Atrial Arrhythmia Following Extra-cardiac Fontan Surgery: Is the Arrhythmia Mechanism Characterized?

We discuss a case of symptomatic atrial arrhythmia in a 31 year old man who had previously undergone extra-cardiac Fontan surgery and a MAZE procedure. Besides the peculiar anatomy that restricted percutaneous atrial access only by perforation through the extra-cardiac tunnel, changing cycle lengths of the tachycardua necessitated innovative mapping to define the electro-morphology of the arrhythmia.

Damus-Stansel-Kaye procedure: current indications and results.

Between October 1983 and August 1991, 29 consecutive Damus-Stansel-Kaye procedures were performed. Indications for operation included restrictive bulboventricular foramen or subaortic stenosis associated with complex univentricular congenital heart disease (25) and Taussig-Bing heart, subaortic stenosis, or both associated with complex biventricular congenital heart disease (4). Twelve patients underwent concurrent Fontan procedures. Average age at operation was 39.8 months (range, 1 to 132 months). Average outflow tract gradient was 28 mm Hg (range, dynamic to 80 mm Hg). Of the 29 patients, 2…

Atrioventricular valve repair or replacement in atriopulmonary anastomosis: surgical considerations.

Three patients who underwent a modified Fontan-Kreutzer (F-K) operation required additional procedures to correct left atrioventricular valve regurgitation. Valve replacement was performed in two additional procedures, and initial annuloplasty with a Carpentier ring was carried out in the third. All 3 survived the combined procedures and are doing well after a follow-up period ranging from five months to three years. More frequent presentation of these types of patients can be expected with broadening indications for F-K procedure to more complex lesions.

Arrhythmia Management in Complex Congenital Heart Disease.

BACKGROUND: Atrioventricular block is a common complication in L-looped single ventricles, and it may be exacerbated by palliative surgical procedures. CASE SUMMARY: We describe the case of a 43-year-old woman with history of double inlet left ventricle after classic Fontan procedure with subsequent Fontan revision, atrial flutter, and complete heart block managed with an epicardial pacemaker with progressive ventricular lead dysfunction. The patient was managed with leadless pacemaker implantation and atrial flutter ablation. DISCUSSION: Epicardial pacing is the preferred modality for patie…

Impact of Pulsatile Bidirectional Cavopulmonary Shunt on Pre-Fontan Hemodynamics in Single Ventricle Physiology: A Meta-Analysis Reveals Favorable Outcomes.

PURPOSE: This study aims to determine the outcomes of maintaining antegrade pulmonary blood flow (APBF) during the bidirectional cavopulmonary shunt (BCPS) procedure in patients with single ventricle physiology undergoing staged palliative surgeries. METHODS: A systematic search of electronic databases was conducted and focused on studies comparing pulsatile BCPS (with APBF) with non-pulsatile BCPS (without APBF). Outcomes were categorized into early (post-BCPS) and late (pre-Fontan). Data were analyzed using Mantel-Haenszel random effects model with odds ratios (ORs) and mean differences (MD

Follow-up study of pulmonary artery configuration in hypoplastic left heart syndrome.

OBJECTIVE: Pulmonary artery (PA) distortion significantly compromises the outcome of the staged approach to the Fontan operation in patients with hypoplastic left heart syndrome (HLHS). This retrospective study was designed to investigate the influence of the initial operation on postoperative PA anatomy. METHODS: Forty-nine patients with HLHS and its variant were enrolled in this study. As an initial palliation, the Norwood operation with a modified Blalock-Taussig (BT) shunt was performed in 12, the Norwood operation with a right ventricle to pulmonary artery (RV-PA) shunt in 31, and bilate

Regenerative Therapy for Patients with Congenital Heart Disease.

Congenital heart disease (CHD) is the most common birth defect, affecting 1 in 100 babies. Among CHDs, single ventricle (SV) physiologies, such as hypoplastic left heart syndrome and tricuspid atresia, are particularly severe conditions that require multiple palliative surgeries, including the Fontan procedure. Although the management strategies for SV patients have markedly improved, the prevalence of ventricular dysfunction continues to increase over time, especially after the Fontan procedure. At present, the final treatment for SV patients who develop heart failure is heart transplantation…

Effects of landiolol on refractory tachyarrhythmia after total cavopulmonary connection: a retrospective, observational, cohort study.

The onset of tachyarrhythmia after the Fontan procedure (total cavopulmonary connection; TCPC) should be considered a medical emergency. Landiolol is an ultra-short-acting β1-selective blocker whose effect on tachyarrhythmia after TCPC is unclear. We evaluated the efficacy and safety of landiolol for tachyarrhythmia after TCPC. Consecutive patients undergoing TCPC were enrolled from January 2007 to December 2011. Of 435 pediatric open heart surgeries, 28 patients underwent TCPC. Of the 28 patients, 13 were treated with landiolol for critical tachyarrhythmia. Excluding three patients who receiv…

Impact of Norwood versus hybrid palliation on cardiac size and function in hypoplastic left heart syndrome.

OBJECTIVE: The hybrid approach for hypoplastic left heart syndrome (HLHS) could theoretically result in better preservation of right ventricular (RV) function then the Norwood procedure. The aim of this study was to compare echocardiographic indices of RV size and function in patients after Norwood and hybrid throughout all stages of palliation. METHODS: 76 HLHS patients (42 Norwood, 34 hybrid) were retrospectively studied. Echocardiography was obtained before stage I, before and after stage II, and before and after Fontan. Median follow-up was 4.9 years (range 1.1-8.5). RESULTS: Baseline ch

Changes in pulmonary artery size during and after staged extracardiac total cavopulmonary connection.

BACKGROUND: Optimal development of the pulmonary arteries (PAs) is crucial for Fontan circulation. This study investigated longitudinal chan … off of 0.61 discriminated transplantation-free survival (P = .006). CONCLUSIONS: During the staged Fontan procedure, the left PAI decreased, whereas the PAI and right PAI remained stable or increased. A PA symmetry index <0.61 predicts mortality following TCPC. This progressive asymmetry in left-to-right PA size affects in-hospital morbidity and emphasizes the im …

Biomechanics and clinical implications of Fontan upsizing.

BACKGROUND: The Fontan operation, a palliative procedure for single ventricle patients, has evolved to improve outcomes and reduce complications. While extracardiac conduit (ECC) is favored for its simplicity and potential hemodynamic benefits, concerns arise about conduit size adequacy over time. Undersized ECC conduits may cause hemodynamic inefficiencies and long-term complications, while oversizing can lead to flow disturbances, stagnation, and thrombosis, necessitating surgical revision or upsizing to optimize hemodynamics. OBJECTIVES: The study aimed to predict the impact of upsizing by
