Bio-Inspired In Situ Injectable Hemostatic Agent with Covalent-Ionic Hybrid Network for Non-Compressible Hemorrhage.

Non-compressible hemorrhage is an arduous clinical challenge that primarily occurs at thoracic/abdominal solid organ injuries, deep arterial ruptures, and major vascular trauma, and cannot be stopped by compressing the hemorrhage site. Currently available commercial hemostatic materials for non-compressible hemorrhage are plagued by issues such as poor sealing ability and insufficient hemostatic performance. In the current work, tetra-PEG-succinimidyl glutarate (TPEG-SG), carboxymethyl chitosan (CMCS), and the coagulation factor Ca2 + were used as the primary ingredients to create a covalent-i

Role of snake venom proteases and tissue factor in venom induced hemostatic disorders and local hemorrhage in mice.

Bothrops jararaca venom (BjV) induces hemostatic and inflammatory manifestations. Tissue factor (TF) is essential for hemostasis, but aberrant TF expression can cause coagulopathy. We determined the effect of inhibition of BjV serine proteases and metalloproteinases, as well as host TF, on the systemic coagulopathy and local hemorrhage in envenomed mice. Venom serine proteases induced hypofibrinogenemia whereas venom metalloproteinases induced hemorrhage. TF did not contribute to venom-induced coagulopathy, but played a protective role in venom-induced bleeding in mice.

Oleanolic acid, a herb extract, acts as a strong and safe enhancer in the collagen-mediated hemostatic cascade in flow.

Considerable attention has been paid steadily for efficacy and safety of herb extracts with medicinal benefits, recently. Tranexamic acid (TXA) is a well-known hemostatic agent with strong efficacy but potential thrombus risk. Here, we identified oleanolic acid (OA) as a strong enhancer in promoting adhesion, spreading or aggregation of platelets on immobilized collagen, which exceeds TXA among six herb extracts. In addition, the study demonstrated that TXA could facilitate morphological changes of platelets on collagen, accelerate and enhance intracellular calcium bursting with and without me

Optimized Intraoperative Monitoring With Real-Time Hemostatic Guidance: A Key Component in Managing Catastrophic Obstetric Hemorrhage.

Massive obstetric hemorrhage is a leading cause of maternal morbidity, especially in placenta accreta spectrum (PAS) cases. We report the anesthetic management of a 38-year-old woman undergoing emergency cesarean section complicated by hemorrhage exceeding 13 liters. Preoperative planning included risk assessment, blood product preparation, and a multidisciplinary approach. Intraoperatively, anesthesia was managed with combined neuraxial and general techniques, continuous hemodynamic monitoring, and a guided massive transfusion protocol. Despite the need for hysterectomy and complex surgical i

Influence of Hemostatic Disorder on Type II Endoleak Development After Endovascular Abdominal Aortic Aneurysm Repair.

Endovascular aneurysm repair (EVAR) is a widely used minimally invasive treatment for abdominal aortic aneurysms. However, postoperative type II endoleak (T2EL) remains a relevant complication associated with a risk of aneurysm rupture and the need for repeated imaging follow-up, resulting in exposure to ionizing radiation. Identification of biological factors predisposing to T2EL may improve risk stratification. This pilot study aimed to investigate whether disturbances in hemostasis are associated with early T2EL development after EVAR. A total of 103 patients treated with EVAR for symptomat

Dual-Functionality of Polysaccharide-based Hemostatic Powder: Effective Bleeding Control and Adhesion Reduction in Surgical Models.

Postoperative adhesions are a frequent and serious complication of abdominal and pelvic surgeries, leading to chronic pain, infertility, and intestinal obstruction, with significant clinical, and economic burdens. While barrier agents are widely used for prevention, current solutions often suffer from limited efficacy and short retention times. This study evaluates the dual-functionality of a polysaccharide-based medical device, 4SEAL Hemostatic Powder, by evaluating hemostatic performance and the reduction of postoperative adhesions. Using rat models, the hemostatic efficacy of 4SEAL was asse

Endothelial Dysfunction and Hemostatic System Activation in Relation to Shift Workers, Social Jetlag, and Chronotype in Female Nurses.

Circadian misalignment, due to shiftwork and/or individual chronotype and/or social jetlag (SJL), quantified as the difference between internal and social timing, may contribute to cardiovascular disease. Markers of endothelial dysfunction and activation of the coagulation system may predict cardiovascular pathology. The present study aim was to investigate the effects of shift work, SJL, and chronotype on endothelial function and coagulation parameters. One hundred female nurses underwent endothelial function testing using the EndoPAT and blood sampling for coagulation markers, repeated at 06

Hemostatic Cellulose Masquerading as a Postoperative Abscess Following Elective Colon Surgery: A Unique Case and Literature Review.

Surgical site infections (SSIs) and intra-abdominal abscesses (IAAs) are common and costly complications following elective colon surgery, requiring prompt diagnosis and treatment. Hemostatic cellulose, a widely used intraoperative hemostatic agent, can mimic pathological lesions such as abscesses on imaging, leading to potential misdiagnoses. We present the case of a 68-year-old man who underwent elective colostomy reversal complicated by a postoperative computed tomography (CT) scan suggestive of an IAA. Upon review of the operative note and repeat imaging, the suspected abscess was determin

Electrospun amine-modified polysuccinimide/polycaprolactone nanofiber hydrogel dressings as antibacterial and hemostatic wound dressing applications.

Design and construction of novel dressings with appropriate structure and multiple bio-functions are urgently required for the wound treatment applications. In this study, a series of wound dressings composed of amine-modified polysuccinimide (PSI)/polycaprolactone (PCL) hydrogel nanofibers were fabricated by using the conjugated electrospinning and the chemical crosslinking post-treatment. All the dressings with different PSI/PCL ratios could effectively maintain the fibrous morphology even after the chemical crosslinking process. The ultimate stress and Young's modulus of the PSI-NH2/PCL dre

Checkpoint Inhibitors, CAR T Cells, and the Hemostatic System: What Do We Know So Far?

Immune checkpoint inhibitors (ICIs) and chimeric antigen receptor (CAR) T cells are novel therapeutic strategies that enhance anticancer immunity by activating or engineering cancer-targeting T cells. The resulting hyperinflammation carries several side effects, ranging from autoimmune-like symptoms to cytokine release syndrome (CRS), with potentially severe consequences. Recent findings indicate that ICIs increase the risk of venous and arterial thromboembolic adverse events. Patients with prior VTE might be at higher risk of developing new events under ICI while other risk factors vary acros

An investigational time-course study using an in vivo ovine laminectomy model for the neurohistopathological evaluation of hemostatic agents.

BACKGROUND: Preclinical in vivo analysis of hemostatic agents is a fundamental requirement in the assessment of implant safety and efficacy prior to utilization in the clinical operative setting. The purpose of this study served to investigate the hemostatic, peri-operative outcomes and histopathologic responses following epidural application of a novel hemostatic agent in an ovine lumbar laminectomy model. Despite routine utilization of hemostatic agents, the potential for inflammation and compression of neural structures, fibrosis, and neurotoxicity remains a clinical concern. Epidural fibro

An Antibacterial and Anti-Inflammatory Hemostatic Adhesive with Dual Zn2+ and NO Release for the Neurovascular Electrostimulation of Infected Diabetic Wounds.

Diabetic wounds are difficult to heal because chronic infection, reactive oxygen species (ROS) in excess, and persistent inflammation repeatedly damage skin structures including blood vessels and nerves. Efficacious dressings developed for treating diabetic wounds should address these issues concurrently. Here, a multifunctional hydrogel adhesive system made by photocrosslinking arginine-functionalized methacrylated chitosan with gallic-acid-functionalized gelatin methacryloyl is reported. The resulting scaffold (ACGG) is loaded with a laponite delivery system gorged with Zn2+ (Zn@LAP; ZL). Be
