Mechanism, pathophysiology, diagnosis, and management of renal transplant rejection.

In recent years, there has been a steady progress in basic research (immunogenetics and cellular immunology) that helped us in understanding the mechanisms underlying allograft rejection. Several laboratory tests were developed, and the results were shown to correlate with clinical rejection. However, most of these studies have not found a place in clinical practice because of their nonspecificity, lack of sensitivity, time lag, added expense, and inconvenience. The commonly employed diagnostic tests (i.e., renal transplant ultrasound and 131I hippuran scintigram) are helpful in differentiatin…

Small bowel transplantation.

The small bowel, the largest lymphoid organ in the body and host to a myriad of foreign antigens has presented a formidable challenge for transplantation. In the past 5 years, small bowel transplantation has become a viable clinical entity. Successful transplantation of the small bowel has been achieved as part of multivisceral grafting, combined small bowel and liver grafts and even as isolated small bowel grafts. Allograft rejection remains an important post-transplantation problem, although graft versus host disease has been less of a clinical problem than initially feared. Markers that may…

Animal models of high-risk corneal transplantation: A comprehensive review.

Over the past century, corneal transplantation has become the most commonly performed allogeneic solid tissue transplantation. Although more than 80% of the corneal transplantations have favorable outcomes, immune-mediated rejection continues to be the major cause of failure in well over 50% of graft recipients that have inflamed and vascularized host beds. Over the past two decades, the progress in our understanding of the immunological pathways that mediate graft rejection has aided in the development of novel therapeutic strategies. In order to successfully test the efficacy of these interv…

Topical bevacizumab for corneal neovascularization after penetrating keratoplasty.

PURPOSE: To evaluate the efficacy of topical bevacizumab for corneal neovascularization in graft rejection. METHODS: A 55-year-old man presented with corneal neovascularization and graft rejection 6 months following penetrating keratoplasty (triple procedure) in the left eye. His best-corrected visual acuity (BCVA) was counting fingers at 1 meter. He was administered topical bevacizumab (4 mg/4 mL) in a dose of one drop twice a day for 15 days. No adjunct therapy was given during bevacizumab administration. RESULTS: After 1 month, his BCVA improved to 20/120. Corneal vascularization and stro…

IL-6 inhibition prevents costimulation blockade-resistant allograft rejection in T cell-depleted recipients by promoting intragraft immune regulation in mice.

The efficacy of costimulation blockade with CTLA4-Ig (belatacept) in transplantation is limited due to T cell-mediated rejection, which also persists after induction with anti-thymocyte globulin (ATG). Here, we investigate why ATG fails to prevent costimulation blockade-resistant rejection and how this barrier can be overcome. ATG did not prevent graft rejection in a murine heart transplant model of CTLA4-Ig therapy and induced a pro-inflammatory cytokine environment. While ATG improved the balance between regulatory T cells (Treg) and effector T cells in the spleen, it had no such effect with…

TGF-beta, IL-6, IL-17 and CTGF direct multiple pathologies of chronic cardiac allograft rejection.

Cardiac transplantation is an effective treatment for heart failure refractive to therapy. Although immunosuppressive therapeutics have increased first year survival rates, chronic rejection remains a significant barrier to long-term graft survival. Chronic rejection manifests as patchy interstitial fibrosis, vascular occlusion and progressive loss of graft function. Recent evidence from experimental and patient studies suggests that the development of cardiomyocyte hypertrophy is another hallmark of chronic cardiac allograft rejection. This pathologic hypertrophy is tightly linked to the immu…

Comparison of two different immunosuppressive dosages of methylprednisolone in clinical renal transplantation.

In order to study the immunosuppressive effect of methylprednisolone, thirty cadaver kidney recipients where randomly allocated into two groups: the low dose control group and the high dose experimental group. In 12 patients in the control group a clinical rejection episode occurred within two weeks of the transplantation, 3 rejections were irreversible, whereas 7 patients rejected in the experimental group, there were no irreversible rejections. In the low dose group the clinical rejection occurred significantly earlier. Fine needle aspiration biopsies of the grafts showed that the number of

Reporter immunogenicity as a defined rejection antigen: a noninvasive platform for skin transplant immunobiology.

BACKGROUND: Preclinical transplantation models largely rely on macroscopic or terminal assessments of graft fate, limiting longitudinal evaluation of immune rejection and tolerance induction. Reporter proteins such as enhanced green fluorescent protein (eGFP) and firefly luciferase enable real-time, non-invasive imaging of transplanted tissues, but their utility in immunocompetent hosts is constrained by their immunogenicity as non-murine xenoantigens capable of eliciting robust adaptive immune responses. METHODS: Full-thickness skin grafts from CAG-eGFP-luciferase donor mice were transplante

Long-Term Outcomes of Belatacept Versus Tacrolimus Following T-Cell Depleting Induction in Adult Kidney Transplantation.

BACKGROUND: Belatacept shows promise as an alternative immunosuppressant without the nephrotoxicity of calcineurin inhibitors. Avoiding nephrotoxicity is important with the expanding use of organs at risk of marginal graft function. To date, no large studies have compared belatacept directly with tacrolimus after T-cell depleting induction in renal transplantation. METHODS: The Standard Transplant Analysis and Research file was used to compare adult kidney transplant recipients induced with T-cell depleting agents treated with belatacept to propensity score-matched recipients treated with tac

Effect of Histocompatibility Y Antigen Matching on Graft Survival in Primary Penetrating Keratoplasty.

PURPOSE: To investigate the influence of histocompatibility Y (H-Y) antigen matching on corneal graft survival in primary penetrating kerato … he H-Y-compatible group (χ = 3.9, P = 0.049). CONCLUSIONS: H-Y antigen matching does not influence graft rejection and failure in cases of primary PK. However, matching the H-Y antigen could help reduce graft rejection, especially in preoperatively high-risk patients.

Effects of induction therapy with alemtuzumab versus antithymocyte globulin among highly sensitized kidney transplant candidates.

We retrospectively compared induction therapy utilizing alemtuzumab and antithymoglobulin (ATG) in high-risk kidney transplant recipients in our center. Two hundred and fifty-one patients underwent kidney transplantation between 2009 and 2012. The high-risk patients were defined as those who had two or more times kidney transplantation and/or more than 30% panel reactive antibody. We studied 130 high-risk kidney transplant candidate; 58 (44.6%) patients received induction immunosuppressive therapy with alemtuzumab, and 72 (55.4%) with ATG. Delayed graft function developed in 11 patients receiv

Causes of late renal allograft failure in the ciclosporin era.

A single center experience of 514 ciclosporin-treated renal allografts which survived longer than 1 year was reviewed in order to analyze the causes of renal allograft loss beyond the 1st year post-transplantation and the contribution of selected parameters to long-term survival. 83 grafts were lost between 1 and 5 years with the most common causes of graft loss being chronic rejection (54%), death (14%), noncompliance (13%) and sepsis (11%). Actuarial 5-year graft survival rates, decaying from 100% at 1 year, of living related and cadaveric grafts were 88.6 and 79.5%, respectively. Parameters

Kidney graft survival in transfused and nontransfused sibling beagle dogs.

In 6 pairs of sibling Beagle dogs, 1 kidney was exchanged between pairs, and contralateral nephrectomy was done. Previously, one dog of each pair was given blood transfusions from the donor of its allograft. All dogs were given azathioprine and prednisone postoperatively for immunosuppression. Four of 6 dogs given pretransplantation transfusions were healthy 1 year after surgical manipulation was done, and 2 died for reasons other than graft rejection. Of the 6 dogs that were not given pretransplantation transfusions, 3 were healthy after 1 years, but 2 were euthanatized because of graft rejec…

Preventing Rejection of the Kidney Transplant.

With increasing knowledge of immunologic factors and with the advent of potent immunosuppressive agents, the last several decades have seen significantly improved kidney allograft survival. However, despite overall improved short to medium-term allograft survival, long-term allograft outcomes remain unsatisfactory. A large body of literature implicates acute and chronic rejection as independent risk factors for graft loss. In this article, we review measures taken at various stages in the kidney transplant process to minimize the risk of rejection. In the pre-transplant phase, it is imperative

Effect of steroid-free low concentration calcineurin inhibitor maintenance immunosuppression regimen on renal allograft histopathology and function.

BACKGROUND: The most common cause of late kidney transplant failure is insidiously progressive renal dysfunction associated with organ scarring and fibrosis. Advanced donor age, delayed graft function, calcineurin toxicity and repeated acute rejection episodes are risk factors for this pathophysiology. METHODS: We employed 3, 12 and 24 months surveillance renal biopsies, scored using the Chronic Allograft Damage Index (CADI), with periodic estimates of glomerular filtration rate (eGFR) to assess the effect of a steroid-free maintenance immunosuppression regimen on allograft histology and func

Banff criteria as predictors of outcome following acute renal allograft rejection.

BACKGROUND: The Banff classification of renal allograft rejection grades acute tubulointerstitial rejection (AIR) by severity of tubulitis and acute vascular rejection (AVR) by severity of arteritis. The intensity of tubulitis has not, however, been demonstrated to be of prognostic value and other features such as glomerulitis and eosinophil infiltration are of uncertain significance. This study was performed in order to determine the clinical value of this pathological classification. METHODS: Banff criteria were correlated with outcome in 134 consecutive graft recipients transplanted in our…
