Mucoceles of the paranasal sinuses are benign, chronic, expanding lesions that characteristically develop because of obstruction of the sinus ostium. The frontal sinus is the most common sinus to be affected by a mucocele, which usually results from trauma or inflammatory processes. Patients with these lesions frequently present with visual complaints of decreased visual acuity, visual field abnormalities, proptosis, ptosis, displacement of the globe, or restricted ocular movements secondary to erosion through the thin bone of the superior orbit and compression on the globe. Often, intracrania

OBJECTIVE: To explore observing parameters of frontal sinus using reconstructive coronal nose bone sections of the head CT images and to establish the special identification codes.
METHODS: CT images of the frontal sinus were analyzed by using distance measurement and morphological description. The discrimination codes of frontal sinus identification were: the number of right partial septa, shape-the location of the central septa, shape-the number of left partial septa, shape; the number of arches of right upper scalloping (the location of the right highest scalloping, the location of the rig

Obliteration of the frontal sinus is frequently necessary in the appropriate treatment of major craniofacial trauma of the upper third of the face. Successful frontal sinus obliteration requires (1) meticulous removal of the frontal sinus mucosa, (2) permanent occlusion of the nasofrontal duct and (3) obliteration of the denuded cavity. The current techniques include implantation of autologous fat, bone or muscle. These techniques are effective when the appropriate guidelines are respected, but the problems of donor site morbidity, which has been as high as 5%, recipient site morbidity due to

Although there has been a significant decrease in the incidence of frontal sinus disease since the advent of antibiotics, frontal sinus infection still occurs and may follow a clinical course not unlike that seen during the preantibiotic era. Secondarily to cranial and intracranial invasion the following complications may occur: osteomyelitis, cavernous sinus thrombosis, meningitis, extradural, subdural and cerebral abscess. The proximity of the frontal sinus to both the dura and the marrow of the frontal bone, as well as a rich communicating venous system, lends support to the facility of int…

Seven patients with a corpus callosum lipoma associated with a frontal cranial defect with or without a superficial lipoma or a fronto-nasal dysplasia, were encountered over a period of 12 years. This group of patients is reported as these associated lesions may provide a lead to the cause of lipomas of the corpus callosum as well as of the different lesions in the fronto-facial region. From our experience and that of others, the frontal lesion may be an extracranial lipoma, a frontal bone defect, with or without an external lipoma, a lipomeningocele connecting the extracranial lipoma through …

BACKGROUND: Intracranial epidural hematoma (EDH) is frequently secondary to trauma, but in some rare cases, spontaneous EDH (SEDH) could develop without trauma. Cranial osteomyelitis is an uncommon osseous infection that most frequently presents as a postoperative complication but also rarely originates from paranasal sinusitis and can develop extracranially to form a subperiosteal abscess or intracranially to form an epidural, subdural, or cerebral abscess. Intracranial epidural abscess (EDA) is an uncommon infection that forms in the space between the cranial bone and dura mater. It is rare

Pott's puffy tumor (PPT) is a rare complication of frontal sinusitis, characterized by subperiosteal abscess formation and frontal bone osteomyelitis, with potential for severe intracranial spread. An 18-year-old male presented with forehead swelling and periorbital edema following recent sinusitis. Imaging revealed bilateral frontal epidural abscesses, osteomyelitis, and pachymeningitis. Surgical intervention included bicoronal craniotomy, frontal sinus cranialization, and abscess debridement. The presence of bilateral epidural abscesses and pachymeningitis represented an unusually severe pre…

The frontal sinuses are cavities inside the frontal bone located at the junction between the face and the cranial vault and close to the brain. Despite a long history of study, understanding of their origin and variation through evolution is limited. This work compares most hominin species' holotypes and other key individuals with extant hominids. It provides a unique and valuable perspective of the variation in sinuses position, shape, and dimensions based on a simple and reproducible methodology. We also observed a covariation between the size and shape of the sinuses and the underlying fron…

Frontal osteoplasty with exogenous material has been uniformly unsuccessful both experimentally and clinically. Our experiment was designed to test the long-term behavior of a new poly (tetrafluoroethylene)-carbon fiber implant material (Proplast) in a canine frontal sinus model. Varying conditions such as removal of the sinus mucoperiosteum, closure of the nasofrontal ducts, exposure of dura mater, and cerebrospinal fluid leaks were studied at intervals up to one year. Proplast was 100% successful in obliteration of the canine frontal sinus. The ultraporous nature allowed rapid vascularizatio

Osteoma, the most frequent tumor of the paranasal sinuses (PNS), is often asymptomatic. Men are more commonly affected than women. A 52-year-old patient complained of increasing incidence of headaches following the resolution of frontal sinusitis. Computer tomography demonstrated a pedunculated osteoma of the frontal sinus. Histomorphologically, PNS osteomas usually show cortical bone differentiation with sparse medullary space and a lamellar configuration. At 6 months following surgical resection, the patient was still symptom-free.

Facial penetrating injuries can cause retention of foreign bodies in the frontal sinus. This rare condition can remain underdiagnosed for years, since non-specific symptoms, such as headaches and nasal obstruction, can be developed. So far, removal by an endoscopic approach is the most preferred treatment option because it is less invasive with a short recovery time. However, removal by an open surgical approach remains the method of choice for large foreign bodies, especially in cases of coexistent non-reducible fractures of the anterior table of the frontal bone. We present a case where a co…

Composite frontal scalp defects involving the frontal bone and full thickness scalp can pose a reconstructive challenge. Often, they necessitate the use of microsurgical free tissue transfer, which can be physiologically demanding in high-risk patients with multiple comorbidities, with exposure to prolonged general anesthesia time and increased surgical morbidity. The experience of a single surgeon was reviewed with reconstruction of composite frontal scalp wounds in 4 patients with structural framework and a bipedicled scalp advancement (ie, "bucket handle") flap, thus obviating the need for …

A 61-year-old Chinese man presented to the emergency department with a 1-day history of painful swelling of the right eyelid with loss of vi … hematoma was probably a result of an acute hemorrhage into the mucocele of an isolated cell in the frontal bone. However, in patients with a history of multiple myeloma, it is important to consider lytic bony involvement as the cause of an orbital hemorrhage.

The craniostenotic birth defects seen in patients with Crouzon syndrome have prompted this developmental study on the system of articulations between the human frontal, sphenoid, and ethmoid bones. The Crouzon facies, including midfacial hypoplasia and exorbitism, have been linked to the premature synostosis of calvarial sutures. However, considerable evidence shows that midfacial positioning is linked to increasing length of the midline cranial base. Thirty-seven typical-for-age (8 to 29 weeks) embryos were histologically prepared, read serially, and three-dimensionally reconstructed to map t

We report a case of a 65-year-old man with intradipolic epidermoid cyst of the frontal bone. The patient presented with headache and swelling of the forehead. Skull x-ray showed massive destruction of the frontal bone. Magnetic resonance imaging (MRI) revealed an epidural mass without any encroachment of the dura mater. The cystic tumor was removed followed by cranipolasty with methylmethacrylate. Four years later, the tumor recurred in the same region. At the second surgery the bony defect was again replaced with methylemthacrylate plate. Four years after the second operation, the tumor reeme…

Intracranial hemorrhage in neonates is often found in either the subependymal area or the subdural space. The former is observed particularl … ing surgery led to the conclusion that the hemorrhages were caused by the moulding which forced the frontal bone to slip under the parietal bone at the coronal suture and then press on the fragile cerebral vasculature of the neonates, thus causing contusion.

Three cases are presented illustrating various uses of pericranial flaps. In a patient with Apert's syndrome, a large pericranial flap was folded over bone pâté harvested from the upper frontal bone to improve forehead contour. Silicone and acrylic implants had been inserted in 2 other patients, 1 of whom had previously undergone radiation therapy for the correction of cranial defects. Both were lost due to infection. Autogenous bone grafts covered by pericranial flaps healed without difficulty, emphasizing the superiority of autogenous bone grafts over foreign substances in craniofacial recon…

Fibrous dysplasia is a genetic and rare bone disorder affecting the young subject. Its pathophysiology involves genetic mutations leading to defective skeletal development with fibrous and medullary proliferations. Cephalic extremity is involved in one third of cases. We here report a case of surgically treated fibrous dysplasia of the frontal and ethmoid sinuses. The study involved a 31-year-old man presenting with chronic holocranial headaches. Computed tomography (CT) scan showed voluminous bilateral hyperdense expansive process in the frontal and ethmoid sinuses. Surgery was performed on t

Rhinosporidiosis is a chronic granulomatous infection caused by Rhinosporidium seeberi and mainly involves nasal and ocular mucosa. Bony involvement in rhinosporidiosis is very rare. A young male, previously operated for nasal rhinosporidiosis, presented with two bony swellings on the forehead and multiple subcutaneous lesions on the right lower limb. The diagnosis of disseminated cutaneous rhinosporidiosis with frontal bone involvement was made with the help of fine needle aspiration cytology (FNAC), histopathology, and computed tomography (CT) scan head. Wide excision of the bony lesion was …
