Primary malignant mediastinal germ cell tumours are rare and considered to have poorer prognosis compared with those arising from gonads. Eighteen patients with primary mediastinal germ cell tumour were treated over an 1-year period; 9 had seminoma and 9 non-seminoma. Eight patients, 4 each with seminoma and non-seminoma underwent initial tumour excision and the rest had biopsy only. All patients received cisplatin-based chemotherapy. All patients with seminoma received consolidation radiotherapy to mediastinum. Three patients with non-seminoma received radiotherapy following partial response.

This article deals with observations of the clinical behavior of metastatic germ cell cancers of testicular origin. Therefore, when we speak of biology of metastases, we refer to that seen by the clinician as opposed to the laboratory scientist. BACKGROUND: Reports have implied etoposide as the cause of secondary leukaemia in patients treated for germ cell cancer.
PATIENTS AND METHODS: Between 1979 and 1992, 679 male patients with germ cell cancer received etoposide containing chemotherapy.
RESULTS: Six of 679 patients developed acute myeloid leukaemia (relative risk 150; CI: 55-326). Germ cell tumors (GCT) are neoplasms that originate predominately in the ovary and testis. Tumors of germ cell origin only very uncommonly arise in extragonadal sites. We have diagnosed ten primary malignant extragonadal GCT arising in the mediastinum, retroperitoneum, liver, and sacrococcygeal region by fine-needle aspiration biopsy (FNAB). Fine needle aspiration cytology was performed on six patients with malignant mediastinal germ cell tumor: 1 pure seminoma, 1 pure embryonal carcinoma, 1 pure yolk sac tumor and 3 mixed germ cell tumors containing teratoma. A total of 843 consecutive boys (median age 12.7 years) who had undergone testicular biopsy at operation for undescended testis was followed into adulthood (median age 25.2 years) to examine for testicular germ cell neoplasia. Germ cell alkaline phosphatase (also termed placental-like alkaline phosphatase or Nagao isoenzyme) was assayed in seminal plasma using monoclonal antibody-based enzyme immunoassays in samples obtained both before and after vasectomy. In all cases, germ cell alkaline phosphatase fell to undetectable levels within 14 days of operation. A fall in seminal plasma germ cell alkaline phosphatase may thus be an early indication of successful vasectomy.

A patient with a mediastinal mass detected by plain chest roentgenogram is presented. The echocardiographic-Doppler examination and the computed tomography precise the gross nature and extent of the tumour and its anatomical location producing obstruction of the right atrium and modifying the blood flow pattern in this chamber. From a review of a series of 1,474 intracranial tumors occurring in children, we identified 49 patients (3.3%) with primary intracranial germ cell tumors: 65% germinomatous, 26% nongerminomatous (8 teratomas, 3 endodermal sinus, and 2 choriocarcinomas), and 8 degrees 10 mixed. We report the first cytogenetic investigation of a rare pineal region mixed germ cell tumor. The mean modal number was 78. Multiple numerical and structural abnormalities were noted. Chromosomes 21 and 1q were consistently overrepresented, and chromosome 13 was underrepresented. A translocation involving chromosome 11 occurred in all metaphases examined. The Y chromosome was lost, and three copies of the X chromosome were present. No isochromosome 12p was identified, but four copies of chromosome 12 were present.

Germ cell tumor exists as a primary entity in the anterior mediastinum.
