A longitudinal quantitative analysis of gait in patients with SCA-12.

INTRODUCTION: Spinocerebellar ataxia type 12 (SCA 12) is characterized by late onset tremor, ataxia and pyramidal signs. Parkinsonism and cognitive decline may appear with time. It is considered as slowly progressive but temporal evolution of symptoms has not been reported. METHOD: We report the evolution of symptoms in three SCA12 patients followed over a range of 5-6 years. We focused on the evolution of gait abnormality as it becomes the most disabling symptom as disease advances. Two-dimensional gait parameters were studied using an electronic walkway at various time points to measure obj…

Varenicline for the Treatment of Postural and Gait Dysfunction in Parkinson Disease.

OBJECTIVE: To determine whether varenicline is effective for the balance in Parkinson disease (PD). METHODS: This was an investigator-initiated, double-blind, placebo-controlled study. Participants with a clinical diagnosis of PD were randomized to receive varenicline or placebo for 8 weeks. After dose escalation, participants took 1 mg of drug twice daily until the end of the study. Patients with severe tremor were excluded. Primary outcome was a change on the Berg Balance Scale (BBS) from baseline to 8 weeks. The BBS is a 14-item measure consisting of basic balance tasks. The study had a se

Olfactory Deficits in the Freezing of Gait Phenotype of Parkinson's Disease.

Background: Olfactory dysfunction often occurs before motor onset in Parkinson's disease (PD) and can be detected with the University of Pennsylvania Smell Identification Test (UPSIT). Based on the Braak hypothesis, the olfactory bulb is one of two sites where disease pathology may start and spread to deeper brain structures. Objective: To evaluate whether a specific pattern of odorant identification on the UPSIT discriminated Parkinson's disease patients with and without freezing of gait. Methods: One hundred and twenty four consecutive participants (33 controls, 31 non-freezers, and 60 freez…

Improvement of "Bouncy Gait" in Lance-Adams Syndrome with Perampanel.

Lance-Adams syndrome (LAS) is chronic post-hypoxic myoclonus that is often associated with sudden lapses in muscle tone (negative myoclonus) in the legs, causing a disabling "bouncy gait." Given its relative rarity, there are no controlled treatment studies of LAS. The majority of cases require polypharmacy management, with an incomplete response. "Bouncy gait," in particular, is notoriously medication-refractory. Here, we report a patient with long-standing LAS who improved markedly when low-dose perampanel was added to his existing treatment regime consisting of clonazepam, levetiracetam, so…

Characteristic behaviors associated with gait of individuals with Rett syndrome.

BACKGROUND: Individuals with Rett syndrome (RTT) exhibit impaired motor performance and gait performance, leading to decreased quality of life. Currently, there is no robust observational instrument to identify gait characteristics in RTT. Current scales are limited as individuals with intellectual disorders may be unable to understand instructions. Our primary purpose was to utilize video analysis to characterize the behaviors associated with walking in individuals with RTT and explore the relationship between behaviors during overground and during treadmill walking. METHODS: Fourteen indepe…

[The use of donepezil in gait disorders in eldery patients with dementia].

Donepezil is the most commonly used cholinesterase inhibitor. The indication for its use is Alzheimer's disease. A number of clinical studies have shown its effectiveness in vascular dementia, dementia with Lewy bodies, Parkinson's disease with dementia, dementia due to traumatic brain injury. It is shown that dementia is a risk factor for falls, and standard measures to prevent falls in the elderly are ineffective in patients with cognitive impairment. This article provides a review of publications on the influence of donepezil on gait and balance. Most authors agree that donepezil is able to…

The effects of simulated knee arthrodesis on gait kinematics and kinetics.

Knee arthrodesis is commonly performed to treat joint pain and instability in cases of multiple failed knee replacement surgeries, bone loss, traumatic injury, infection, or loss of the quadriceps extensor mechanism, but its mechanics are poorly understood. This study quantified the changes in gait kinematics and kinetics induced by simulating knee arthrodesis. A total of 10 healthy subjects (M/F, n = 5/5; aged 18-23) with no history of gait abnormalities were recruited for this study, and knee arthrodesis was simulated for 0 through 24 h using an immobilizing knee brace. Gait analysis was con…

Parkinsonism is a Phenotypical Signature of Amyloidopathy in Patients with Gait Disorders.

BACKGROUND: Central neurological gait abnormalities (CNGA) are frequently associated with parkinsonism in older adults. However, the neuropathological substrates and the clinical impact of parkinsonism have been not described in CNGA. OBJECTIVE: This cross-sectional study aims to compare the CSF total tau, Aβ1-42, and phosphorylated tau levels in non-Parkinson's disease (PD) patients with CNGA with and without parkinsonism and to study the clinical impact of parkinsonism on gait and cognition. METHODS: CSF biomarkers were measured by ELISA in 49 non-PD patients with CNGA (77.7±6.6 years; 32.…

Physiotherapy Effects in Gait Speed in Patients with Knee Osteoarthritis.

BACKGROUND: Knee osteoarthritis is a chronic degenerative disease, known as the most common cause of difficulty walking in older adults and subsequently is associated with slow walking. Also one of the main symptoms is a degenerative and mechanics type of pain. Pain is very noticeable while walking in rugged terrain, during ascent and descent of stairs, when changing from sitting to standing position as well as staying in one position for a long time. Many studies have shown that the strength of the quadriceps femoris muscle can affect gait, by improving or weakening it. Kinesio Tape is a phys…

How tandem gait stumbled into the neurological exam: a review.

Tandem gait testing is an integral part of the neurological exam. It is informative in a wide variety of disorders ranging from cerebellar disease to vestibular and peripheral neuropathies, parkinsonism, and other neurodegenerative conditions. We discuss the history and development of tandem gait testing as well as its technique, utility, and limitations in the assessment of neurological conditions. Tandem gait has emerged as a tool in the assessment of cerebellar disease, Huntington disease, idiopathic Parkinson's disease, atypical parkinsonism, peripheral neuropathies, and vestibulopathies. …

[Disturbances of gait and postural stability in chronic cerebral ischemia].

Disturbances of gait and postural stability are characteristic of the chronic progressive vascular lesion of the brain observed in moderate to severe stages of cerebrovascular disease. Disconnections between the prefrontal cortex, basal ganglia and cerebellum due to the damage of the white matter (vascular leukoencephalopathy) underlie disturbances of gait and postural stability (shorter stride length, wider gait). There are difficulties in starting with possible stoppings during the walking, waiting in front of small barriers as well as difficulties in concentration (e.g., talk during walking…

A systematic review of the gait characteristics associated with Cerebellar Ataxia.

BACKGROUND: Cerebellar Ataxias are a group of gait disorders resulting from dysfunction of the cerebellum, commonly characterised by slowly progressing incoordination that manifests as problems with balance and walking leading to considerable disability. There is increasing acceptance of gait analysis techniques to quantify subtle gait characteristics that are unmeasurable by current clinical methods This systematic review aims to identify the gait characteristics able to differentiate between Cerebellar Ataxia and healthy controls. METHODS: Following systematic search and critical appraisal …

Investigation of factors impacting mobility and gait in Parkinson disease.

Mobility and gait limitations are major issues for people with Parkinson disease (PD). Identification of factors that contribute to these impairments may inform treatment and intervention strategies. In this study we investigated factors that predict mobility and gait impairment in PD. Participants with mild to moderate PD and without dementia (n=114) were tested in one session 'off' medication. Mobility measures included the 6-Minute Walk test and Timed-Up-and-Go. Gait velocity was collected in four conditions: forward preferred speed, forward dual task, forward fast as possible and backward …

Gait coordination impairment is associated with mobility in older adults.

BACKGROUND: Impairments to body systems contribute to mobility limitations. The objective of this study was to determine whether impaired gait coordination, as measured by the Phase Coordination Index (PCI), is significantly associated with mobility limitations in older adults, even after adjusting for other gait features. METHODS: We conducted a cross-sectional analysis of performance-based measures of mobility in older adults (N=164) 77-101years of age, participants in the population-based MOBILIZE Boston Study. Mobility outcomes included the Short Physical Performance Battery (SPPB) and ea…

Effect of Tibia Marker Placement on Kinematics in Pathological Gait.

This study aimed to determine the effect of tibia marker placement on walking kinematics in children with pathological gait. Three-dimensional lower extremity gait data were collected using both a traditional tibia wand (protruding laterally from the distal shank) and a tibia crest marker on 25 children with pathological gait. Kinematic variables during walking and quiet standing were calculated using each marker and the "Plug-in Gait" implementation of the conventional gait model. For walking, average differences in kinematics between tibia markers ranged from 0.1° to 1.9° at the knee and ank…

[Loss of balance, gait disturbance and falls in an elderly woman].

We report the diagnostic workup of a 75-year old woman seen in the outpatient clinic and complaining of a very common symptom that is walking difficulty and falls. We stress the complexity of the clinical examination and of the pathophysiology of equilibrium and gait disorders. They are numerous aetiologies that are very often associated in elderly patients. The therapeutic approach is necessarily multidisciplinary, both medical and paramedical. In the particular case of the patient illustrated in this vignette, the diagnosis was myofibrillar myopathy, a rare and recently described muscle dise…
