Compression plating of fractures of the femur has a number of logistic advantages over intramedullary stabilization techniques though it is equally demanding for the surgeon. In this retrospective study of 77 cases, we try to show that compression plating of fractures of the femoral shaft is still a viable alternative to intramedullary fixation, especially in the light of local complications, postoperative systemic implications for the patient with multiple injuries, and the time required to achieve bone union. The data relating to all three of these aspects permit the conclusion that the resu

Intramedullary fixation is increasingly used for intertrochanteric fractures in the elderly population. The management of hardware migration and non-union remains a challenge for orthopaedic surgeons, often necessitating complex revision procedures. We report a case of medial lag screw migration with propagation of a reverse oblique neck of femur fracture 8 weeks postoperatively in a patient in their mid-70s with pre-existing paraplegia. A revision procedure was performed. Existing hardware was removed without complication. The lag screw tract was packed with layers of a haemostatic agent to p

The treatment of diaphyseal and metaphyseal fractures of the humerus is often controversial, especially when these fractures are complex and/or unstable (i.e. comminuted, segmental, or with a butterfly fragment). The authors review the indications, advantages, and disadvantages of various open and closed procedures, concluding that a combination of internal and external fixation is a valid treatment for these fractures. The purpose of the external fixation is immediate stabilization of the fracture, which is difficult if not impossible with intramedullary internal fixation alone. Internal fixa

PURPOSE: In cases of closed tibia fractures with acute compartment syndrome (ACS), there is no established agreement on whether performing internal fixation at the time of closure or after healing of the fasciotomy wounds would affect the likelihood of non-union or infection risk. The study aims to compare fracture union rates, incidence of infection, and overall outcomes between early and delayed definitive fixation.
METHODS: Retrospective analysis of closed tibia fractures (AO/OTA 41, 42) with ACS between 2010 and 2019 with a minimum two years follow-up. The patients were grouped into group

BACKGROUND: Elastic intramedullary nails (ESIN) have been the treatment of choice in many long bone fractures in children for more than 20 years. The introduction of ESIN has drastically reduced tissue traumatization during fracture fixation procedures and decreased the risk of growth cartilage damage, as well as allowing for preservation of the natural biology of closed fracture healing. The objective of the present report is to draw attention to a small group of patients with bone mineralization disorders, who consequently demonstrate decreased mechanical resistance of the skeletal system, i

Current methods of treatment for anisomelia are based on distraction osteogenesis. This involves creating callus during gradual distraction using an external fixator. The lengthening of long bones by Ilizarov's method requires keeping the external fixator in place for a long period of time, which increases the risk of complications, such as pin - track infection, stiffness of the joint, misalignment, and fracture of the regenerate. In order to shorten the period during which the external fixator must remain in place, we have introduced a method which enables the external fixator to be removed

INTRODUCTION: Lower extremity fractures are a frequent complication in patients with chronic spinal cord injury due to significant bone mineral density loss and altered bone metabolism. While non-operative management has traditionally been favored, advances in surgical techniques have expanded treatment options, including intramedullary nailing and plate fixation. However, the optimal approach remains controversial, especially in paraplegic patients with distal femoral fractures. This case report highlights the challenges and complications associated with surgical management in this unique pat

BACKGROUND The choice of optimal internal fixation device for distal tibial fractures remains controversial. The purpose of our study was to evaluate the effectiveness and safety of open reduction and internal fixation, minimally invasive percutaneous osteosynthesis, and intramedullary nailing of distal tibial fractures in adults using network meta-analysis of data from clinical trials. MATERIAL AND METHODS The studies were abstracted from MEDLINE, EMBASE, CNKI, and the Cochrane Central Register of Controlled Trials. Randomized controlled trials meeting inclusion and exclusion criteria were se

Author compared the material of supracondylar femur fractures of the National Institute of Traumatology during a period of 5 years starting from 1962 until 1966 with that of a period of 6 years starting in 1972 until 1977. From among their 108 cases osteosynthesis was performed in 38 cases. He gives account of methods with wire pinning, medullar splinting, and nailing with Rush's, Jewett's and Küntscher's technique (transfixation of the knee in the latter) as well as of solutions with angulated nails performed mainly in the second period. For nonunion in this region external fixation was also

INTRODUCTION: Intramedullary nailing has been used as a standard in the treatment of the long bone fractures with its clinical and mechanical advantages. However, using distal locking screws has been associated with longer operative times, higher radiation exposure rates, and complications like breakages of distal screw or nail at the screw hole level. Therefore, attempts to eliminate distal locking screws has been always present for intramedullary nail fixation. With a similar purpose, the present study has been carried out to compare mechanical behaviors of intramedullary nail fixations with

BACKGROUND: Subtrochanteric fractures can be a treatment challenge. The substantial forces that this region experiences and the fact that the proximal fragment is frequently displaced make accurate reduction and internal fixation difficult. The purpose of this study was to evaluate a series of patients who had undergone clamp-assisted reduction and intramedullary nail fixation to determine the impact of this technique on fracture union rates and reduction quality.
METHODS: Between December 2003 and January 2007, fifty-five consecutive patients with a displaced high subtrochanteric femoral fra

BACKGROUND: Intramedullary nail (IMN) fixation of metacarpal fractures is an alternative to Kirschner wire (K-wire) fixation. The goal of this study was to compare the biomechanical properties of K-wire fixation with a threaded IMN (InNate; ExsoMed, Aliso Viejo, California).
METHODS: The study design was based on previously described biomechanical models for evaluating metacarpal fractures. Sixteen fresh frozen small finger-matched and ring finger-matched pairs were randomized to either IMN or 0.045 in K-wire fixation after receiving a standardized neck osteotomy. Proper implant placement was

OBJECTIVE: Study included surgical management of radius and ulna fracture in a 2-year-old, 220 kg crossbred heifer presented with a 2-day-old history of injury resulting in severe, non-weight bearing lameness of the left forelimb.
METHODS: The condition was diagnosed clinically and confirmed radiographically as a middle third oblique comminuted fracture of the left radius and ulna. Static intramedullary interlocking nailing was performed for fracture fixation. An interlocking nail of 12-mm diameter and 26-cm length was introduced in a retrograde manner from the distal aspect of the radius.

INTRODUCTION: Pediatric radial neck fractures represent 5-10% of all elbow fractures. Open reduction allows anatomic fracture fixation but compromises the vulnerable blood supply of the radial head. Intramedullary nailing combines the advantages of closed reduction and stable internal fracture fixation.
MATERIAL AND METHODS: During a 6-year-period the medical records of all children sustaining a radial neck fracture treated operatively at our institution were analyzed retrospectively.
RESULTS: Forty-two patients with dislocated fractures of the radial neck were treated with intramedullary na

Non-union following intramedullary nailing of diaphyseal long bone fractures represents a serious complication necessitating timely and effective intervention. The situation becomes considerably more complex in the presence of a broken femoral nail, which poses additional technical challenges for the operating surgeon. Conventional management strategies typically entail removal of the existing nail, followed by re-fixation with a larger intramedullary nail, plate fixation, or external fixation constructs. Our case describes a 34-year-old male patient who presented with aseptic femur non-union

Although general recommendations exist regarding the correct placement of the skin incision and the direction of deep dissection for closed antegrade intramedullary nailing of the femur, in surgical practice simultaneously establishing the correct entry point and exact direction for insertion of the entry instrument in the lateral (sagittal) plane may be difficult. This is due to sub-optimal radiographic images in the lateral plane as a result of the overlying shadows of the pelvis, variations in the degree of rotation of the femur during patient positioning and fracture reduction manoeuvres,

The majority of femur fractures are closed fractures in the mid-third of the femoral shaft. A very well known fixation technique is the one using an intramedullary pin, which is inserted from the hip side. Open reduction of the fracture is mostly applied but the majority, between 85 and 93%, of the fractures are closed and therfore a more logical and safer technique would be closed reduction. However, this technique, also called blind-nailing, is more difficult. Besides an x-ray scanner with a monitor it also requires experience of the surgeon and some special tools to supply the necessary red

INTRODUCTION: The 5th metacarpal fractures accounts for 38% of all hand fractures given that the neck is the weakest point in metacarpals, so neck fracture is the most common metacarpal fracture. Surgical fixation is also advocated for such fractures to prevent mal-rotation of the little finger which will lead to fingers overlap in a clenched fist. Various methods are available for fixation of such fractures, like intramedullary & transverse pinning. There are very few reports in the literature comparing both techniques. Authors wanted to compare outcomes and complications of transverse pinnin
