The presence and roles of nurse navigators in acute care hospitals.

OBJECTIVE: A survey of hospital-based nurse executives was conducted to determine the extent, approaches, and outcomes of nurse navigator (NN) programs. BACKGROUND: Nurse navigators are distinct from other recognized healthcare roles. Navigators most commonly focus on a single health condition with the goal of improving the provision of specified health services for an individual patient. METHODS: An 11-question Internet-based survey was e-mailed to 580 nurse executives in Texas. RESULTS: Of the respondents, only 24% implemented any type of NN program. Most of the respondents with navigator

Ambiguity and workarounds as contributors to medical error.

Why are some organizations error-prone-regularly subject to interruptions and inconveniences, some of which periodically coalesce catastrophically-whereas other organizations, although similar in the products and services they generate and the process technologies they use, are reliable, adaptable, and continuously self-improving, relentlessly learning from experience to get ever better? Analyzing medical error reports and studies of high-performing, non-health care organizations reveals 2 differences. High performers know how to prevent problems from producing further consequences once they o

Acute life-threatening asthma.

Although adrenergic therapy may be lifesaving in patients with severe acute asthma, attention must also be directed toward reversing underlying inflammation with corticosteroids. Other therapy for acute severe bronchospasm is controversial, although inhalant anticholinergic therapy may have adjunctive benefit. Aminophylline or magnesium sulfate may be indicated in those patients with life-threatening asthma who do not respond to conventional therapy. Chest radiographs are needed in patients who (1) present in extreme distress, (2) need to be hospitalized, or (3) have clinical manifestations of

The patient perspective on use of Omalizumab in the in-hospital setting.

BACKGROUND: Omalizumab is approved for the treatment of chronic spontaneous urticaria (CSU), allergic asthma, and chronic rhinosinusitis with nasal polyps. While self-administration is licensed in Ireland, reimbursement restrictions require hospital-based delivery, placing significant burdens on patients and healthcare services. AIMS: To evaluate patient perspectives on hospital-based administration of Omalizumab, and assess the practical, financial, and environmental implications of current practices, alongside interest in transitioning to home-based therapy. METHODS: A cross-sectional surv

How leadership attention is associated with online-offline healthcare service synergy: differentiated effects of noticing, interpreting, and acting.

AIM: This study examines how three types of leadership attention (noticing, interpreting, acting) are associated with online-offline healthcare service synergy (process and organizational synergy), along with motivational antecedents (credit claiming, blame avoidance) and contextual boundary conditions, using the APCC framework in the Chinese context. METHODS: An exploratory sequential design was adopted. During the qualitative phase, data were triangulated from 41 interviews (hospital administrators, health bureau officials, clinicians), "Internet + Healthcare" policy documents, and literatu…

Co-creating patient safety education: Bridging learning, practice, and policy in emergency care.

BACKGROUND: Pre-analytical errors persist as the leading cause of laboratory errors despite technological advances, reflecting translational failures in continuing professional development (CPD). Traditional patient safety education, such as didactic lectures and online learning modules, achieves only temporary improvements, with error rates returning to baseline within months. Co-creation, where frontline professionals function as equal partners in educational design, offers a fundamentally distinct approach, yet evidence regarding operationalization, sustained clinical outcomes, and departme
