Real-time 3-dimensional echocardiography in assessing atrial and ventricular septal defects: an echocardiographic-surgical correlative study.

OBJECTIVE: The aim of this study is to explore the feasibility and the value of real-time 3-dimensional echocardiography (RT3DE) in quantitative evaluation of the size of atrial septal defect (ASD) and ventricular septal defect (VSD) and to correlate with the surgical findings. METHODS: Thirty eight patients with ASD and/or VSD were examined with RT3DE. Three-dimensional image data-base was post-processed using 3D work-station. The results were compared with the results measured by 2-dimensional echocardiography and surgical findings. RESULTS: RT3DE produced novel views of congenital septal

The Gerbode defect - a frequent echocardiographic pitfall.

We present a case of Gerbode type defect (left ventricular to right atrial communication) discovered in a 52 year-old man with atrial and ventricular septal defects. The patient was diagnosed using two-dimensional colour Doppler and transoesophageal echocardiography. We describe the echocardiographic features and review the anatomical consequences of such defects. Our aim was to remind readers about this rare organic heart disease that an inexperienced echocardiography specialist might easily mistake for a recoil wave of tricuspid valve incompetence and thus diagnose pulmonary hypertension.

Sound envelope averaging and the differential diagnosis of systolic murmurs.

Phonocardiography previously has been limited in scope because the attained suboptimal signal-to-noise ratio has interfered with the sensitivity and clarity of recordings. A new system is described that reduces this problem by utilizing demodulation and synchronous averaging of the "envelop" of cardiac sounds. A limited survey of the differential diagnostic capabilities of this technique is presented for 80 patients having one of six common forms of pathological systolic murmurs and an 89 per cent diagnostic accuracy is demonstrated. This system promises to be a valuable noninvasive adjunct in

Secundum atrial septal defect before and after surgery.

Twenty patients with secundum atrial septal defect, who had undergone open heart surgery were studied retrospectively. Girls were more affected than boys; the sex ratio was 1.5:1. Associated cardiac defects were diagnosed in two patients, one with moderate valvular pulmonic stenosis and the other one with small ventricular septal defect. Typical clinical findings consisted of loud first heart sound, widely fixed split second heart sound and soft ejection systolic murmur at the upper left sternal border were heard in all cases. Mid diastolic murmur due to relative tricuspid stenosis was detecte

Rare case of mesocardia with ostium secundum ASD and double IVC.

We report an extremely rare case of mesocardia with double inferior vena cava (IVC) in this case report. The patient's associated atrial septal defect (ASD) with left to right shunt leading to pulmonary congestion and recurrent episodes of pneumonia was the leading diagnosis for which care was sought at our hospital. The diagnoses of these rare congenital cardiac anomalies were uncovered only after a thorough workup. Mesocardia is extremely rare and accounts for only 0.2% of congenital anomalies. Its association with other cardiac and vascular anomalies, however, warrants a meticulous workup f

[Study of cor triatriatum. Apropos of 6 cases].

The authors report 6 cases of triatrial heart with various and non-specific manifestations and attribute the difficulty of its clinical identification above all to the frequency of associated lesions, which were present in 5 of the 6 cases reported here. An atrial septal defect and left superior vena cava were each seen twice while a ventricular septal defect, sub-valvular aortic stenosis, mitral insufficiency and transposition of the great vessels were each seen once. It was routine echocardiography which led to the discovery of triatrial heart in each of these 6 cases. The value of this inve

Three-dimensional echocardiographic assessment of atrial septal defects.

Echocardiography provides a useful tool in the diagnosis of many congenital heart diseases, including atrial septal defects, and aids in further delineating treatment options. Although two-dimensional echocardiography has been the standard of care in this regard, technological advancements have made three-dimensional echocardiography possible, and the images obtained in this new imaging modality are able to accurately portray the morphology, location, dimensions, and dynamic changes of defects and many other heart structures during the cardiac cycle.

Correction of corrected transposition of the great arteries associated with cardiac anomalies.

Reported in this paper are 8 cases of corrected transposition of the great arteries associated with anomalies. All the patients underwent the repair of the ventricular septal defect and atrial septal defect, the replacement of left-side tricuspid value, the pulmonary valvotomy and the placement of extra-cardiac conduit to the pulmonary trunk respectively with no hospital death after operation. The paper also described the selection of the heart incision, the prevention of injury of the conduction system and indication of the placement of extra-cardiac conduit.

Atrioventricular septal defect with intact atrial and ventricular septal structures.

A heart with an atrioventricular septal defect is characterized by absence of the atrioventricular muscular and membranous septa, a common atrioventricular junction, an unwedged position of the aortic valve annulus and disproportionate inlet--outlet dimensions of the ventricular septum. The clinical and post-mortem findings are described of a case which had intact atrial and ventricular septa but had all the other anatomical hallmarks of atrioventricular septal defect. The problems in clinical diagnosis may be overcome by cross-sectional echocardiography.

Variation in severity of cardiac disease in Holt-Oram syndrome.

We describe a family with Holt-Oram syndrome (HOS) with variable hand and cardiac manifestations. One affected relative had complex congenital malformations of the heart consisting of an endocardial cushion defect and hypoplasia of the left ventricle. The literature from 1974 to 1995 is reviewed. Atrial septal defect is the most cardiac abnormality (60.3% of 189 cases) occurring singly or in combination with other malformations. Thirty-three individuals (17.5%) of literature cases) have more complex congenital malformations of the heart requiring complicated medical management and extensive ca

[Surgical treatment of congenital heart defects with pulmonary hypertension in children during their 1st year of life under the conditions of extracorporeal circulation].

In the period between 1983 and 1987 operations were carried out on 204 children aged from 25 days to 12 months (body weight of 3 to 10 kg) for congenital heart diseases and pulmonary hypertension. Surgery for ventricular septal defect was undertaken on 173 patients (total mortality 6.9%), transposition of the major vessels with concurrent ventricular septal defect was corrected by Senning's method in 9 patients (mortality 11%), total anomalous pulmonary vein drainage was relieved in 14 patients (mortality 28.6%). No fatal outcomes occurred in correction of an aorto-pulmonary fistula, origin of

Subxiphoid cross-sectional echocardiography in infants and children with congenital heart disease.

We developed anatomically related cross-sectional echocardiographic views to image the heart from the subxiphoid area in 100 children and infants with various forms of congenital heart disease studied prospectively before cardiac catheterization. Wide-angle cross-sectional views were achieved using a mechanical sector scanner in a scan plane oriented parallel to a line between the patient's shoulders (a coronal plane) showing the equivalent anatomy of an anteroposterior angiogram. The subxiphoid technique appeared to be better than chest wall-based imaging in demonstrating obstructive lesions

Repair of complete atrioventricular septal defects: results with maintenance of the coronary sinus on the right atrial side.

BACKGROUND: This study was undertaken to determine that maintaining coronary sinus on the right atrial side during the surgical repair of complete atrioventricular septal defect (AVSD) does not increase the risk of postoperative complete heart block. METHODS: This is a retrospective study of 51 consecutive patients who underwent biventricular repair of complete AVSD from September 2000 to January 2005. Electrocardiograms and operative data were analyzed. RESULTS: The mean age was 13.3 months (4 to 60). In all the 51 patients, except 13 cases, repair was performed using the two-patch techniqu
