Anesthesia for patients with dehydration/hypovolemia.

Anesthetic drugs normally depress cardiovascular function and in a patient that is already dehydrated and hypovolemic can cause severe hypotension. Reestablishing intravascular volume with a balanced salt solution can return cardiovascular function to normal, and anesthesia must not be induced until the animal has been stabilized.

What the practicing nurse should know about thiamine.

Thiamine (vitamin B1) is an essential nutritional component that acts as a coenzyme in the oxidative decarboxylation of alpha-keto acids. It also serves as the connection between the glycolytic cycle and the high energy-producing Krebs (or citric acid) cycle. Unlike other B vitamins, it activates the guanylate cyclase/cyclic guanosine monophosphate (GMP) system but not the adenylate cyclase system. The active coenzyme, thiamine pyrophosphate (TPP) is an antiberiberi substance. Thiamine itself is a pharmacologic antagonist of acetylcholine, which may explain the nerve lesions caused by thiamine

Risk of Electrolyte Disorders in Acutely Ill Children Receiving Commercially Available Plasmalike Isotonic Fluids: A Randomized Clinical Trial.

IMPORTANCE: The use of isotonic fluid therapy is currently recommended in children, but there is limited evidence of optimal fluid therapy in acutely ill children. OBJECTIVE: To evaluate the risk for electrolyte disorders, including hyponatremia, hypernatremia, and hypokalemia, and the risk of fluid retention in acutely ill children receiving commercially available plasmalike isotonic fluid therapy. DESIGN, SETTING, AND PARTICIPANTS: This unblinded, randomized clinical pragmatic trial was conducted at the pediatric emergency department of Oulu University Hospital, Finland, from October 3, 20…

From cholera to burns: a role for oral rehydration therapy.

According to the practice guidelines of the American Bum Association on burn shock resuscitation, intravenous (i.v.) fluid therapy is the standard of care for the replacement of fluid and electrolyte losses in burn injury of > or = 20% of the total body surface area. However, in mass burn casualties, i.v. fluid resuscitation may be delayed or unavailable. Oral rehydration therapy (ORT), which has been shown to be highly effective in the treatment of dehydration in epidemics of cholera, could be an alternate way to replace fluid losses in burns. A prospective case series of three patients was c…

AKI Associated with Acute Pancreatitis.

Acute pancreatitis is a common disorder of the pancreas. It is the most frequent gastrointestinal cause for hospitalization and one of the leading causes of in-hospital deaths. Its severity ranges from mild self-limited disease to severe acute necrotizing pancreatitis characterized by systemic complications and multiorgan failure. Severe acute pancreatitis develops in about 20% of patients with acute pancreatitis and may be associated with multiorgan failure (respiratory, cardiovascular, and kidney). AKI is a frequent complication of severe acute pancreatitis and develops late in the course of

Managing hyperemesis gravidarum: a multimodal challenge.

Up to 90% of pregnant women experience nausea and vomiting. When prolonged or severe, this is known as hyperemesis gravidarum (HG), which can, in individual cases, be life threatening. In this article the aetiology, diagnosis and treatment strategies will be presented based on a selective literature review. Treatment strategies range from outpatient dietary advice and antiemetic drugs to hospitalization and intravenous (IV) fluid replacement in persistent or severe cases. Alternative methods, such as acupuncture, are not yet evidence based but sometimes have a therapeutic effect. In most cases,

Complexity of blood volume control system and its implications in perioperative fluid management.

The use of fluid therapy attempts to optimize blood circulation by manipulating the circulating blood volume (BV). BV may be a key intermediate parameter between fluid therapy and the blood circulation, and it has been assumed that BV can be controlled by fluid therapy. In order to construct a fluid therapy protocol, firstly, we have to confirm whether BV can actually be controlled by fluid therapy. Volume kinetics studies and dilution techniques for BV measurements have enabled the actual effects of fluid management on BV to be analyzed in the presence of various pathological conditions. Vari…

[Fluid management in shock patients : New targets in the initial phase of shock].

BACKGROUND: Early i.v. fluid administration is a cornerstone in modern therapy of shock, especially in septic shock. However, there is much uncertainty concerning the amount and rate of fluid and which goals and measures could guide fluid management. Administering the optimal fluid volume is important because fluid overload can lead to severe negative consequences like organ failure and worsening of patient's outcome. AIM: This review aims to describe the importance of fluid therapy and discuss possible strategies in fluid management as well as possible measurements and goals to guide such th…

Fluid overload in the ICU: evaluation and management.

BACKGROUND: Fluid overload is frequently found in acute kidney injury patients in critical care units. Recent studies have shown the relationship of fluid overload with adverse outcomes; hence, manage and optimization of fluid balance becomes a central component of the management of critically ill patients. DISCUSSION: In critically ill patients, in order to restore cardiac output, systemic blood pressure and renal perfusion an adequate fluid resuscitation is essential. Achieving an appropriate level of volume management requires knowledge of the underlying pathophysiology, evaluation of volu

Indications and management of mechanical fluid removal in critical illness.

BACKGROUND: The Acute Dialysis Quality Initiative (ADQI) dedicated its Twelfth Consensus Conference (2013) to all aspects of fluid therapy, including the management of fluid overload (FO). The aim of the working subgroup 'Mechanical fluid removal' was to review the indications, prescription, and management of mechanical fluid removal within the broad context of fluid management of critically ill patients. METHODS: The working group developed a list of preliminary questions and objectives and performed a modified Delphi analysis of the existing literature. Relevant studies were identified thro…
