Public Health Communication Challenges in Eastern Europe and Central Asia: A Scoping Review.

This scoping review examines public health communication across nine Eastern European and Central Asian states-Armenia, Azerbaijan, Belarus, Kazakhstan, Kyrgyzstan, Russia, Tajikistan, Turkmenistan, and Uzbekistan-highlighting how these systems have transitioned from Soviet-era legacies to contemporary practices. Eligibility criteria included the English- and Russian-language literature published from 1998 onwards, focusing on nine post-Soviet states. Sources of evidence comprised searches in Google Scholar, ScienceDirect, SSRN, Heliyon, MEDLINE/PubMed, and official government websites. Data w

Integrating data from legacy systems using object linking and embedding technology: development of a reporting system for heavy metal poisoning results.

Integrating data that reside in different systems remains an often laborious process, requiring either manual steps or complicated programming. This paper describes a method for state-mandated reporting of childhood blood lead testing results that makes use of object linking and embedding technology and readily available software products to pull together information from different legacy systems. A terminal session emulator employs object linking and embedding automation to extract host data, and Visual Basic routines specify the user interface and database manipulation. This system has signi

Rethinking the frequency of between-visit monitoring for patients with diabetes.

BACKGROUND: Health systems increasingly look to mobile health tools to monitor patients cost-effectively between visits. The frequency of assessment services such as interactive voice response (IVR) calls is typically arbitrary, and no approaches have been proposed to tailor assessment schedules based on evidence regarding which measures actually provide new information about patients' status. METHODS: We analyzed longitudinal data from over 5000 weekly IVR monitoring calls to 298 diabetes patients using logistic models to determine the predictability of IVR-reported physiological results, pe

Pediatric post-marketing safety systems in North America: assessment of the current status.

PURPOSE: It is critical to have pediatric post-marketing safety systems that contain enough clinical and epidemiological detail to draw regulatory, public health, and clinical conclusions. The pediatric safety surveillance workshop (PSSW), coordinated by the Food and Drug Administration (FDA), identified these pediatric systems as of 2010. This manuscript aims to update the information from the PSSW and look critically at the systems currently in use. METHODS: We reviewed North American pediatric post-marketing safety systems such as databases, networks, and research consortiums found in peer

The neural systems of emotion regulation and abnormalities in major depressive disorder.

Major depressive disorder (MDD) is a mental disorder characterized by aberrant emotion regulation. The capacity for emotion regulation stems from diverse neural circuits including higher level cognitive structures involved in processing contextual information, and lower level limbic structures involved in triggering emotional expression. Cognitive theories of depression posit that the MDD-specific abnormalities in emotional control derive itself from dysfunctional cognitive processes including biased attention, rumination, and altered information processing and memory. The main objectives of t

The role of hospital infection control in the quality system of hospitals.

Like other highly specialized fields, quality systems have their own vocabulary which we must be familiar with; it has been internationally standardized. This standard should be adhered to in order to avoid unnecessary ambiguities and confusion, and to facilitate exchange of information between disciplines. We, in the infection control field, are quality pioneers in hospitals. We have, within our discipline, created quality systems and practised quality surveillance for decades. This must be recognized. Medical quality audits intended for comparisons between hospitals, services and wards requi

Data interoperability for a systems approach to developmental conditions.

The rising prevalence of developmental conditions such as autism, ADHD, and learning disorders underscores the urgent need for an integrated approach to account for the dynamic interaction between genetic, environmental, and societal factors. At present, the information on these factors is scattered and fragmented across a wide range of caregivers using different systems, structures and semantics. As a result, healthcare providers often face excessive administrative burdens to gather relevant information from disparate sources, and in many cases, must repeat diagnostic tests due to a lack of a

Effect of distance to health facility on the maintenance of INR therapeutic ranges in rheumatic heart disease patients from Cape Town: no evidence for an association.

BACKGROUND: Lack of adherence to international normalised ratio (INR) monitoring in rheumatic heart disease (RHD) patients is a contributor to cardio-embolic complications. This population-based observational study investigated whether the distance between home and an INR clinic affects the maintenance of therapeutic INR in RHD patients on warfarin. METHODS: Residential addresses, INR clinics, and INR results of patients with RHD were extracted from the Cape Town component of the Global Rheumatic Heart Disease Registry (REMEDY) database. Addresses of homes and INR clinics were converted to ge

Overview of occupational epidemiology: issues/dilemmas.

Occupational epidemiology is an essential discipline in the practice of occupational medicine. Many issues/dilemmas that plague the success … been discussed. The current approach to solving these many issues/dilemmas is by the development of health information systems that prospectively gather employee medical surveillance data, tracking of employee work history and exposure monitoring data and have the built-in ability to relate these data for health studies. Care needs to be taken to assure that comparable data …

Community perspectives on HIV, violence and health surveillance in rural South Africa: a participatory pilot study.

BACKGROUND: South Africa faces a complex burden of disease consisting of infectious and non-communicable conditions, injury and interpersonal violence, and maternal and child mortality. Inequalities in income and opportunity push disease burdens towards vulnerable populations, a situation to which the health system struggles to respond. There is an urgent need for health planning to account for the needs of marginalized groups in this context. The study objectives were to develop a process to elicit the perspectives of local communities in the established Agincourt health and socio-demographic

What do radiology incident reports reveal about in-hospital communication processes and the use of health information technology?

BACKGROUND: There has been recent rapid growth in the use of medical imaging leading to concerns about an increase in unnecessary investigations, patient exposure to radiation, and incorrect diagnoses. Incident reporting systems provide a portal for staff to catalogue adverse events which occur within a hospital or department. Analysing incident reports can reveal trends and provide guidance for quality improvement efforts. METHODS: Classification of medical imaging related-incidents from a major teaching hospital in Sydney, Australia using WHO International Classification for Patient Safety

Using automated medical records for rapid identification of illness syndromes (syndromic surveillance): the example of lower respiratory infection.

BACKGROUND: Gaps in disease surveillance capacity, particularly for emerging infections and bioterrorist attack, highlight a need for efficient, real time identification of diseases. METHODS: We studied automated records from 1996 through 1999 of approximately 250,000 health plan members in greater Boston. RESULTS: We identified 152,435 lower respiratory infection illness visits, comprising 106,670 episodes during 1,143,208 person-years. Three diagnoses, cough (ICD9CM 786.2), pneumonia not otherwise specified (ICD9CM 486) and acute bronchitis (ICD9CM 466.0) accounted for 91% of these visits,

Towards gender equality in health in Afghanistan.

The Afghanistan gender inequality index shows that 70% loss in development is due to the limited participation of women in the workforce, low education and poor women's health outcomes. However, since the fall of the Taliban regime in 2002, gender inequalities in health have improved. This paper will review factors that led to these improvements. The review draws upon information from various sources, including formative and applied research, surveys and existing information systems. The review showed gender differentials in morbidity, mortality and accessing and utilising health services. Hea

System-Wide Implementation of Routine Outcome Monitoring and Measurement Feedback System in a National Network of Operational Stress Injury Clinics.

This manuscript reviews the development and implementation process of the Client Reported Outcome Monitoring Information System in the VA Canada Operational Stress Injury National Network, and reports on outcomes of an evaluation to assess usage, barriers, and facilitators to implementation. The majority of clinicians reported regular use of routine outcomes monitoring, although objective data suggested somewhat lower actual use. In general, clinicians endorsed all barriers and most facilitators as influencing their use of routine outcomes monitoring in a minor way. However, users and non-user

DDInter: an online drug-drug interaction database towards improving clinical decision-making and patient safety.

Drug-drug interaction (DDI) can trigger many adverse effects in patients and has emerged as a threat to medicine and public health. Despite the continuous information accumulation of clinically significant DDIs, there are few open-access knowledge systems dedicated to the curation of DDI associations. To facilitate the clinicians to screen for dangerous drug combinations and improve health systems, we present DDInter, a curated DDI database with comprehensive data, practical medication guidance, intuitive function interface, and powerful visualization to the scientific community. Currently, DD

Obesity hypertension: the emerging role of leptin in renal and cardiovascular dyshomeostasis.

PURPOSE OF REVIEW: Adipose tissue is now considered to be an active physiologic system operating in concert with multiple other organs. Leptin is a peptide hormone that is primarily synthesized and secreted by adipose tissue whose principal action is the control of appetite and energy balance. However, current information suggests that leptin exerts pleiotropic effects on several organ systems. Herein, we review the potential role of leptin in cardiovascular and renal physiological conditions as well as pathophysiological situations including obesity and hypertension. RECENT FINDINGS: Increas

Avoiding "shiny objects" and heeding "red flags": a sensemaking framework for emerging technology procurement.

OBJECTIVES: Health systems can be vulnerable to "shiny object syndrome," adopting unneeded emerging technologies (ETs), or may ignore "red flags" if they rely on linear health technology procurement processes that exclude the influence of information exchanges between the health system, vendors, and industry partners. METHODS: We conduct a post-hoc comparison of 2 procured communication-related clinical workflow ETs and examine ET-related IT Committee meetings within an academic medical center. We apply sensemaking theory, highlighting examples where sense-giving, sense-demanding, and sense-b
