative therapeutic methods such as placement of a bladder catheter and electrocoagulation of the borders of the fistula.

A 66-yr-old man was anaesthetized for repair of a suspected ruptured aortic aneurysm. He became deeply cyanosed and suffered two episodes of asystole as the surgeons entered the abdomen. After the aorta was cross-clamped his condition improved markedly and no further problems ensued. Surgical exploration demonstrated intact aneurysms of the aorta and left iliac artery, the former containing a 3-4 cm long aortocaval fistula. The aneurysms and fistula were repaired and his further course was uncomplicated. Potential causes for the cyanosis and cardiac arrest in this patient are discussed.

The authors report five cases of benign oesophago-bronchial fistula in adults. These chronic bronchitic patients presented with recurrent episodes of secondary lung infections (or even haemoptysis). Questioning of the patients revealed a history of coughing when swallowing liquids in three cases. Two women concealed this symptom, which was only revealed retrospectively. Arteriovenous fistulas of the lung occur as a disease limited to the lung, or as a pulmonary manifestation of hereditary Rendu-Osler-Weber's disease. Typical complications are haemoptysis, haemothorax and cerebral embolism. We report on 3 patients in whom arteriovenous fistulas of the lung were removed by wedge excision or lobectomy. One of them had already suffered cerebral infarction. The indication for operative removal of single lesions is always given with respect to the severe complications of this disease. Decrease of the right-to-left shunt volume can usually be achieved.

Acquired coronary cardiac fistulas are rare. To date, thirty cases have been published in the literature, all secondary to thoracic trauma. The right cardiac chambers were more frequently involved. A recent continuous cardiac murmur is the most constant finding. Nontreated, these lesions tend to induce coronary insufficiency and congestive heart failure. 22 of the 30 reported cases were managed surgically. We hereby report a case of postoperative coronary cardiac fistula secondary to left ventricular venting, in which the fistula involved the left ventricular cavity.

A 59-year-old man was hospitalized with complaints of diarrhea and pollakisuria. Retrograde urethrography, urethroscopy and urethral pressure profile revealed a fistula between the urethra and the rectum situated immediately above the external urethral sphincter. While barium esophagography is useful in revealing fistulas between the esophagus and trachea or bronchus, it is not adequate to show those between esophagus and lung or mediastinum. In this study, a patient with esophago-pulmonary and esophago-mediastinal fistulas was evaluated by CT, which proved to be more effective in showing fistulas between the esophagus and lung or mediastinum.

Seventy-eight tympanotomies were performed to determine the presence of perilymph fistulas (PLFs); of these, 51% were found. The oval and round windows of all patients were grafted, whether PLFs were present or not. Of those patients with PLFs, 64% had resolution of their major symptom; when no PLF was found, 44% had a similar outcome.
