Severity of vertebral fracture reflects deterioration of bone microarchitecture.

INTRODUCTION: Bone microarchitecture, a component of bone strength, is generally measured on transiliac bone biopsy samples. The objective of this study was to determine whether assessment of four grades of vertebral fracture severity could serve as a noninvasive surrogate marker for trabecular bone volume and microarchitecture. METHODS: Baseline vertebral fracture severity was determined by semiquantitative assessment of spine radiographs from 190 postmenopausal women with osteoporosis. Bone-structure indices were obtained by 2D histomorphometry and 3D microcomputed tomography (CT) analyses.

Closed reduction of fractured anterior wall of the frontal bone.

The authors report two cases of closed reduction of the anterior wall of the frontal bone. In the first case, the fractured segment was successfully reduced with a small periosteal elevator. In the second case, a tip of the periosteal elevator was broken during maneuvering the fractured bone and lost into the sinus cavity. The sinus was opened to retrieve the broken piece and repaired by internal fixation. The technique has a few merits: (1) the procedure is simple; (2) the operation time is short; and (3) only a small incision is needed.

[Consideration of positioning in radiography for the waist of the scaphoid bone fractures].

PURPOSE: The most common type of scaphoid bone fracture is the transverse fracture of the waist (central 1/3). However, traditional Positioning frequently fail to visualize fracture line. This may be caused by the fact that X-rays cannot be irradiated perpendicularly onto the scaphoid long axis. In this study we searched with the optimal limb position for visualizing the waist of the scaphoid bone. METHOD: In 30 volunteers, the scaphoid bone was taken in the ulnoradial direction at 40 degrees ulnar deviation (group A) and 40 degrees ulnar deviation with the fist clenched (group B) , and measu

New implant designs for fracture fixation in osteoporotic bone.

Screws are one of the limiting factors for fixation of implants, particularly in poor bone quality. A class of new implants with an implant-bone-interface optimized regarding load transition by increasing the peripheral area might improve the anchorage of implants in osteoporotic bone. However, the shape of these implants requires new technologies for insertion. The goal of the work presented here was to analyze the relevant parameters regarding implant geometry and to demonstrate the effect of new procedures for their insertion. The investigation was divided into three parts: 1) implant desig

[Estimate of osteoporosis fracture risk with ultrasound bone assessment].

BACKGROUND: Multiple studies suggest that ultrasound measurement of the bone can be a rapid, cheap, and radiation-free alternative to determine the fracture risk. In this paper the ultrasound measurement of the bone was performed among 288 postmenopausal women, and the influence of gynecologic history and factors related to lifestyle on the obtained values was examined. PATIENTS AND METHODS: One hundred nineteen healthy postmenopausal women and 169 women with previous osteoporotic fractures were included in the study. Both weight and height were determined and a clinical questionnaire was adm

[A new type of iliac fracture caused by bone insufficiency].

BACKGROUND: The iliac bone is an uncommon localization for bone insufficiency fractures. We report a new type. CASE REPORT: A 51-year-old woman with post-menopause osteoporosis was seen for a fracture of the ischio-pubic branch of the iliac bone. X-ray also revealed an unknown fracture situated above the anterior superior iliac spine continuing cranially and medially towards the iliac crest. DISCUSSION: Three types of iliac fractures due to bone deficiency have been described. Type 1, (oblique iliac), the fracture is oblique beginning in the greater sciatic notch and extending a more or less

Cortical bone perfusion in plated fractured sheep tibiae.

The limited contact dynamic compression plate and partial contact plate were designed to decrease contact with cortical bone in an attempt to decrease cortical ischemia, remodeling, and eventual porosis under the plate after use of standard dynamic compression plates. This study quantified cortical bone blood flow beneath the plate with these three different designs in a sheep tibia fracture model. In 18 skeletally immature sheep, the right tibia was fractured and then was internally fixed with an interfragmentary screw and a dynamic compression plate, limited contact dynamic compression plate

The utility of a bone scan in the diagnosis of clinical scaphoid fracture.

The incidence of true scaphoid fracture and the value of radionuclide scan was evaluated in a 1-year retrospective review of 54 emergency department patients with a preliminary diagnosis of clinical scaphoid fracture (snuff box tenderness and negative initial X-ray study). All emergency department charts of patients with a discharge diagnosis of clinical scaphoid fracture were reviewed to determine the number ultimately found to have a true scaphoid fracture and to ascertain how the diagnosis was actually achieved. All patients were managed by immobilization in a thumb spica cast for a period

The healed Segond fracture: a characteristic residual bone excrescence.

This report describes the natural history of the Segond fracture and documents the radiographic appearance of the healed Segond fracture. The clinical and radiographic records of 129 patients with acute anterior cruciate ligament (ACL) injuries were reviewed. Four (3.1%) of these patients had Segond fractures. On follow-up radiographic examination, seven patients demonstrated a characteristic bone excrescence arising 3-6 mm inferior to the lateral tibial plateau. In four of the five patients for whom acute injury films were available this excrescence arose at the site of the earlier Segond fra

[An unusual cause of fracture of the femoral neck: bone sarcoidosis].

A case of pathological fracture of the femoral neck in a 84-year old female is presented. The histological examination of the femoral head showed sarcoidosis. This diagnosis was not known before the trauma. Other localisations of bone sarcoidosis were found in the tarsus and the fingers after a systematic X-ray check-up.

[Metal osteosynthesis of open fractures of the long tubular bones].

An analysis of treatment of 123 patients with open fractures of long tubular bones has shown that automobile crash is the most frequent cause of the trauma (67.5%). The prehospital aid was given only to 9.7% of the patients. The primary surgical treatment of the wounds was carried out by traditional rules, the local application of antibiotics for prophylactics of purulent complications did not justify itself. Better results of treatment of open fractures were obtained after metallo-osteosynthesis of open fractures of the femur bone. The highest percentage of complications were observed after e

Subchondral bone cysts with fractures of the extensor processes in a horse.

Probable cause of fracture in a Paint gelding was a congenital bone defect involving the extensor process area of the right and left forelimb distal phalanges. Radiographically and histologically, subchondral bone cysts at the fracture lines were suspected. These cysts were thought to have developed in association with abnormal ossification centers at the extensor process areas of the third phalanx. Partial avulsion of weak extensor processes would then occur from natural forces exerted on these weakened bony prominences via the common digital extensor tendon. Support for osteochondrosis as a

The architecture of cancellous and cortical bone in femoral neck fracture.

The architecture of cancellous bone as well as quantity and quality is considered important in maintaining mechanical integrity. To determine whether abnormalities of architecture occur in femoral neck fracture we measured trabecular width and number in iliac crest bone biopsies of 68 women with femoral fracture and compared them with data from a postmortem series of age-matched women without known bone disease. Cortical thickness was measured in 27 of the fracture patients and 17 controls. After exclusion of ten biopsies with increased osteoid surface no significant difference was seen in mea
