National stakeholder consultation on how to measure care home residents' quality of life.

BACKGROUND: The Developing research resources And minimum data set for Care Homes' Adoption and use or DACHA study aims to create a prototype minimum data set combining residents' information recorded by care homes with their data held in health and social care data sets. The DACHA minimum data set will contain information on quality of life. Internationally and in the UK, there is no consensus on collecting information on quality of life in a standardised format equivalent to the consensus for health measures. OBJECTIVE: This paper describes an online consultation with stakeholder representa

Role of home-based pulmonary rehabilitation programs for disease progression and quality of life in patients with stable bronchiectasis: a single-center RCT.

OBJECTIVE: This study aims to evaluate the effectiveness of a home-based pulmonary rehabilitation program for patients with stable bronchiectasis in improving their quality of life and reducing acute exacerbations. METHODS: A randomized controlled trial design was employed, with patients randomly assigned to an intervention group (receiving the pre-designed home-based pulmonary rehabilitation program for bronchiectasis) or a control group (receiving standard respiratory care). The effects of the program on quality of life, lung function indicators, and the frequency of acute exacerbations wer

[Postoperative care following hip fracture in a hospital or a psychogeriatric nursing home? A comparative study of short or long-stay demented hospitalized elderly].

In this article the question under discussion is, whether a short stay in hospital, for an inmate of a psychogeriatric nursing home with a hip-fracture, shows better results for mortality, morbidity and mobility than a long stay. Two matched groups, of 27 inmates each, have been compared retrospectively. The first remained in hospital for a week or more (long stay), the second for a period of less than a week (short stay). There was no significant difference in mortality risk between both groups. There were less urinary infections in the short-stay group and one month after the operation mobil

Non-invasive hemodynamic monitoring as a guide to drug treatment of uncontrolled hypertensive patients: effects on home blood pressure in the BEAUTY study.

BACKGROUND: In the BEAUTY study we investigated whether utilizing non-invasive monitoring of hemodynamic parameters combined with a drug selection algorithm (integrated hemodynamic management-IHM) compared to conventional drug selection may improve home BP in patients with uncontrolled hypertension. METHODS: Uncontrolled (office systolic blood pressure (SBP) > 140 mmHg and ambulatory daytime SBP >135 mmHg while taking ≥2 antihypertensive drugs) essential hypertensive patients were referred to 5 European Hypertension Excellence Centers and, if eligible, were randomized into IHM-guided vs conve

Nursing care needs and services utilised by home-dwelling elderly with complex health problems: observational study.

BACKGROUND: In Norway, as in many Western countries, a shift from institutional care to home care is taking place. Our knowledge is limited regarding which needs for nursing interventions patients being cared for in their home have, and how they are met. We aimed at assessing aspects of health and function in a representative sample of the most vulnerable home-dwelling elderly, to identify their needs for nursing interventions and how these needs were met. METHODS: In this observational study we included patients aged 75+ living in their own homes in Oslo, who received daily home care, had th

Education and Provision of a Pest Management Tool Kit to Residents in Low-Income Housing: Effect on Cockroach Reduction.

High rates of pest infestations are frequently reported in low-income homes in the U.S. However, even with access to effective insecticides, most residents are unaware of how to properly control indoor pests. We designed and evaluated the effect of in-home integrated pest management (IPM) education and provision of pest management tool kit on German cockroach (Blattella germanica L.) infestations. Twenty-nine apartments were enrolled based on the presence of German cockroaches. They were divided into two groups: an intervention group (12) and a control group (17). The intervention group receiv

Home rectal evacuation prior to a colorectal clinic appointment: a prospective, randomized, controlled, single-blind trial.

OBJECTIVE: The presence of faeces in the rectum can limit the effectiveness of diagnostic rigid sigmoidoscopy and therapeutic proctoscopy. This study was undertaken to determine whether home self-administration of a Microlette Micro-enema® or two glycerine suppositories improves the view on rigid sigmoidoscopy and to assess patient acceptability. PATIENTS AND METHODS: Patients referred with colorectal symptoms were prospectively randomized into three groups of 50. Patients in Groups I and II were sent a Microlette Micro-enema® or two glycerine suppositories, respectively, along with instructi

The Benefits of Licensed Midwifery and Community Birth Among BIPOC Birthing People in New Mexico.

BACKGROUND: Black, Indigenous, and people of color (BIPOC) families comprise a disproportionately low percentage of home and freestanding birth center births in New Mexico (NM), despite NM Medicaid coverage of care by Licensed Midwives (LMs) in these settings. The purpose of this study was to examine why low income BIPOC seek out LM care, how they benefit from this model of care, and which factors facilitate and obstruct access. METHODS: We conducted 7 focus groups with 55 low income BIPOC individuals who had birthed in New Mexico in the past 5 years. Participants in four of the groups intend

Countering social exclusion through inclusive homecare provision: Utilising a participatory life-course approach to influence policy.

Homecare models can be effective mechanisms in securing wellbeing and ageing-in-place goals prevalent in health policy agendas. However, the relevance and utility of these models for those living in socially and environmentally insecure conditions has rarely been considered. This is marked by an assumption of in-situ care delivery in private dwellings, and the absence of such groups from homecare development processes. This study aims to harnesses the experiences and preferences of older people in homelessness (OH), older Irish Travellers (OT), and professional stakeholders in an evidence-base

Does community residence mean more community contact for people with severe, long-term psychiatric disabilities?

The focus of the debate over hospital versus community care for those with severe and enduring mental health problems has shifted somewhat towards ensuring the quality of care provided by services regardless of their location. One factor that is generally assumed to be an important indicator of quality of care is the opportunity for community contact that users of a residential service have. This is illustrated by its inclusion in most instruments designed to assess quality of care in psychiatric services. The present study set out to examine the amount of community contact made by residents o

Nursing home case-mix classification and residents suffering from cognitive impairment: RUG-II and cognition in the Texas case-mix data base.

Case-mix reimbursement systems for nursing home care are now in use or under consideration in over 20 states. Though many in the long-term care field still debate the relative merits of the various systems, those systems based on the Resource Utilization Groups-II (RUG-II) system used in New York seem to be increasingly popular. However, one of the more serious questions raised concerning RUG-like systems is whether they fully acknowledge the role played by cognitive deficits in determining nursing home residents' care needs. The results of this research indicate: 1) that resource provision an

Preserving integrity: experiences of people with mental health problems living in their own home in a new neighbourhood.

For patients with mental health problems, de-institutionalization has meant a shift from institutional care to living in the community. However, several studies show that problems of stigmatization, loneliness and negative attitudes devalue the dignity and autonomy of these patients. The aim of this study was to gain a deeper understanding of how people with mental health problems experience living in an apartment of their own. The data collection method was focus group interviews. The constant comparative method revealed the main category 'preserving integrity'. The subcategories were: the ne

Activities of daily living in nursing home and home care settings: a retrospective 1-year cohort study.

OBJECTIVE: Korea introduced universal long-term care insurance (LTCI) for physically dependent older adults in 2008. Older adults, their family members, and policy makers in Korea want to know patient outcomes in different care modalities because older adults who have a similar functional status and LTC needs can choose either nursing home care or home care. The aim of this study was to compare activities of daily living (ADLs) in nursing home care and home care settings for physically dependent older adults in Korea. DESIGN: A retrospective 1-year cohort study using national LTCI data. SETT

A randomized controlled trial of an ambulatory approach versus the hospital-based approach in managing suspected obstructive sleep apnea syndrome.

Comparisons of home-based versus hospital-based approach in managing patients with suspected obstructive sleep apnoea syndrome(OSAS). A prospective, controlled CPAP parallel study of new referrals with suspected OSAS randomized into group A) home-based or B) hospital-based approach. Following detection of AHI ≥ 15/hr by Embletta sleep study (group A) or polysomnography (group B), patients received CPAP for 3 months after an overnight autoCPAP titration at home or in hospital respectively. Modified intention-to-treat analysis of those with AHI ≥ 15/hr on CPAP (n = 86 vs 86) showed no difference
