Pisotriquetral arthrodesis for the management of chronic pisotriquetral fracture-dislocation: a case report.

INTRODUCTION AND IMPORTANCE: Isolated dislocation of the pisotriquetral joint is a rare and often overlooked condition, with diagnosis frequently delayed because of nonspecific clinical symptoms and inconclusive initial imaging. CASE PRESENTATION: We report the case of a young, active patient with chronic distal dislocation of the pisiform bone, associated with an initially undiagnosed articular fracture of the triquetrum. Clinically, the patient reported pain rated at 4/10 during wrist flexion, which was slightly limited (76°), while extension and pronation remained preserved. His grip stren

Locked Posterior Shoulder Dislocation with Accompanying Fracture: A Debilitating Rare Injury for Athletes?

Shoulder dislocation is undoubtedly one of the most common injuries in traumatology. Posterior dislocation of the shoulder is regarded as comparatively rare compared to anterior dislocation, whilst it is strikingly extraordinary for this type of injury to be accompanied by a concomitant fracture. Accurate diagnosis and appropriate treatment can be considerably demanding, while being vitally important in this exceedingly rare condition. These injuries are broadly considered to be under-reported in the existing literature, especially among high-demand athletes. We present a rare case of a high-d

Two Cases of Ipsilateral Posterior Hip Dislocation and Shaft Femur Fracture and Management.

INTRODUCTION: Femoral neck fractures in young adults, particularly Pauwels' 3 fractures with angles exceeding 70°, pose challenges in determining optimal surgical interventions due to limited clinical trial data. Complications such as avascular necrosis, non-union, and shortening hinder the healing process. Existing fixation methods, including multiple cannulated screws and sliding hip screws, have shown non-union rates of nearly 9% in young patients. Bone healing relies on various factors, both patient-independent and patient-dependent, and deficient bone regeneration may necessitate interven

Brachial Plexus Injury Following Fracture-Dislocation of the Proximal Humerus: A Case Report.

Fracture-dislocations of the proximal humerus with brachial plexus injury are exceedingly rare, and although infrequently encountered, it is important to recognize this complication due to its potentially devastating impact. We present the case of a 75-year-old female who sustained the described injury following a fall onto their left arm, demonstrating combined sensory and motor deficits in the radial, median, and ulnar distribution of the left forearm and hand shortly afterward. Immediate management involved closed reduction under anesthesia, resulting in the improvement of neurological symp

Palmar Dislocation Fracture of the Hamate: A Case Report and Review of Literature.

Objective  Hamate dislocations are rare lesions of the carpus, which make their diagnosis difficult or even missed, and their management non-standardized. Case Description  We report the case of a 34-year-old victim of a high kinetic traffic accident. The initial evaluation revealed multiple fractures of both upper limbs, including a palmar fracture dislocation fracture of the hamate, without any vascular or nerve complications. Open surgical treatment via a dorsal approach allowed reduction and fixation of the hamate by temporary multiple pinning. At 12-month follow-up the patient had pain fr…

Open Fracture Dislocation of the Talus With Partial Talar Extrusion: A Case Report.

We report a case of open talar fracture-dislocation (Gustilo-Anderson type IIIA) associated with a posterior tibial artery injury. The limb … ecrosis (AVN) of the talus. This case report highlights the high risk of talus AVN after open talar fracture dislocation. Preservation of the extruded talus and anatomical reduction can maintain ankle alignment, which is essential for arthrodesis in cases of AVN complications.

Talus dislocation and pereonal malleolar fracture after high-energy trauma: a case report.

Talus dislocation is a rare injury and it is usually the consequence of high energy traumas, despite the anatomical features of the talus which predispose to its dissociation (absence of muscle insertions with over 60% of the surface covered by cartilage). It may be associated with malleolar fractures. Standard treatment of closed talar dislocation is a controversial issue. The most common early complications is avascular necrosis. Authors report a case of a complete talar dislocation associated to displaced lateral malleolar fracture in a 18-years-old male following a high energy trauma treat

Traumatic Fracture of the Thoracic Spine With Severe Posterolateral Dislocation: A Case Report.

Thoracic spine fracture-dislocation injuries are always associated with high-energy trauma and result in severe neurological symptoms. Surgical reconstruction and stabilization are essential for the early mobilization and rehabilitation of patients with this type of injury. Here, we present a unique case of a 57-year-old Greek male who sustained a posterolateral T3-T4 fracture-dislocation, with a complete spinal cord injury (SCI), after falling from a height. The patient was treated with surgical reduction and internal fixation with screws and rods, with satisfactory subsequent realignment.

Staged Treatment for Unstable Open Fracture-Dislocation of the Ankle: A Case Report.

Unstable fracture-dislocation of the ankle is a common lower-extremity injury. Treatment is challenging when the fracture-dislocation is open and cannot be treated with conventional open reduction and internal fixation (ORIF). Immediate ORIF may not be possible for severe, unstable ankle injuries, such as those with ischemic foot because of a poor blood supply caused by soft-tissue injury, or open fracture-dislocation of the ankle with a deltoid ligament rupture. We describe a staged treatment for unstable open fracture-dislocation of the ankle with a deltoid ligament rupture. The first stage

Case report of a delayed iatrogenic Pipkin type III femoral head fracture-dislocation.

RATIONALE: Pipkin III femoral head fracture dislocation (FHFD) is rarely observed in clinical practice, and its outcome is alarming. A considerable proportion of Pipkin III fractures result from repeated or forceful closed reduction of an irreducible FHFD. Pipkin type III fractures pose a therapeutic challenge. Most patients underwent total hip arthroplasty. PATIENT CONCERNS: A 34-year-old man experienced high-energy trauma to the left hip from a terrible traffic accident. Radiography and computed tomography (CT) at the local hospital revealed a left posterior FHFD. Emergency close reduction …

Superolateral Dislocation of Bilateral Mandibular Condyles With a Mandibular Body and Condyle Fracture.

An 82-year-old man fell from a motorcycle and was admitted with superolateral dislocation of the bilateral mandibular condyles. The displaced condyles on the right and left were hooked in the posterior root of the zygomatic arch and in the temporal fossa beyond the zygomatic arch, respectively. There was a sagittal split fracture on the medial side of the right mandibular condyle and a right mandibular body fracture. Under general anesthesia, manual reduction of the dislocation was successfully performed the day after the injury. Bilateral superolateral dislocations hooked to the temporal foss

Staged Reconstruction of a Moore Type 4 Fracture Dislocation, Parts 1 and 2.

High-energy tibial plateau fractures are associated with knee fracture dislocations and concomitant ligamentous injury. Both bony and ligamentous injuries can require surgical fixation, often requiring a multidisciplinary team and staged treatment. This article and accompanying video describe the workup and treatment of a Moore type 4 tibial plateau rim compression fracture with posterolateral corner and anterior cruciate ligament rupture that underwent open reduction internal fixation of the tibial plateau with posterolateral corner reconstruction and then staged anterior cruciate ligament re…

Simultaneous Clavicle Fracture and Acromioclavicular Joint Dislocation: Novel Surgical Technique: A Case Report.

CASE: A 41-year-old man sustained a midshaft clavicle fracture with associated type IV ipsilateral acromioclavicular joint (ACJ) dislocation. The clavicle was fixed with a locking plate and ACJ stabilized with a novel technique: 2 metal anchors with preloaded suture fixed at the coracoid process, looping the sutures over the clavicle, and passing through the plate holes without clavicle bone drilling. Excellent functional outcomes for range of motion, pain, and functional scores persisted 2 and a half years after surgery. CONCLUSION: The described surgical technique achieved exceptional short

"Locked-Lateral" Calcaneal Fracture-Dislocation Treated with Primary Subtalar Fusion: A Case Report.

CASE REPORT: A 65-year-old man presented with pain in his left ankle. After a delay, he presented to the orthopaedic clinic where computed tomography scan revealed an intra-articular, locked-lateral calcaneal fracture with dislocation of the peroneal tendons. CONCLUSIONS: This combination of injuries can produce subtle signs, such as an atypical double-density sign and varus talar tilt, on plain radiographs; however, the consequences of delayed treatment can be catastrophic. Early recognition and treatment are essential to avoid chronic pain and limited function.

Radial Head Dislocation with Ipsilateral Proximal Shaft of Radius Fracture: A Case Report.

Radial head dislocations are uncommon in adults. They are commonly seen in children and are generally associated with proximal ulna fracture. Radial head dislocation with associated proximal radial shaft fracture is rarer than isolated radial head dislocation in adults. Due to the rarity of this complex injury, in the absence of keen observation and meticulous attention, the correct diagnosis might be missed leading to unsatisfactory management and related complications. Here, a similar case of radial head dislocation with associated proximal radial shaft fracture has been presented.

Recurrent acromioclavicular joint dislocation with an associated coracoid fracture following acromioclavicular joint reconstruction.

Acromioclavicular joint injuries are common and account for up to 12% of all shoulder girdle injuries and have been reported as being as high as 50% in sports-related shoulder injuries. While the majority of acromioclavicular joint dislocations can be treated non-operatively, there are certain injury configurations, which can include high-grade dislocations in overhead athletes, where surgery may be indicated. The surgical management of acromioclavicular joint instability has moved towards recreating the action of the coracoclavicular ligaments by resuspending the clavicle on to the coracoid.

Bilateral posterior fracture dislocation of the shoulders : review of case reports and treatment.

Bilateral Posterior Fracture Dislocation (BPFD) of the shoulder is an uncommon but not rare presentation. We describe etiology, diagnostics, treatments and outcome and give a historic review and with a current approach of this pathology. We reviewed 55 cases (110 shoulders), mostly men (49/55), with a mean age of 49.2 years, mean follow up 21.9 months, mean delay until diagnose of 12.7 days (0-112 days), with a seizure as the cause in 80.0% . Other causes are electrocution, trauma or other. If the mechanism is not clear an epileptic insult should be considered the cause until proven otherwise.…

Clinical outcomes after absorbable suture fixation of patellar osteochondral fracture following patellar dislocation.

BACKGROUND: Osteochondral fracture (OCF) is one of the severe complications following a patellar dislocation. The appropriate fixation method for patients with OCF remains controversial. METHODS: Eighteen patients who had undergone surgery after a patellar dislocation were recruited retrospectively. Patellar OCF was fixed with an absorbable suture in an unreported method. The medial patellofemoral ligament (MPFL) was repaired or reconstructed if necessary. The Lysholm and Kujala knee scoring systems were used to evaluate the knee function. Imaging examinations were used to confirm the fractur
