Traditionally, it was believed that both proximal and distal locking are essential for achieving stability during intra-medullary fixation for extra-capsular hip fractures. However, recent literature has presented varying perspectives on the necessity of distal locking. Distal locking plays a significant role in managing hip fractures with uncertainties regarding longitudinal and rotational stability. This includes cases of comminuted intertrochanteric fractures with subtrochanteric extension, reverse oblique and high oblique fractures, broad medullary canals, comminution of the lateral wall,

BACKGROUND: It has been advocated that reduction of nasal bone fractures should be followed by internal packing and/or external splinting. Despite the ample literature concerning the advantages and limitations of various splint types, the necessity and effectiveness of external splinting has not been well documented.
OBJECTIVE: To present the authors' experience and review the literature on treatment of nasal bone fractures, focusing on the indications and effectiveness of external splinting following closed reduction.
STUDY DESIGN: Retrospective analysis and literature review.

BACKGROUND: Subacute rupture of the flexor tendons secondary to distal radius fractures is well documented. Recently, accounts of flexor tendon rupture following open reduction internal fixation have been associated with volar plate fixation. However, discovery of an occult traumatic flexor tendon laceration during fixation of an acute distal radius fracture is not well described. This case indicates the importance of careful preoperative and intraoperative examination of the flexor tendons in the setting of comminuted distal radius fractures.

UNLABELLED: The relationship between radiographic and functional outcomes in older patients with distal radius fractures is controversial. We explored this relationship by assessing the influence of radiographic displacement and fracture comminution on the functional outcomes of these fractures. We also asked whether operative intervention and demographic factors (age, gender, duration of followup) influenced outcome. We examined 53 patients older than 55 years with distal radius fractures with various functional assessments: range of motion (ROM) and strength measurements, three subjective su

Ulnar nerve palsy subsequent to a fracture of the distal radius is extremely rare compared to a median nerve injury. The lesion tends to occur in younger patents with a high-energy mechanism of injury and a severe injury pattern consisting of wide displacement, comminution, combined distal ulnar fracture and open fracture. The mechanism of injury can contribute to a direct contusion and traction, compression secondary to prolonged edema and tissue fibrosis, intraneural fibrosis and laceration. We report 2 cases of progressive ulnar nerve palsy subsequent to closed fractures of the distal radiu

BACKGROUND: Treatment of complex three and four-part proximal humeral fractures with hemiarthroplasty in elderly patients has yielded mixed clinical results. Reverse shoulder arthroplasty has emerged as a treatment option for comminuted proximal humeral fractures for these patients. The purpose of the study was to perform a prospective evaluation of patient outcomes comparing hemiarthroplasty and reverse shoulder arthroplasty for the treatment of comminuted proximal humeral fractures in elderly patients.
METHODS: Fifty-three consecutive elderly patients (average age, 74.4 years) underwent an

Despite the common occurrence of radial head fractures, there is still a lack of consensus on which radial head fractures should be treated surgically. The radial head is an important secondary stabilizer in almost all directions. An insufficient radial head can lead to increased instability in varus-valgus and posterolateral rotatory directions, especially in a ligament-deficient elbow. The decision to perform surgery is often not dictated by the fracture pattern alone but also by the presence of associated injury. Comminution of the radial head and complete loss of cortical contact of at lea

PURPOSE: Segmental fractures, being associated with high energy trauma, are quite intimidating for the treating surgeon due to wide incidence of complications. The soft tissue damage and the fracture pattern in segmental fractures further disrupt the precarious nutrition of the floating intercalary segment, affecting fracture healing. Our study analyses the parameters influencing time to union in segmental fractures of femur.
METHODS: A retrospective analysis of all patients who presented to our level 1 trauma centre between January 2015 and December 2020 with traumatic segmental fractures of

Patellar fractures are rare in children and usually result from high-energy trauma. Treatment guidelines generally are the same as for patellar fractures in adults. Between 1942 and 1987, 66 children with patellar fractures were treated. Exclusion of those with < 2-year follow-up, osteochondral fractures, or incomplete records left 24 patients for long-term follow-up. Overall results were good in 13, fair in eight, and poor in three. Children with fractures of the ipsilateral femur and/or tibia and those with comminuted, displaced fractures had the poorest results.

BACKGROUND: The aim of the treatment of radial head comminuted fractures is the restoration of anatomical normalcy to avoid the risk of several complications such as joint instability. Among the options for the treatment of such fractures, it is worth mentioning osteosynthesis, resection of the radial head or prosthetic replacement. In the presence of comminution or severe dislocation of the fracture's fragments, as in our patient's type III Mason fracture, prosthesis implantation is the treatment of choice.

INTRODUCTION: Management of periarticular fractures requires a careful understanding of fracture characteristics and proper pre-operative planning. We suggest a new computerized tomography (CT) based classification system, which assists in planning surgical approaches and in implant selection, and provides an addition to the metaphyseal comminution along with the articular type.
MATERIALS AND METHODS: 50 patients with biplanar distal articular fractures were included in our study and were classified using the CT-based classification. The CT images and intra-operative photographs were reviewed

BACKGROUND: Lateral compression (LC)-type pelvic fractures encompass a wide spectrum of injuries. Current classification systems are poorly suited to help guide treatment and do not adequately describe the wide range of injuries seen in clinical practice.
QUESTIONS/PURPOSES: We therefore (1) defined the spectrum of injuries that compose LC fractures with respect to both anterior and posterior ring injuries, with particular focus on the morphology of sacral fractures, and (2) identified fracture patterns associated with displacement at presentation.
METHODS: We retrospectively reviewed 318 LC
