Nondisplaced shearing-type talar body fractures.

The majority of talar body fractures represent avulsive injuries or are associated with obvious displacement or dislocation. Minimally displaced, nonavulsive fractures of the talar body are rare and can be difficult to recognize. Delay in diagnosis and therapy may be associated with an increased risk of osteonecrosis, malunion, secondary osteoarthritis, and other complications. We present two cases of nondisplaced talar body fractures with delay in diagnosis and stress the importance of recognizing these fractures, given the appropriate mechanism of injury.

Humeral avulsions of the inferior glenohumeral ligament complex involving the axillary pouch in professional baseball players.

INTRODUCTION: Humeral avulsion of the inferior glenohumeral ligament is a rare injury in athletes and can involve different parts of the complex. Axillary pouch avulsion, in particular, has only recently been recognized in the literature, but has never been described in professional baseball pitchers. MATERIALS AND METHODS: A retrospective review of professional baseball players presenting to our institution over 5 years was performed. Patients with Bankart lesions or fractures were excluded. Preoperative MRI was retrospectively correlated with the clinical and arthroscopic findings, as avail

Iselin's disease: a case presentation of nonunion and review of the differential diagnosis.

The confusing pathology of the proximal fifth metatarsal can make Iselin's disease difficult to distinguish from Jones', avulsion, or stress fractures, or os vesalianum. While lselin's disease appears to be rare due to the scarcity of cases in the medical literature, this may be due to misdiagnosis. The case report discusses the course of treatment for a young male with Iselin's disease that developed into a nonunion. The differentiating factors of the above-mentioned fractures are discussed.

A clinical decision aid for patients with suspected midfacial and mandibular fractures (the REDUCTION-I study): a prospective multicentre cohort study.

PURPOSE: To assess physical examination findings related to maxillofacial trauma to identify patients at risk of midfacial and mandibular fractures and then to construct a clinical decision aid to rule out the presence of midfacial and mandibular fractures in emergency department patients. METHODS: We performed a prospective multicentre cohort study in four hospitals in the Netherlands, including consecutive patients with maxillofacial trauma. Each patient received a standardized physical examination consisting of 15 and 14 findings for midfacial and mandibular traumas, respectively. Conseque

Transverse posterior element fractures associated with torsion.

Six examples of a previously undescribed class of transverse vertebral element fractures are presented. These fractures differ from Chance and Smith fractures and their variants in the following respects: (1) the etiology is torsion and not flexion; (2) there is neither distraction of posterior ring fragments nor posterior ligament tears; (3) in contrast to Chance and Smith fractures, extension of the fracture into the vertebral body is absent or minimal; (4) the transverse process of the lumbar vertebra is avulsed at its base with a vertical fracture, not split horizontally. These fractures o

[Surgical tactics and the results of the treatment in patellar fractures].

The work analyses problems of the diagnosis, choice of therapeutic tactics, and the results of operative treatment in 119 patients with various fractures of the patella. Emergency operative treatment is recommended in patellar fractures with damage to the extension apparatus of the knee. The authors believe osteosynthesis with application of a "tightening" wire loop after Weber to be the method of choice in transverse and macrofragmental fractures of the patella (92 cases). Avulsion fractures of the lower patellar pole are an indication for removal of the freely lying bone fragments and subseq

Dental injuries in paediatric mandibular fracture patients.

PURPOSE: Dental injuries (DIs) are associated with facial fractures, particularly mandibular fractures. As paediatric mandibular fractures have special features, we sought to clarify the occurrence and types of DIs among this patient group. We assessed how age, injury type, and fracture location affects the occurrence of DIs and thereby defined which patients are most susceptible. METHODS: This retrospective study included patients < 18 years with a recent mandibular fracture. Predictor variables were gender, age group, mechanism of injury, type of mandibular fracture, and other associated fa

[Para-basal metatarsal V fracture: conservative functional treatment].

Fractures of the fifth metatarsal bone, mostly avulsion fractures that result from forced inversion of the plantar foot, are relatively common. There are two distinct types of fracture that involve the proximal portion of the fifth metatarsal bone. The first type is the proximal diaphyseal fracture, the so-called Jones fracture, and the second type involves the area of the metatarsal tuberosity. They are usually not displaced or only slightly displaced. The standard treatment is a below-the-knee walking cast Osteosynthesis for fractures of the fifth metatarsal bone is indicated only in cases w

Prevalence and Predictors of Peroneal Tendon Instability Accompanying Calcaneal Fractures.

BACKGROUND: We aimed to find the prevalence of peroneal tendon instability (PTI) accompanying different types of calcaneal fractures and to determine predictors of PTI based on preoperative CT scanning. METHODS: In a retrospective cross-sectional study, preoperative CT scans of 400 consecutive calcaneal fractures undergoing surgery were reviewed for comminuted fragments in the lateral gutter of the ankle, fractures at the tip of the lateral malleolus, dislocated peroneal tendons, excessive displacement of the lateral calcaneal wall, calcaneal fracture-dislocation, superior peroneal retinaculu

Bladder neck rupture following perineal bull horn injury: a surgical challenge.

Pelvic-abdominal injuries caused by goring are serious lesions which require rapid diagnosis and urgent treatment in the context of a polytraumatized patient. The simultaneous rupture of both the bladder and the prostatic-membranous urethra occurs in 10%-29% of males with pelvic fractures but bladder neck injuries in adults are rarer. Unstable pelvic fractures, bilateral fractures of the ischiopubic branches (also referred to as fractures from falling astride) and the diastasis of the pubic symphysis are those that have the greatest likelihood of injuring both the posterior urethra and the bla

Radiographic pitfalls in lower extremity trauma.

OBJECTIVE: Radiography remains the imaging standard for fracture detection after trauma. However, fractures continue to be the most common type of missed injuries. In this article, we describe common radiographic pitfalls in lower extremity trauma and describe strategies for dealing with them. CONCLUSION: Pitfalls include insufficient views, improperly positioned or technically imperfect radiographs, nondisplaced fractures, commonly missed locations, small avulsions portending large injury, sesamoid injuries, satisfaction of search, incomplete or faulty reasoning, and periprosthetic fractures

The Medial Malleolar Fleck Sign: A Case Report Highlighting Retinacular Origin Rather Than Deltoid Ligament Avulsion.

The medial malleolar fleck sign (MMFS) is a radiographic indicator of medial ankle instability, conventionally interpreted as an avulsion of the deltoid ligament from its tibial insertion. However, its precise anatomical origin remains unclear, with limited intraoperative confirmation reported in the literature. We present a case of a 51-year-old woman with a left ankle inversion injury, resulting in a Weber B fibular fracture, posterior malleolar fracture, and a cortical fragment consistent with MMFS, accompanied by medial clear space widening. Intraoperatively, the deltoid ligament was ruptu

Pathological Fracture of Calcaneum: A Case Report.

INTRODUCTION: Only a few reports have described calcaneus fractures after total knee arthroplasty (TKA). Therefore, in this report, we describe a case of calcaneal avulsion fracture that occurred 5 weeks after a TKA in a relatively young male patient with syphilis. CASE REPORT: A 63-year-old man with syphilis had Charcot joint of the right knee. The patient developed severe varus deformity and contracture and experienced severe pain in the knee. TKA was performed to alleviate the pain and improve the patient's gait. The patient noticed slight heel pain 4 weeks after the TKA, and a calcaneus a

[An unusual displacement fracture of the 1st metacarpal bone. Apropos of a case reviewed after 3 years].

An unusual fracture of the base of the first metacarpal with in contrast with Bennett's fracture displacement of the thumb and avulsion of the Abductor Pollicis Longus tendon is described after a "Rugby" trauma. Immediate repair and stabilization of the injured structures followed by specific rehabilitation, produced a satisfactory result with normal function of the thumb three years after the injury.

Concurrent avulsion of posterior cruciate ligament and semimembranosus: a case-based discussion and literature review.

A 30-year-old male sustained a road traffic accident and presented to our trauma centre with injuries to his pelvis and right knee. Radiology showed closed fractures of the right posterior wall and posterior column of the acetabulum and PCL bony avulsion with posteromedial tibial plateau osteochondral fracture, without any distal neurovascular deficit. He was managed with surgical intervention for both injuries. His hip and knee healed well with a good functional range of motion at 12 months of follow-up. The aim of highlighting this case is that it underscores the rarity of concurrent PCL and

Delayed reimplantation: a case report.

A 12-year-old boy reported to the clinic with a history of trauma, whereas playing 1 day before. On examination it was noted that the patient had an avulsed tooth in relation to 11, Class III fracture in relation to 12 and Class II fracture in relation to 21. The avulsed tooth was brought by the patient wrapped in a newspaper. Although the tooth was in highly unfavorable storage condition, it was decided to reimplant the teeth since the patient was young and to relieve him from psychological, cosmetic and functional trauma. Now after 24 months, the tooth is, functional, firm and free of sympto

Treatment of acute injuries of the triangular fibrocartilage complex associated with distal radioulnar joint instability.

One hundred thirty patients who had an injury of the triangular fibrocartilage complex (TFCC) with distal radioulnar joint (DRU) instability were treated operatively. The TFCC injury with DRUJ instability occurred as an isolated lesion in 20. After appropriate fracture stabilization, 86 patients were treated with closed reduction of the DRUJ and radioulnar transfixation with one to two Kirschner wires with the forearm in neutral rotation. In 19 patients the avulsed ulnar styloid was internally fixed, and the DRUJ supplemented with radioulnar transfixation. In 13 patients the ulnar styloid was

[Fracture of the acetabulum].

Severe trauma of the acetabulum is uncommon in children. However, it must be remembered that these apparently benign lesions can be responsible for serious complications. In a group of 31 children under the age of 16 years presenting with such lesions, 12 already had an adult type skeleton. Amongst the remaining children, 15 presented with fractures or fracture-dislocations of the acetabulum and 4 presented with marginal avulsions of the acetabular ridge. Two epiphysiodeses of the Y cartilage were observed, one in a 2 year old child, revealed by acetabular dysplasia at the age of 8 years, and

History of diagnostics and treatment of scapular fractures in children and adolescents and its clinical importance.

Almost 70 scapular fractures in children and adolescents, up to the age of 17 years, have been described in detail in the literature since 1839. The diagnosis of these injuries was based on autopsy, radiographs, CT and MRI examinations. The most frequent findings were fractures/epiphyseolyses of the coracoid, followed by fractures of the infraspinous part of the body and avulsion of the inferior angle of the scapular body. Less common were fractures of the acromion. Intra-articular fractures of the glenoid, or separation of an intact glenoid along the line of the anatomical or surgical necks,
