Downstream Impact of a Sleep Surgery Program; Optimizing Patient Care and Generating Clinical Volume.

OBJECTIVES: In recent years, a growing number of Otolaryngologists have incorporated the management of sleep disorders into their practice. We aim to assess the downstream impact of a Sleep Surgery division within a department of Otolaryngology. METHODS: We assess all new patients with a diagnosis of sleep apnea presenting to our division of Sleep Surgery, consisting of two surgeons, from January 1, 2021 through December 31, 2021. Our outcome measures included testing and procedures that occurred because of our workup of new patients presenting to our program. We exclusively assessed professi

Greening orthopaedic surgery: Carbon footprint, waste generation, environmental impact, and mitigation strategies.

BACKGROUND: Orthopaedic surgery is among the most resource-intensive areas of healthcare, generating substantial waste, energy consumption, and carbon emissions. Operating rooms contribute disproportionately to a hospital's environmental footprint due to their high energy loads, extensive use of consumables, and complex supply chains. Despite rising global attention to sustainable healthcare, evidence specific to orthopaedic practice remains fragmented. OBJECTIVES: This narrative review synthesises current evidence on the environmental impact of orthopaedic surgery, identifies major contribut

Anomalous circumflex artery: a benign variant generating a malignant potential after valve surgery.

Coronary artery anomalies are a diverse group of congenital disorders presenting with highly variable clinical manifestations. The anomalous origin of left circumflex artery from the right coronary sinus following a retro-aortic trajectory is a well-recognized anatomic variation. Despite its benign course, it can prove lethal in association with valvular surgery. When single aortic valve replacement or combined with mitral valve replacement is performed, the aberrant coronary vessel may be compressed by or between the prosthetic rings triggering postoperative lateral myocardial ischemia. If le

'Moderate global aphasia': A generalized decline of language processing caused by glioma surgery but not stroke.

Unlike stroke, neurosurgical removal of left-hemisphere gliomas acts upon a reorganized language network and involves brain areas rarely damaged by stroke. We addressed whether this causes the profiles of neurosurgery- and stroke-induced language impairments to be distinct. K-means clustering of language assessment data (neurosurgery cohort: N = 88, stroke cohort: N = 95) identified similar profiles in both cohorts. But critically, a cluster of individuals with specific phonological deficits was only evident in the stroke but not in the neurosurgery cohort. Thus, phonological deficits are less

Safety of heat generated by endoscope light sources in simulated transcanal endoscopic ear surgery.

OBJECTIVE: To determine whether heat generated by endoscope light sources during ear surgery is safe. METHODS: Transcanal endoscopic ear surgery (TEES) was simulated using 2.7-mm or 4-mm endoscopes coupled to xenon or LED light sources and a 3D model of human temporal bone. The endoscope tip was fixed at the center of tympanic annulus. Light sources were tested at clinical (30% for xenon and 40% for LED) and 100% settings. Temperatures were measured using thermocouples attached to the endoscope tip and three points within the middle ear cavity: promontory, horizontal portion of the facial ner
